It is part of a larger complex that includes sheltered housing; warden controlled flats; and privately owned homes for older people. Near to the nursing home, on the same site, there is a two storey listed building. Ellerash Ltd, which is part of the Care UK group, owns the Little Holland Hall complex. Care UK is a national care organisation. This nursing home has 41 single rooms divided between two floors. All of the rooms have en suite facilities. The home offers nursing care for up to 41 older people. Within this number, three beds are also registered for residents with palliative care needs. The home is not registered for dementia care. Current resident fees range between 442.54 pounds and 725.00 pounds, per week. 1 7 0 6 2 0 0 9 Care Homes for Older People Page 3 of 8 What we found:
The reason for this inspection was to assess progress towards meeting the requirements made following the random inspection on 17th June 2009. We made a requirement that where medical oxygen is prescribed, there must be records kept of its prescription and administration; and include the flow rate and length of use. We looked at the records of oxygen administration on the medication record forms and also care plan documentation. We found that for the 5 people currently using oxygen, 4 of the records showed the flow rate but none of them had a verified instruction from the prescriber of what rate to use. There was also no record of the duration of use when this was intermittent. So although there has been progress towards meeting the requirement by the given timescale of 31/07/2009, the requirement has not been met in full and has been given a new timescale for action. We also made a requirement that where medical gases were used and stored, there must be appropriate risk assessment and risk management procedures documented and in place. We looked at the care files for each of the 5 residents currently prescribed oxygen and found that all had an identical form listing the hazards and controls for oxygen usage but the relative risks for each person were not rated. We also looked at the rooms where medical oxygen is in use and 4 of the 5 room doors carried suitable hazard signs but one did not. The person resident in the room was using oxygen at the time. Again the requirement has not been met in full by the timescale of 31/07/209. Additionally, on arrival at the home, we found several racks containing blister packs of medication in the nurses office on the ground floor. The door to the room was open, the medication clearly visible from the corridor and the room was unattended for a period of at least 15 minutes. This is clearly a risk that medication could be accessible to unauthorised people and people it is not prescribed for. In such circumstances we would normally leave an immediate requirement notice, but it was resolved during the inspection. However, a further requirement has been made.
