The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Residents said that they like living in the home. The staff are experienced and knowledgeable. The owners have considerable relevant experience. The home is maintained to a high standard. Records and policies and procedures are generally well kept.

What has improved since the last inspection?

The owners have ensured that the reviews for placements of residents placed by Brighton and Hove will take place. Staff now receive regular recorded supervision. The home has introduced quality assurance and monitoring systems. Staff have received manual handling training. Terms and conditions of residency have been developed. The owners have received more comprehensive evidence of the diagnosis of one resident`s mental health needs. One provider has commenced the registered managers award/NVQ 4 in care course and the home has attained the recommended level of NVQ qualification for staff.

What the care home could do better:

The home must keep available for inspection records required to be available. Core training should be provided as needed. Supervision should be provided for care staff at least six times per year. The activities for residents could be developed over time.

Only adults with a learning disability who have been assessed as requiring residential care are to be accommodated. 2. The maximum number of service users to be accommodated is twelve (12). 3. Service users must be aged between eighteen (18) and sixty-five (65) years on admission. Date of last inspection 2nd March 2005 Brief Description of the Service: Loxwood House is registered to provide accomodation and personal care for twelve adults with learning disabilities. The registered providers and managers are Peter Mallinson and Angela Mallinson. The home is located in Hove, with good access to local transport and amenities. Loxwood House is a large detached property set out over three floors. There are eight single bedrooms and two shared bedrooms and a range of communal space. This unannounced inspection took place during the mornings of the fifth and fifteenth of September 2005. Before the inspection the inspector read records held on the home by the Commission of Social Care Inspection and prepared to those sections of the standards to be covered at that visit. The actual inspection of the home took 5.7 hours in total. The second visit was necessary because some records needed for the inspection were not available on the first occasion. The inspector made a tour of most of the premises, and read a variety of policies, procedures and records. The owners, three staff and six residents were spoken with. JUDGEMENT – we looked at outcomes for standard(s) 1,3,4 and 5. The home provides full information to prospective residents and their representatives to inform their decision about coming to live in the home. The home meets the needs of those living there. Prospective residents have the opportunity to visit the home. The home has a contract with each resident. EVIDENCE: The home has a statement of purpose and service users’ guide which gives full information. A necessary amendment was made during the inspection. From discussion with staff and residents and examination of records it is clear that staff individually have the skills and experience to meet the needs of residents. Observation confirmed that they are able to communicate with residents. Arrangements have now been made for all residents who have been placed in the home by Brighton and Hove City Council to be reviewed in November 2005 and evidence of this was seen. The home encourages prospective residents to visit and/or stay before admission. The owners said that this process has varied according to the individual needs of residents. Residents are admitted for a trial period. The home does not make emergency admissions, but has on occasion admitted a resident over a foreshortened time scale where this was needed and could be achieved. The home has drawn up terms of conditions of residence. It also has a contract with the responsible placing authority for each resident. JUDGEMENT – we looked at outcomes for standard(s) 6,8 and 9. Comprehensive care plans and risk assessments are kept and regularly reviewed. Residents are involved in the day to running of the home. EVIDENCE: The care plans of four residents were examined in some detail. One plan was not signed as the owners were awaiting consultation with a representative. It is recommended that it be signed in any event. Plans were seen to be regularly updated. The owners said that they review the plans of residents over 65 monthly. The home does not operate a key worker system. Residents do not attend staff meetings. They complete user satisfaction questionnaires. Residents are asked to give informal feedback on prospective staff members who are being considered for appointment. Risk assessments are completed, and staff confirmed that these are available to them. Records inspected showed that these are reviewed with the relevant placing authority and with the family. Staff said that they give advice and guidance to residents as needed. The home has a suitable policy regarding action to be taken in the event of a resident going missing. Residents undertake valued activities and this will be developed over time. The home participates in the local community. Visitors are made welcome. Meals and mealtimes promote the health and well being of residents. EVIDENCE: Residents have opportunities to learn and use practical life skills. They have varied programmes at the day centres which they attend, and they spoke about these with enthusiasm. An older resident attends an appropriate day centre. Residents have access to appropriate professional guidance. Residents have opportunities to fulfil any spiritual needs. The owners said that no residents are at present engaged in paid supported or volunteer jobs. Residents take part in valued activities, and the recent quality assurance exercise elicited a range of responses, and these are being reviewed. The owners said that a member of staff is about to start “floating hours” to give the opportunity to residents to be assisted to do more valued activities with staff. Staffing is currently increased for two evenings when residents are out later at clubs/activities. Staff said that the staff group is knowledgeable about local resources. The home has its own transport and residents are taken out. No residents use public transport or go out independently currently. Residents said that visitors are made welcome. Staff said that visitors are made welcome