What the care home does well

This home is well run by an experienced nurse supported by a team of caring, kind and well-trained staff. Staff provide residents with a range of interesting activities each day. Care records are good and provide staff with enough information to meet the needs of residents effectively. The home is clean, comfortable and homely and is always welcoming to visitors. Food is nicely presented, tasty and well cooked.

What has improved since the last inspection?

The senior nurse has decided to apply to be the Registered Manager, which will provide the home with stability and leadership. One of the bedrooms has been redecorated to provide users with a homely place to live. The Chef has had training in food hygiene to ensure safety in the kitchen.

What the care home could do better:

The ceiling in the sun lounge needs redecorating where it is water damaged.

The home is privately owned by the Trustworth Group Ltd and can accommodate 11 service users over the age of 65 years, 4 of whom may be mentally frail.The home is an adapted premises on 2 floors serviced by a passenger lift. The home has 4 shared rooms and 3 singles. There are parking facilities to the front of the home and a large garden laid mostly to lawn at the rear. This unannounced inspection took place between 09:50 and 13:20 on a weekday morning and afternoon. The inspector toured the home, checked a number of care and health & safety records and spoke to the Acting Manager and staff, 6 service users and 5 visitors. There were no requirements outstanding from previous inspections and only 1 new requirement was made as a result of this visit. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 10 Service users are provided with appropriate health and personal care in a manner, which maintains their right to dignity, privacy and autonomy and as defined in their care plan. EVIDENCE: Examination of two of the newly reviewed service user plans demonstrated that they included nursing tools to assess the risks associated with nutrition, manual handling, falls, skin integrity and use of cot sides. Where risk was identified a comprehensive care plan was in place. Appropriate aids and equipment to reduce the risks identified were being deployed appropriately throughout the home. Service users confirmed that the care they were receiving was appropriately meeting their needs and that they were regularly consulted on health issues and offered appropriate screening and preventative treatments. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 15 The routines in the home are designed flexibly around the needs of users and provide opportunities to engage in meaningful activities and pursuits. Service users are provided with a varied and nutritious menu. EVIDENCE: The inspector observed that the home runs flexibly around the needs of its users within a semi-structured framework. Meals tend to be at set times but can be adjusted to meet the needs of the service and individuals. Service users confirm that they may rise and retire at a time of their choosing and are offered appropriate choices in relation to everyday life. Service users are provided with the opportunity to engage in a range of activities that provide mental and physical stimulation. The ‘music man’ is a particular favourite of residents. He visits fortnightly and provides musical entertainments. Staff engage users in card games, quizzes, puzzles, ball games, dominoes, ‘frustration’, reality orientation and passive exercises on a daily basis. Service users confirmed that they were rarely bored as staff spent time each day talking to them and encouraging them to participate in LANGLEY NURSING HOME H51-H01 11000 Langley N H V234584 250805 Stage 4.doc Version 1.30 Page 12 activities. A number of the users are big readers. The mobile library visits regularly and provides users with a range of large print reading materials. Discussion with the chef evidenced that users are regularly consulted about the menu and offered alternatives to the main menu at each mealtime. At the time of inspection users were provided with chicken burger casserole, green beans, carrots and mashed potatoes followed by tapioca. Tea was ‘bubble & squeak’ followed by bananas and custard. Service users said that they enjoyed the meals, which were tasty, plentiful and well cooked. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. JUDGEMENT – we looked at outcomes for standard(s) 16 Service users and their relatives confirm that their views and complaints are taken seriously by the homes management and are investigated fully with written outcomes provided to complainants. EVIDENCE: Examination of documentation evidenced that there have been no complaints or concerns raised by users or their relatives friends and advocates since the last inspection. Service users and visitors told the inspector that they rarely had any concerns but felt confident that they would be listened to and their concern would be dealt with quickly and effectively. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 26 The standard of the environment within this home is good providing service users with a clean, comfortable, attractive and homely place to live. EVIDENCE: A tour of the home evidenced that the accommodation was clean, safe and well maintained. All communal space was well used and bedrooms were light airy and comfortable. Service users confirmed that they were able to personalise their bedrooms and could bring with them small items of furniture and favourite possessions when admitted. There is a need to redecorate the ceiling in the sun lounge where it has been recently water damaged to enhance the appearance for users. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 Staff individually and collectively were able to demonstrate that they have the necessary skills and experience to effectively meet the needs of service users in their care. EVIDENCE: Examination of the rosters demonstrated that the home has a full complement of permanent staff. Agency workers are never used at the home. At the time of inspection there were sufficient staff on duty to meet the needs of users. The staff team was made up of a Registered Nurse (Acting Manager), two carers a chef and domestic assistant. The inspector had the opportunity to speak with six users and five relatives or friends that were visiting at the time of inspection. All were highly complimentary about the quality and caring attitude of staff. They confirmed that staff were “kind, caring and committed” and “went that extra mile to provide high quality care to the residents”. Staff were observed to provide personal care in a caring, discreet and sensitive way, which afforded users dignity and respect. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38 Service users live in a well maintained home where risks to their safety are assessed, minimised and managed effectively. EVIDENCE: Examination of a number of health & safety records evidenced that all necessary checks and maintenance of equipment is routinely carried out to safeguard the health & safety of users. The Environmental Health Officers recommendations from their visit dated 22nd June 2005 have been carried out in full and a new extinguisher has been provided on the advice of the Fire Safety Inspecting Officer. Unnecessary risks to users safety are identified using a range of clinical tools and risk assessments. The identified risks are reduced or eliminated by effective risk management procedures.
