Conditions of registration: 1. 2. Admission of one named resident under the age of 60. On admission persons should be aged 60 years and over. Date of last inspection 24th May 2005 Brief Description of the Service: Meadowview is a privately owned nursing home located in Standlake, Oxfordshire, and is situated in a residential area of the village. The two-storey building has been extended to provide private rooms and communal areas for residents, and has recently been extended to provide a conservatory. The majority of the private accommodation is provided downstairs with two double rooms provided upstairs. There is a passenger lift and toileting, bathing and showering facilities for all the residents. The village of Standlake has shops and pubs and has a local bus service. The proprietor is planning to further extend the home to improve the homes environment and to provide extra rooms. Meadowview Nursing Home DS0000027163.V261923.R01.S.doc Version 5.0 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This CSCI unannounced inspection was conducted over one morning when the nurse manager was not on duty. A registered nurse was on duty and the proprietor was in the home during the inspection. The nurse manager is in the process of becoming the registered manager and the process is expected to be completed soon. The inspector spoke to several residents, visited most areas of the home, saw records, policies and procedures, and discussed the management of the home with the proprietor and staff. The proprietor is planning to extend the home and is continuing to work with the appropriate authorities to meet all the planning and building requirements. What the service does well: What has improved since the last inspection?
References for staff recruited through agencies are verified so that the proprietor knows that all staff in the home are properly recruited. Infection control procedures have improved. The training of staff to care for individuals with dementia has improved, and is continuing. The new conservatory provides more space and is regularly used by residents. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 and 10. Medication is well managed at the home, though there is some difficulty with supply. Residents are treated with respect and privacy is respected. EVIDENCE: The home’s medication is supplied from the pharmacy attached to a GP practice, and staff report problems in getting the pharmacy to act quickly in replying to their requests. Medication is delivered weekly and recorded for individual residents in the appropriate book. To comply with the Royal Pharmaceutical Society’s guidelines the nurse recording the delivery must sign and date the entry. The inspector spoke to two residents who said that staff treat them with respect, and the inspector witnessed good practice regarding privacy. The inspector saw the home’s induction policy and recommends that the topics of privacy and dignity be included in the induction programme. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13 and 14. Improvements are planned to the lifestyle at the home so that residents can have more opportunities for stimulation. Residents can maintain contact with relatives, friends and the local community as they wish. Residents are helped to be as independent as possible. EVIDENCE: The inspector spoke to several residents about their experiences of life at the home. Some go to the nearby ‘Over 60’s Club’ regularly, and it was clear that individual preferences are respected from the detail given to the inspector. The proprietor has plans in hand to improve opportunities for stimulation for the residents by appointing an activities organiser, as at present there is little provision for individual preferences. There is no printed activities list, although staff are aware of when entertainers regularly visit the home, and remind residents of the day’s activity. Residents said that visitors could come at any reasonable time, and that there were no restrictions on contacting relatives when they wished to. Residents are encouraged to be as independent as possible, and can bring personal possessions with them into the home. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17 and 18. Residents’ legal rights are protected. Residents are protected from abuse, but the procedures need to be amended. EVIDENCE: Relatives help residents with legal matters, and residents can access an advocacy service if necessary. Staff and relatives help residents who want to vote. The home has policies and procedures to protect residents from abuse, and reference to the Oxfordshire codes of practice should be included in the home’s adult protection procedures. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27. The numbers and skill mix of staff meet the residents’ needs. EVIDENCE: The inspector saw the staff rota, showing which staff were on duty during the days and nights of the week of inspection. It was clear that appropriate numbers of skilled staff are available to meet the needs of the residents. Household staff are employed to cover kitchen, laundry and cleaning duties. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35, 36 and 38. Residents’ financial interests are protected. Staff are appropriately supervised. Due to uncertainty around fire procedures and a broken window, it was not clear that the safety of residents was safeguarded at the time of inspection. EVIDENCE: Few residents are able to manage their own financial affairs and when residents are not able to manage, then relatives or Money Management takes responsibility. Written records of all transactions are kept. Formal supervision is in place for staff and appropriate records kept. The inspector saw the fire safety records and discussed with staff the home’s arrangements regarding safety of residents in the event of a fire. One member of staff was not sure how a particular fire door was opened, and it was clear that wheelchair users would not be able to use the same door in the event of a

From the recording it was not clear to the inspector that all staff had attended fire lectures, as a matrix is not used. The inspector recommends that the individual responsible for Health and Safety at the home should review procedures to ensure that all staff are appropriately trained in the fire procedures, and that staff are familiar with evacuation procedures for residents using wheelchairs. The inspector noted that a secondary glazed window in a shared room upstairs had 2 broken panes of glass. The proprietor complied with the requirement to arrange for replacement glass to be fitted as soon as he was informed. The proprietor must make arrangements for the broken glass to be replaced in the identified window.
