What the care home does well

Lathbury Manor is homely and comfortable, providing adaptations and equipment necessary to support the Service Users in a safe and inviting environment. A suitably qualified and skilled Manager who ensures the day-to-day operation of the Home is in the best interest of the Service Users manages the Home. The Manager is approachable; operating an open door policy to ensure issues of concern can be raised appropriately. All Service Users are subject to a pre-admission assessment, with information obtained from the Service User, family members and outside agencies to ensure the Home is fully able to meet the individuals needs.All Service Users have an individual plan of care, which supports staff to implement appropriate care in a manner preferred by the Service User. Personal care is implemented in a manner that ensures the privacy and dignity of Service Users is maintained. Staff have a clear understanding of individual Service Users needs, receiving additional training and support as needed to fulfil their roles. The Homes recruitment procedure ensures all necessary security checks are undertaken prior to employment. Staff receive regular formal supervisions to ensure practice issues and the changing needs of Service Users can be discussed. The Staff team are dedicated and committed to providing a professional service. Medication procedures are robust ensuring the protection of Service Users. Activities provided at the Home are based on the likes and dislikes of Service Users, are age appropriate, enjoyable and provide opportunities for Service Users to meet and form new friendships. Visitors are welcomed at the Home with no restrictions in place. The Home has a comprehensive complaints procedure, there have been no formal complaints received at the Home or the Commission in the past 12 months. The Home protects the health and safety of Service Users, Staff and visitors to the Home by ensuring robust procedures are in place, supported by policies, and procedures, which are in line with current legislation and guidance.

What has improved since the last inspection?

The Home has purchased new furnishings to further support Service Users. An evident programme of redecoration is in place. The Home continues to provide a professional and skilled staff team, which ensures the needs of Service Users are met.

What the care home could do better:

The Inspector strongly recommends short-term goals as outlined in the individual Careplans be re-written to ensure they provide clarity as to what the identified issue is and the actions to be taken. A recommendation is made to this effect. The Inspector has also strongly recommended thorough interview notes are held on file to ensure the Home is able to provide evidence-based practice when meeting its equal opportunities policy.

