The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The residents live in a house that is clean, comfortable, homely and well maintained. They all have their own rooms, which have been personalised to suit their likes and dislikes. Staff and residents get on well with each other and there is a very relaxed atmosphere at the home. One of the staff said there is a lot of laughter at the home and another said that the residents are brilliant. All of the residents attend day services for four days each week and when at home they go out to places of their choice with staff.

What has improved since the last inspection?

The home has been completely redecorated inside and outside and new net curtains have been bought. The home looks very nice. Difficulties within the management and staff team have been sorted out and staff feel that communication and teamwork have improved.

What the care home could do better:

The manager and staff team continue to work to provide a good service for the residents and to meet each person`s needs. The requirements in the previous inspection have been met and there are no requirements from this visit.

There is easy access to public transport, shops and leisure activities. Ground floor accommodation comprises a lounge, kitchen/dining area, laundry area, toilet, shower and two bedrooms. There are three bedrooms, a bathroom with toilet, a separate toilet and a small office/sleep in room. Residents’ bedrooms are individually decorated and furnished and reflect their interests. There is also a small well-maintained garden with a large summerhouse. This is heated, comfortably furnished and has a TV and stereo. Five adults with learning difficulties live at the home and they need assistance and support from the staff team. Residents have access to day centres and other leisure pursuits. This unannounced inspection lasted for about three hours and took place during the afternoon. It was the second of the two inspections that each home must have during the inspection year. During the two visits all of the key standards have been checked. The manager, staff and residents were spoken to. All of the communal rooms and two of the bedrooms were seen. Care and other records were checked. The progress of the requirements from the previous inspection was monitored. Feedback forms were left for staff that were not on duty at the time of the inspection to give their comments on the service. What the service does well: What has improved since the last inspection? Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 If a vacancy arose the required information would be gathered on a prospective resident and they and their relatives could spend time in the home to find out what it would be like to live there and to enable the resident to make a choice about living in the home, within their capacity to do so. EVIDENCE: The residents have all lived together for several years and the newest residents moved in more than seven years ago. It is therefore not possible to directly test Standard 2 with regard to prospective users needs being assessed. However, the organisation has an admissions procedure that includes gathering of information and assessments. It also contains details of how a prospective resident would be introduced to the home. The staff would be able to assess and introduce a new resident to the home if needed. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): These standards were not tested on this visit. However evidence from the last inspection was that residents’ care plans reflect their needs and choices in a format they understand. Residents make decisions about their lives and participate in all aspects of life at the home. Also that residents are supported to take risks and to be as independent as possible. EVIDENCE: The above standards were not specifically tested on this visit, as there were no outstanding requirements in relation to the five standards. At the time of the last inspection standards six, seven, eight & nine were tested and assessed as met. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13, 14, 15 & 16 The residents are encouraged to be as independent as possible, to take part in activities and to be part of the local community. EVIDENCE: All the residents attend day services for four days per week. Each person has a different day off each week and during that time they get individual support from the staff team. On their days off they all like to do different things. One resident likes to go out for lunch and by his newspaper. The care plan of another says that he likes sailing and enjoys trips to the pub. One of the residents goes to church and also to a club in Dagenham. All of the residents seem to be relaxed and very much “at home”. The residents said they were happy and liked living at the home. The residents are encouraged to help around the home. One of the residents in particular likes to do housework and mops, does some hoovering and empties the bins. Others need a bit more encouragement. Daily logs record what residents have done and the choices that they have made. For example one log said that the resident had chosen what clothes to wear and what to Kingswood Road (47) DS0000025907.V284492.R01.S.doc Version 5.1 Page 11 have for breakfast. Another said that the resident had put crockery in the sink and helped to make lunch. Some of the residents have contact with their families. One of them stays with his parents for weekends and another phones her sister regularly. Relatives are invited to reviews but in most cases declined to attend but asked for notes from the meeting. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 & 20 Residents receive personal care that meets their individual needs and preferences. The staff team support the residents to get the healthcare that they need. Medication is appropriately administered by staff that have been trained to do this. EVIDENCE: The residents all need some support with their personal care. Details of the help that they need and how they prefer to be supported are in their individual plan. For example the care plan and of one resident says the staff must avoid scented soaps and shampoo, and must not use aftershave directly onto his skin, as this will irritate his psoriasis. All of the residents go to the local doctor and specialist help is received when needed. Staff take residents to all of their medical appointments. Residents’ files have details of health care issues and show that residents have regular access to health care professionals. Records are kept of medical appointments and these show that residents have checks from the optician, dentist and when needed the chiropodist. Medication is securely stored in a locked Cabinet in the office. Medication administration records are kept and are up-to-date. In addition of the medication file has photographs of each resident and information about the medication that they take and how it is taken. For example one resident needs lots of fluid to take their medication. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. JUDGEMENT – we looked at outcomes for the following standard(s): These standards were not tested on this visit. However evidence from the last inspection was that there is a complaints procedure that would be followed in the event of any complaints being made. Also that staff are aware of issues of abuse and work to protect residents from abuse. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 26, 27 28, 29 & 30 The residents live in a clean and comfortable home that is suitable for their needs. EVIDENCE: The home is in a quiet residential road in Seven Kings. There is easy access to public transport, shops and leisure activities. Ground floor accommodation comprises a lounge, kitchen/dining area, laundry area, toilet, shower and two bedrooms. Upstairs there are three bedrooms, a bathroom with toilet, a separate toilet and a small office/sleep in room. There are enough baths, showers and toilets and these meet the residents’ needs. Residents’ bedrooms are individually decorated and furnished and reflect their interests. There is also a small well-maintained garden with a large summerhouse. This is heated, comfortably furnished and has a TV and stereo. Since the last inspection the house has been redecorated both inside and outside. In addition new net curtains have been purchased. Therefore the house was well decorated throughout and was very comfortable and homely. None of the residents require any specific adaptations at this time. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34, 35 & 36 Staff are competent and receive the necessary training, support and supervision to carry out their duties. Staff have the experience and qualifications to meet the residents needs. Staff are properly recruited and the necessary checks carried out. This helps to protect residents and keep them safe. EVIDENCE: The staff team have experience of working with people with learning disabilities. All of the care staff have successfully completed NVQ level 2 and some of them have completed NVQ level 3. On the staff files were training records. Training covers a variety of areas including Moving and Handling, first aid, fire safety, health & safety, infection control, and food hygiene. Therefore competent and qualified staff support the residents. At the last inspection staff files seen contained Criminal Records Bureau checks and references. However one of the staff had worked for Mencap before they took a break in their career and it had not been felt necessary for two references to be taken. The previous inspection required that that two references must be taken up prior to staff commencing employment. A second reference was obtained from another Mencap home for the person concerned and staff files seen contained two references. The organisation operates an appropriate recruitment procedure that protects and safeguards residents. Staff meetings and staff supervision have been taking place regularly, providing staff with the opportunity to discuss problems and to be involved in the development of the service. Feedback from staff was that they receive a good level of support. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 38, 39, 41 and 42 The home is well managed and provides a safe environment for the residents. EVIDENCE: The manager has considerable experience of working with people with learning disabilities and of managing residential services. Feedback from staff was that they are well supported by the manager and that he is approachable. They also said that the conflict within the management and staff team, which was identified at the time of the previous inspection, had been resolved. Communication and teamwork were much better. The quality of the service provided to the residents is monitored by the home manager and by Mencap. The service manager carries out monthly monitoring visits to assess how effectively the home is operating to meet its stated aims and objectives, and reports from him indicate the action to be taken when deficiencies are identified.
