Conditions of registration: Date of last inspection 9th August 2005 Brief Description of the Service: Larkswood is a care home registered to accommodate up to eighteen residents aged sixty-five years and over. The home is a large detached property located in a residential area of Worthing about one mile from the town centre and seafront, close to local parks and gardens. Accommodation is provided on two floors with the facility of a passenger lift. Residents’ bedrooms are for single accommodation, with twelve having en-suite facilities. A lounge/dining area and a separate small lounge is provided. In addition there is a well-tendered garden. This is an overview of what the inspector found during the inspection. This was an unannounced inspection, the second of a minimum of two statutory visits that an inspector must make to each care home during the course of a year. The first inspection, which was announced, was undertaken earlier in the year. At this inspection the Inspector looked at standards that were not looked at during the previous inspection, as well as any outstanding issues from the last report or concerns raised about the home since the last inspection. The reader is advised to look at the reports of both inspections for a fuller picture of the home. The Manager was not present at the time of the inspection, however the person in charge, the Deputy Manager, very ably assisted the Inspector throughout the inspection. During this inspection the Inspector examined records of care being provided to the residents; as well as records of any accidents, issues, or complaints, to make sure that the residents at Larkswood were being taken care of. In addition, the Inspector talked to a visitor, shared a cup of tea with the residents and spoke to many of them about how they found living at Larkswood. The majority of comments made were very complementary with regard to the home, the food and the staff members. At this inspection Larkswood was audited against the National Minimum Standards for Older Persons. The vast majority of the elements in each of the standards assessed were met, however, although there was evidence that the majority of staff members have received the appropriate fire training, not all had and a requirement is made in this report for this matter to be addressed. A recommendation is also made for all staff members to have refresher training in the administration of medication. The Inspector would like to thank everyone who cooperated with her on the day of this inspection. What the service does well:
The residents, who the Inspector met on the day of inspection, spoke highly of the staff members, the food and their bedrooms. The building is maintained safely, and it was seen that parts of the home had recently been redecorated. The vast majority of the records required by the Inspector on the day of inspection, were found to be in order. A visitor to the home told the Inspector that she was made to feel very welcome when she visited the home and that the person she visited was very comfortable there. The Inspector was told that Larkswood does not use outside agency workers, thus ensuring that the residents always know their carers. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 and 4 Each resident has a written contract/ statement of terms and conditions with the home. Residents and their representatives know Larkswood will meet their needs. EVIDENCE: The Inspector was shown a copy of the home’s statement of the terms and conditions, which contains all the requirements of the National Minimum Standards. The person in charge assured the Inspector that all residents and their representatives receive a copy. Evidence from care plans and staff training records confirmed that Larkswood was able to meet the assessed needs of the residents accommodated there. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 9 The resident’s health, personal and social needs are set out in an individual plan of care and are reviewed regularly. Not all staff members are signing the record sheet each time they administer medication. EVIDENCE: Care plans of some residents were looked at during the course of the inspection, and it was noted that the care plans, including risk assessments, had been reviewed regularly. It was seen that there is lockable storage for medication. It was noted that some staff members had administered medicines but had not always signed to say that they had. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): These standards were not assessed on this occasion, however these standards were met at the last inspection. EVIDENCE: Residents told the Inspector they would be happy talking to the Manager or any staff member about any problems they might have. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20, 21, 23 and 24 Residents have access to safe and comfortable indoor and outdoor communal facilities. Residents live in safe comfortable surroundings with sufficient and suitable lavatories and washing facilities. Resident’s rooms suit their needs. EVIDENCE: The home was found to be clean and fresh, and to offer sufficient communal space to meet the standards, with separate sitting and dining areas. The Inspector was told previously of plans to build a conservatory, but there was no evidence of this at the time of this inspection. There is a large garden that is accessible to people in wheelchairs, or with other mobility problems. There are accessible toilets close to the communal sitting areas and the dining room. There was evidence that some decorating had taken place. On the day of inspection several doors were seen to be wedged open, the Deputy Manager was made aware of the safety implications surrounding this. JUDGEMENT – we looked at outcomes for the following standard(s): 28 Residents are in safe hands at all times EVIDENCE: The Inspector saw evidence of ongoing training by the staff at the home, and the Inspector was told that the Manager is very keen to ensure that the people who live at Larkswood are supported by staff members who have been trained to know what they are doing. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 34, 36 and 37 The home’s accounting and financial procedures, record keeping and policies and procedures safeguard residents. Staff members are appropriately supervised. EVIDENCE: It is apparent that the needs of the residents are uppermost at Larkswood and that the home ensures that resident’s rights and best interests are safeguarded at all times. The inspector saw that insurance cover is in place to meet the National Minimum Standards and there is a business and financial plan for the home, which would be made available upon request. Residents are encouraged to manage their own affairs with the help of relatives or advocates. Larkswood was seen to offer formal bi-monthly supervision to all care staff.