This home is registered for 23 older people with up to 5 people with a learning disability, 6 people over 65 with dementia 1 person with a physical disability Date of last inspection 11th March 2005 Brief Description of the Service: Lathbury Manor is an attractive old house with listed building status, conveniently situated on a main road to Northampton, just north of Newport Pagnell. There is a landscaped garden and additional car parking to the rear of the property. Service user’s bedrooms and the communal rooms of the house have been comfortably furnished and pleasantly decorated. The home is registered to provide care for twenty-three service users. This was an Unannounced Inspection, which took place over a 4.5-hour period. The Manager was available throughout the inspection. During the inspection 28 of the Older Persons National Minimum Standards were assessed as fully met. Two Recommendations to further improve practice have been made. There have been no requirements made. The Inspector spoke with members of the staff team and met Service Users during the environmental tour of the Home. There were no visitors at the Home at the time of inspection therefore their views could not be sought. Records sampled during this inspection included Pre-Admission Assessments, Careplans, Recruitment Files, Fire Records, Risk Assessments, MAR (medication administration records) sheets and Complaints received in the past 12 months. The Inspector would like to thank the Service Users, Management and Staff team for the warm welcome received and support to complete this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3, 5, 6. All Service Users have a pre-admission assessment on file ensuring the Home is able to meet the needs of Service Users prior to any admission taking place. Prospective Service Users or significant others are invited to the Home prior to admission to ensure the Home is suitable. The Home does not supply Intermediate Care Services therefore this standard does not apply to Lathbury Manor. EVIDENCE: The Manager or Deputy Managers undertake a full pre-admission assessment in the Service Users Home to ascertain the immediate care needs. These documents were completed to a high standard and ensure the Home only admits Service Users whose needs can be met at Lathbury Manor. Service Users or significant others are invited to the Home prior to admission to ensure the placement meets their expectations, the Home have an open and transparent philosophy when approaching admissions ensuring all parties are Lathbury Manor Residential Home For The Elderly DS0000015061.V260491.R01.S.doc Version 5.0 Page 10 satisfied with the facilities and support offered prior to a decision for admission. Trial periods are in place to further support the Service User and the Home. Standard 6 of the Older Peoples Standards does not apply to Lathbury Manor. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Assessed Careplans were found to be up-to-date and were reflective of review, which enables staff to provide the required care. Staff demonstrated a sound understanding of the needs of Service Users and their interactions and relationships with Service Users was positive, which enables Service Users to express their thoughts, feelings and needs. Medication Administration Records were found to be appropriately signed with all medication administered, stored and disposed of in a satisfactory manner; ensuring Service Users are protected by the systems in place. Staff practice in giving and assisting with such elements as personal care and arrangements for medical examination ensure the privacy and dignity of Service Users. EVIDENCE: Careplans assessed were found to be up-to-date and reflective of the current needs of Service Users. The Careplan contain a Careplan checklist, which ensures all areas of the Careplan are reviewed within recommended Lathbury Manor Residential Home For The Elderly DS0000015061.V260491.R01.S.doc Version 5.0 Page 12 timescales. Careplans are informative and are maintained to a high standard with minimal areas for improvement highlighted by the Inspector. On inspection it would appear some designated senior members of staff have still not reviewed the Careplans on a monthly basis, a system has been put in place in response to the previous requirement to undertake these reviews. The Home is reminded to ensure all Careplans are subject to a monthly review using the systems in place. Discussion with the Manager and one Deputy Manager took place with evidence of the new system provided; Due to the Home addressing this issue of concern the Inspector has not made a further requirement. The Inspector will review this important element of Careplanning at the next inspection. The Careplans contain valuable information and assessments, which support Staff to implement individual care. Short-term goals are in place for all care needs that have been identified as a priority, these will require further work to ensure they are comprehensive and provide clarity as to what the identified issue is and the actions to be taken. Discussion as to how these can be further improved took place with the Manager and a Deputy Manager, the Inspector strongly recommends these plans be addressed as soon as is reasonably practicable to further improve the system in place. A recommendation is made to this effect. Staffs spoken with at the time of inspection were knowledgeable of the individual needs of Service Users and the need for a holistic approach to care provision. Staff conveyed a sense of pride in their work and dedication, ensuring the needs of Service Users is central to the day-to-day operation of the Home. Service Users were addressed using their preferred name and interactions were relaxed and sensitive to the individual personalities and needs of the Service User. MAR (medication administration records) sheets were found to be appropriately signed with no gaps. A thorough system for the ordering, storing and disposal of medication is in place with consultation with the contracted Pharmacist taking place on a regular basis. The Inspector did express concern that the current system of returning all PRN medication at the end of each month was not particularly cost effective, however as this has no effect on the way in which medication procedures meet the standards and regulations the Inspector is satisfied this is a matter for the Home and the Pharmacy to discuss without C.S.C.I. involvement. All staff administering medication have undertaken extensive certificated training. All care is undertaken in the privacy of the Service Users own room or communal bathrooms. Staff ensure the privacy and dignity of Service Users is maintained implementing care in a manner that is in keeping with the individual needs and wishes of the service User. All medical examinations are undertaken in the Service Users own room with staff support should this be Lathbury Manor Residential Home For The Elderly DS0000015061.V260491.R01.S.doc Version 5.0 Page 13 required by the Service User. Staff were observed knocking on doors before entering to further protect the privacy of Service Users. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13. The Home is implementing activities suitable for the Service User group, this enables Service Users to interact on a social level and form new friendships with each other as well as staff. Visitors are welcomed at the Home during all reasonable hours with no restrictions in place. This enables Service Users to maintain their friendships and relationships whilst residing in the Home. EVIDENCE: The Home provides activities, which are age appropriate, interesting and enjoyable. All staff are involved in the planning and implementing of activities. Outside entertainers come to the home for such things as pantomimes, quizzes, and games. A trained physiotherapist attends the home to undertake exercise sessions, in addition the home are planning to provide K.F.A. qualification training to provide appropriate seated exercise programmes which can be undertaken on a daily basis. Visitors are welcomed at the home, which ensures Service Users are able to maintain their relationships after admission to the Home. There are no restrictions on visiting times. Staff are supportive of family members including them in the daily routines of the Home. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16. The Home has a comprehensive complaint procedure, the complaints procedure is made available to Service Users and significant others, which ensures all relevant persons, are able to make a formal complaint appropriately. EVIDENCE: The Home has received no complaints at the Home or directly to the Commission in the past 12 months. The Home has an open door philosophy for Service Users, families and Staff to ensure that issues of concern can be discussed and problem solved before they become formal complaints. The Homes complaints procedure is accessible to Service Users and family members. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25, 26. The Home is well maintained with an evident programme of decoration and refurbishment. Comfy furnishings, ornaments, T.V, fixtures and fittings are supplied in the communal areas making the environment for homely for Service Users. Suitable single and double sleeping accommodation is provided with evidence of personal possessions to make these more homely for Service Users. The Home is cleaned to a high standard with no offensive odours providing a pleasant and hygienic environment for Service Users. Sufficient numbers of toilets and bathrooms are available to ensure the needs of Service Users are met. The Home is situated in a pleasant well-maintained garden, which is accessible to Service Users. EVIDENCE: Lathbury Manor Residential Home For The Elderly DS0000015061.V260491.R01.S.doc Version 5.0 Page 18 The Home is an older listed building which has retained many of its original features. An evident programme of decoration and refurbishment is in place, with new lounge furnishings and mattresses arriving at the Home on the day of inspection. Maintenance issues are addressed as soon as is reasonably practicable to ensure a safe environment is maintained for Service Users. The Home has three communal lounges and a dining room all accessible to Service Users. Comfy furnishings are provided with lighting, ventilation and heating necessary to support Service Users in a comfy and homely environment. Service Users spoken with did comment that additional lighting was needed in one lounge area as the lighting was quite dim, this has already been addressed by the Proprietors with the purchase of side lamps that can be used by individuals for reading without causing a high glare environment for those preferring softer lighting. The Home provides two double bedrooms as well as its single occupancy accommodation. All bedrooms have been individually decorated with adequate facilities to support Service Users. Call bells and a variety of adaptations necessary to support Service Users are provided. Bedrooms are pleasant and homely with evidence of personal possessions. The Home is cleaned to a high standard taking into consideration infection control measures and Health & Safety guidance. The Home provides laundry facilities for both washing and drying clothes at the Home. These facilities are sufficient for the needs of Service Users providing a sluicing cycle for soiled items. Schedules for shampooing carpets is in place, which is updated on a daily basis. All items of C.O.S.H.H. are appropriately stored in a lockable facility to further protect Service Users. Sufficient numbers of toilets and bathrooms are situated throughout the Home in close proximity to bedrooms and communal areas, these all provide adaptations and equipment to further support the needs of Service Users. All toilets and bathrooms were found to be clean, tidy and free from offensive odours at the time of inspection. The Home is set in a pleasant and well maintained garden that is accessible to Service Users. This area is used frequently during the summer months with suitable garden furniture providing seating and shade, enabling Service Users to enjoy and relax comfortably. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30. Service Users are supported by sufficient numbers of suitably trained and skilled Care staff ensuring the needs of Service Users is met at all times. The Home has a full recruitment procedure in place, which is in line with Schedule 2 and Standard 29, however a documented interview process is required to provide evidence of equal opportunities and further enhance the protection of Service Users. EVIDENCE: The Home provides sufficient numbers of suitably trained and skilled staff to meet the needs of Service Users. Numbers are above the recommended staffing level to ensure activities and additional needs of Service Users can be met. On the day of inspection Staff were observed going about their duties in a professional and sensitive manner. The Home has a thorough recruitment procedure in place, ensuring all necessary security checks are undertaken prior to employment at the Home. CRB disclosures and POVA checks of newer staff members were seen by the Inspector, in addition two written references were on file for each Staff member. The Home do need to ensure thorough interview notes are kept on file to ensure they are able to provide evidence of an equal opportunities employment system. On saying this, the Inspector is satisfied the Home are working within current guidance and legislation as notes are available, the Lathbury Manor Residential Home For The Elderly DS0000015061.V260491.R01.S.doc Version 5.0 Page 20 recommendation to include more thorough notations will aid improvements in the recruitment procedures. A recommendation is made to this effect. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 36, 37, 38. The Manager has the necessary skills and training for her role, to ensure practice in the Home is in the best interest of Service Users. Staff supervisions are taking place at regular intervals, which provides a forum to discuss practice issues and ensures the changing needs of Service Users is met. Individual records were up-to-date and stored appropriately, ensuring the Home are safeguarding Service User information and working within guidance of the Data Protection Act 1998. The Health and Safety procedures and record keeping of the Home ensures Service Users, Staff and visitors to the Home are protected as far as is reasonably practicable. EVIDENCE: Lathbury Manor Residential Home For The Elderly DS0000015061.V260491.R01.S.doc Version 5.0 Page 22 The Manager/Provider of the Home is suitably qualified and skilled to ensure the Home is working within the best interest of Service Users as well as ensuring the home is working within current legislation and guidance. The Manager has obtained her Registered Managers Award. Staff find the Manager approachable and supportive with a clear vision of how the Home will meet its objectives. All staff receive a minimum of six formal and documented supervisions annually, to ensure a forum is made available to discuss practice issues and the changing needs of Service Users. Records held in relation to Service Users were found to be up-to-date, relevant and reflective of review. All documentation is held securely in lockable facilities to further ensure the confidentiality of Service Users is maintained. The Home ensures the health and safety of Service Users is protected through its policies, procedures and practice. As previously mentioned in this report items of C.O.S.H.H. are stored appropriately in lockable facilities to protect Service Users from harm. Fire records for the Home were assessed during this inspection, records assessed were the fire risk analysis, fire procedure policy, training manual and recordings for such things as fire drills and alarm, fire point and emergency lighting tests. All systems of recording were found to be up-to-date with staff receiving regular fire briefing to ensure they are up-to-date with changes in the environment or policy. A more thorough check of Health & Safety systems and maintenance records will be assessed at the next inspection. There were no issues of concern identified on the day of this inspection.
