Conditions of registration: None Date of last inspection 22/02/05 Brief Description of the Service: 22 Pendarves Road, Penzance, is a small care home providing accommodation and personal care for up to two adults with a learning disability. The Registered Provider lives on site with her family and provides all the necessary care and support to service users with the assistance of two part-time assistants. The home is situated in Penzance within easy reach of transport routes, shops and local community facilities. Accommodation is provided in a mid-terrace, two storey house that is spacious, well decorated and domestic in character. Service users have their own bedrooms and share a bathroom on the first floor. This is an overview of what the inspector found during the inspection. The inspector visited 22 Pendarves Road, Care Home on the 06 September 2005 and spent two and a half hours at the home. This was an announced visit. The purpose of the inspection was to gain an update on the progress of compliance to the requirements that were identified in the last inspection report dated 22 February 2005. In addition the inspector focused on the following key areas of care: choice of home, care planning, lifestyle, adult protection, staffing, some of the environment and some management areas. On the day of inspection 2 residents were living in the home. The methods used to undertake the inspection were to meet with the registered provider to gain her views on the services that 22 Pendarves Road offer. The residents were not at home at the time of the inspection however positive comment cards were received from them and their relatives. The home’s records, policies and procedures were examined and the inspector toured the building. This report summarises the findings of this inspection. What the service does well: What has improved since the last inspection?
The flooring in the kitchen has been replaced. A shower has been installed in the residents’ bathroom; this resulted in extensive work being undertaken on the drainage system of the house. New decking has been laid in the garden. There is now a record of all visitors to the home. The residents daily records are kept in separate diaries for confidentiality and to comply with data protection legislation. JUDGEMENT – we looked at outcomes for standard(s) 1 Prospective residents will be given information about the home enabling them to make an informed decision. EVIDENCE: The home has a suitable statement of purpose and service users guide that include all of the information required by legislation. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6 and 10 Individual care plans are generated with each resident, representative and Social Worker, they inform and direct the staff in their care provision. Systems are in place to ensure that information is handled appropriately and residents know that confidentiality is respected. EVIDENCE: Each resident has an individual care plan that is reviewed every six months and signed by the resident, relative and social worker. The Registered Provider reviews the care plan informally, with the resident. She then meets with the social worker to go through it again. When it is agreed by them the care plan is typed and sent to the next of kin or representative to review. Individual risk assessments are undertaken and documented. Daily records are maintained in separate diaries for each resident and records are stored in a locked filing cabinet. The registered provider said that residents can access their records if they wish. The home has a written confidentiality policy. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,15,16 and 17 Residents attend clubs and can attend courses giving them the opportunity for personal development. Residents have jobs and take part in appropriate activities that encourage their independence and individuality. Links with family, friends and the community are good and allow residents the opportunity to socialise. Dietary needs of residents are well catered for with a varied selection of food available to meet their taste and preference. They are provided with information on activities, advocacy, holiday options and local facilities. One resident is engaged in local church activities. The Registered Provider said that both residents were independent, they assist in the household tasks and go into town and out walking at the weekends, for example. Both residents attend weekly activities. One resident is going to go to organised cycling sessions. Both residents went on an organised holiday earlier in the year and one attends an annual holiday organised by the Gateway club. Both residents go out to work during the week. They can attend courses if they wish. Residents have photographs of family and friends in their rooms. The Registered Provider said that both residents had regular contact with their families by visits or telephone. There is a record of all visitors to the home. The daily routines promote independence and are flexible within reason, more so at the weekends. The daily routines and house rules are included in the care plans. Both residents have front door keys and are able to enter and leave the home as they wish. Neither resident wants a key to their room. Both have mobile phones for contact when they go out. A healthy diet is encouraged and the home has a very flexible menu plan. Residents are provided with a cooked evening meal and are able to access fresh fruit and snacks at all times. They have lunch at their work or day centres during the week and meal times are more flexible at the weekend. Residents have their meals in their own lounge / dining room. A special meal and a cake are provided on birthdays and friends are invited for tea if they wish. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 20 Residents have access to health care services as necessary to ensure their physical and emotional needs are met. There are suitable systems and policies in place for dealing with residents medicines and assure residents safety. EVIDENCE: Residents are assisted to manage their healthcare needs. They are registered with a General Practitioner, sight tests are undertaken annually and they visit a dentist regularly. There are relevant risk assessments undertaken for both residents. The Registered Provider maintains strong links with social workers and the epilepsy nurse specialist.. The home has a written policy in respect of medication and relevant guidelines. Residents’ have consented to the Registered Provider administering their medication. A monitored dosage system (MDS) is used. EVIDENCE: The home has an appropriate Adult Protection policy and the registered provider has a copy of the local authority procedures. There is a separate whistle blowing policy. JUDGEMENT – we looked at outcomes for standard(s) 24 and 30 The home and grounds are well maintained providing a safe environment for residents, staff and visitors. The home was clean and free from offensive odours making it a pleasant place to live in EVIDENCE: The home provides a comfortable, homely environment and was clean and tidy on the day of inspection, with no offensive odours. The grounds were tidy and attractive and they are accessible to the present residents, new decking has been laid. The residents participate in the gardening. The home is not accessible to wheel chair users. There is adequate heating, lighting and ventilation. The home is well maintained and there is a programme for redecoration and refurbishment. A new shower has been installed in the residents bathroom and new flooring has been fitted in the kitchen. The laundry facilities are adequate for the size of the home and are situated in an outside building at the back of the home. JUDGEMENT – we looked at outcomes for standard(s) 34, 35 and 36 Recruitment procedures undertaken by the Registered Manager are robust and offer protection to the residents. Staff receive training relevant to their roles to ensure that residents needs are met appropriately. Staff are well supervised and supported and residents benefit from this. EVIDENCE: There has been one new employee since the last inspection. Her file was inspected and contained the documents required by legislation. Relevant employment checks are made. Terms and conditions of employment are given to employees. Photos of the staff, registered provider and her parents are displayed on the notice board so that residents can identify them. There is an induction programme for new staff that includes a checklist. Appraisals each year and regular supervision identify training needs. Training is undertaken as necessary, moving and handling is the next to be done. The Registered Provider supervises the staff when going about their work. Recorded supervision is undertaken every six months. JUDGEMENT – we looked at outcomes for standard(s) 42 and 43 Appropriate training and safety checks are undertaken to ensure the health safety and welfare of residents and staff. Accounts show the business to be a viable concern and residents benefit from the competent management of the home. EVIDENCE: The registered provider endeavours to ensure that working practices are safe. Relevant service checks take place as required although it is recommended that the home have a certificated electricity wiring test undertaken. Staff receive statutory training and there is a person trained in first aid on duty at all times. There is a fire procedure that is on the notice-board and regular fire drills take place. Accident reporting complies with data protection. Health and safety risk assessments have been undertaken. The homes’ accounts showed that the business is viable, records are maintained and receipts kept. The insurance certificate is displayed and there is appropriate cover. The registered provider has an annual business plan for the home.