and given a cup of tea. The home keeps a record of food served and this was inspected. It gave an up to date record of meals provided and alternatives given. Residents’ likes and dislikes are noted. The owners said that special diets are catered for. Residents said that they like the meals. Previous inspections refer to the level of participation of residents in food preparation at the home. The owners said that they plan to involve residents more in some aspects such as the preparation of sandwiches. JUDGEMENT – we looked at outcomes for standard(s) 18 and 20. The home makes effective arrangements to meet the care needs of residents. The medication systems are thorough. EVIDENCE: Staff said that residents are given personal support in private, and are given guidance and support as needed about personal hygiene. Staff said that residents eat in their rooms if they are not well. Most residents get up early during the week to go out to day centres, but can stay in bed longer at weekends. Residents’ medicines are securely stored. The record of medicines administered was inspected and found to be fully kept. Staff who handle medicines said that they had had suitable recent training from a pharmacist and records inspected confirmed this. JUDGEMENT – we looked at outcomes for standard(s) 22 and 23 The home has suitable arrangements for dealing with complaints made to it. The home’s procedures and processes are designed to protect residents in the event of any allegation of abuse. EVIDENCE: The home now has a suitable complaints policy available to residents. Necessary modifications to it were made during the inspection. No complaints have been received by the home in the last year, and none has been received by the Commission for Social Care Inspection. The home has a system for dealing with any complaints received. Staff confirmed that they have received training in adult protection and records inspected confirmed this. Suitable training will be arranged for a recently recruited staff member. Staff confirmed that they are aware of the home’s whistle-blowing and adult protection procedures. The procedures have been invoked in respect of one resident in the past year. Matters were swiftly resolved with the home playing its full part. JUDGEMENT – we looked at outcomes for standard(s) 24,26,28 and 30. The home offers spacious and attractive accommodation. The bedrooms and communal areas are well presented. Laundry facilities are suitable. EVIDENCE: The home offers safe, comfortable and airy accommodation. The premises offer access to local amenities, transport and local support services. They are well maintained. Staff said that items needing attention are dealt with swiftly. The premises have been redecorated inside and out since the last inspection. Residents said that they like their rooms, and that they are able to personalize them. A resident said that when the home was redecorated recently they were able to choose the colour of their room. Residents are able to bring in their own possessions. Residents confirmed that they are able to lock the doors to their rooms if they so choose and were seen to do so. The home has two large communal areas. Both are well furnished and decorated, and lighting is domestic in style. The kitchen is domestic is scale. There is a well-sited office/sleeping in room. Review of the facility for storing staff belongings is recommended. The home has a laundry suitably located away from food preparation and serving areas. It is suitably equipped. The home was found to be clean and tidy throughout. JUDGEMENT – we looked at outcomes for standard(s) 32,35 and 36. Residents are supported by competent and qualified staff. Some attention to provision of core training is needed. Staff are regularly supervised to assist them to meet the needs of residents, but not to the recommended frequency of at least six times per year. EVIDENCE: Residents said that they like the staff group. Since the last inspection the home has attained the recommended level of NVQ qualification. The home has six staff. Two staff hold NVQ level 3 in care and a further member has attained NVQ level 2 in care. Staff show enthusiasm for training and for example a staff member is proposing to undertake further training in Makaton. The record of staff training was made available and the training given to those spoken to was checked. The owners said that they provide their own induction for new staff and staff spoken to confirmed this. The owners plan to put staff on a nationally accredited induction course when places are available. A recently appointed staff member needs refresher training in manual handling. Staff confirmed that they receive regular supervision, but records inspected that showed that this was not to the recommended frequency of at least six times per year. JUDGEMENT – we looked at outcomes for standard(s) 37,38,39,40,and 41. The owners have extensive experience and are undertaking relevant training. The home has strategies for enabling residents and staff to voice issues, and quality assurance mechanisms. Policies and procedures and records are satisfactory. They share the responsibilities of registered provider and manager roles. Mrs Mallinson trained as a nurse but no longer maintains her registration. Mr Mallinson demonstrated that he has commenced the registered managers award and reported that he hopes to complete it early in 2006. Both undertake periodic training from time to time to maintain and update their knowledge and skills. Residents said that they like the owners and staff. The home has regular staff meetings and residents meetings at which issues which affect the way in which the service is delivered can be raised. Copies of minutes of these meetings were made available to the inspector. Since the last inspection the home has introduced a quality assurance mechanism and questionnaires have been completed by residents, their families and relevant stakeholders. The outcomes are being used to inform planning for the future of the service. Staff confirmed that they have access to the home’s policies and procedures, and that they are familiar with them. There is evidence that the policies are regularly reviewed. It is recommended that they are signed by the registered managers. Those records inspected were found to be up to date and well kept. Residents have access to their personal records. Not all records required to be available in the home for inspection were available on the first day of the inspection. They were made available on the second occasion. A requirement has been made about this.
