What the care home does well

The home provides a high level of support to service users within a small homely environment. Staff have a good knowledge of individual service users` needs. Independence is promoted, and staff were observed encouraging service users to exercise choice regarding their meals and daily activities. Staff use British Sign Language, symbols, and pictures to communicate with service users, depending upon their specific needs. Service users each have a spacious bedroom, which has been decorated to reflect their personal tastes and preferences. All service user rooms have en suite facilities.

What has improved since the last inspection?

A new communication system has been introduced at the home for two service users. This is intended to develop their communication skills, and promote independence. Care plans have also been revised. The home now has working file containing current assessments and plans and a separate file for archived documents. The kitchen, dining room and hallway have been re-decorated within the main house. Two service user bedrooms have also been decorated and refurbished. Service users have been fully involved in choosing colours, and furniture for their rooms. Shower units have been upgraded in all bathrooms. A specialist shower, recommended by the RNIB has been provided for one service user. The home also aims to extend the laundry area situated at the rear of the property.

What the care home could do better:

The home has made progress in a number of key areas since the last inspection. The home should continue to use the monthly team meetings and regular supervision to aid staff development and review the service provided. The home must ensure that a signature is recorded for all medications given.

Date of last inspection 21st January 2005 Brief Description of the Service: 30 Norbins Road is small residential home situated within walking distance of Glastonbury town centre. Accomodation is provided for 3 people within the large Victorian house, whilst a fouth person lives more independently within the self-contained bungalow located at the rear of the property. The home is registered with the Commission for Social Care Inspection to provide care for up to four people who have a sensory impairment. The service users residing at this service also have additional care and support needs as a result of learning disabilities. The Registered Manager is Pippa Greed and Registered Provider is SENSE. This inspection was carried out as part of the planned annual programme of inspection. The inspection was announced and carried out by one inspector over one day. The previous inspection was unannounced and took place on 21st January 2005. On the day of the inspection there were three service users residing within the home. The service user living within the bungalow was on holiday; therefore this part of the home was not seen during this inspection. Prior to the announced inspection Comment Cards were sent to service users, relatives and healthcare professionals. Three cards were returned and each of these commented positively on the service provided. Throughout the inspection there was an interpreter present to aid with signed communication. During the course of the visit service users, staff members and the Registered Manager were spoken with. Care practice was also observed, records examined and a tour of the premises was made. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4 & 5 Service users and their families are provided with appropriate information regarding the home. Appropriate assessments are completed to ensure that the home will be able to meet service users’ needs. EVIDENCE: The home has a Statement of Purpose and Service User Guide that provide details of the services and facilities offered at the home. The Statement of Purpose has been updated to include recent photographs of the staff team. The Service User Guide is available in a format that is accessible to service users residing at the home. An assessment of need is completed prior to any service user moving into the home. Prospective service users are invited to spend time at the home. A contract is provided for each service user. The home is in the process of developing these to ensure that they are in a format that is accessible to the current service user group. JUDGEMENT – we looked at outcomes for standard(s) 6,7,8,9 & 10 The home has developed an appropriate care plan for each service user. Service users are supported to make choices regarding their life. Staff demonstrated a good knowledge of individual service users’ communication needs. Appropriate records are maintained in relation to service users’ finances. Records relating to service users are stored securely. EVIDENCE: Care plans are maintained for each service user. These have recently been revised, and now consist of a working file containing key information and a further file containing archived documents. Care plans provide details of service users needs, daily routines and preferences. Staff also maintain daily records addressing service users’ living skills, daily activities, exercise, communication, personal care and diet. Care plans included communication guidelines. Risk assessments had been completed as required. Care plans had been regularly reviewed and updated as necessary. Service users are encouraged to exercise choice regarding their daily routines. There is a board displaying which staff will be on duty. None of the current service users have advocates. There is a circle of support for each service user. Records are maintained of all transactions involving service user finances. Service user monies were examined, and all seen tallied with records kept. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,13,14,15,16 & 17 Service users are provided with a range of activities that are appropriate to their individual needs, and are supported in accessing the local community. Service users are involved in planning menus, and a choice is offered for each meal. EVIDENCE: An activities board is displayed for each service user. Service users are able to participate in a wide range of activities including: arts and crafts, games, sewing, keyboard, music, yoga, exercise bike, trampoline, tactile play, massage, foot spa, hair and beauty, cookery and gardening. Service users are also supported in accessing the local community and enjoy blind club, horse riding, and local walks. There are regular trips out. Service users have also recently been away for short stays to Glasgow, and Blue Anchor. Staff support service users in maintaining contact with family and friends. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. JUDGEMENT – we looked at outcomes for standard(s) 18, 19 & 20 Service users are provided with appropriate assistance to meet their personal care needs. The home supports service users in accessing healthcare services. EVIDENCE: Service users are provided with assistance to undertake personal care tasks as required. Personal care is provided in private. Staff support service users in accessing health care services, and ensure that specialist advice is sought as required. Staff at the home receive medications training. A competence-based assessment is completed for each staff member involved in the administration of medications. Medication Administration Records include a photograph of the service user. A record had been recorded for all medications entering and leaving the home. All hand transcribed entries had been supported by two staff signatures. Clear directions had been recorded for PRN medications. The home must ensure that a signature is recorded for all medications given. All medications are stored securely. JUDGEMENT – we looked at outcomes for standard(s) 22 & 23 The home has appropriate policies and procedures in place to safeguard vulnerable adults. EVIDENCE: The home has a complaints procedure that includes details of external agencies that may be contacted including CSCI. The complaints procedure is explained to each service user, in a format that is suitable to them on a three monthly basis. There have been no formal complaints received by the home since the last inspection. The home has a Protection of Vulnerable Adults and whistle blowing policy. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. JUDGEMENT – we looked at outcomes for standard(s) 24,25,26,27,28,29 & 30 The home has been decorated and furnished to a good standard. There are sufficient communal areas and bathing facilities to meet service users’ needs. The home was found to have a high standard of cleanliness. EVIDENCE: The home is domestic in style. As previously mentioned there has been significant re-decoration since the last inspection. Communal areas comprise of a lounge, dining room, and sensory room. There is also a pleasant garden containing seating areas that is accessible to service users. All service user rooms have en suite facilities. Service users have been involved in choosing colour schemes and furniture for their rooms, and all rooms have been personalised to reflect individuals’ tastes and preferences. Radiators have been guarded and window openings restricted on upper floors. Hot water temperature outlets were tested and found to be within appropriate limits. The home follows good practice with regard to infection control. The Environmental Health Officer visited the home on 1st June 2005 and found the premises and practices to be satisfactory. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,35 & 36 Staffing levels are appropriate to meet service users’ needs. Staff are provided with the training and support required to undertake their role. The home operates a robust recruitment procedure. Staff receive regular supervision. EVIDENCE: Duty rotas are maintained. On the day of inspection, the Registered Manager, a Senior and one social tutor were on duty. The home currently has some staff vacancies. Staff have recently worked additional hours to ensure that service users receive appropriate levels of support during the period whilst recruitment is taking place. Newly appointed staff received Induction training. Staff are encouraged to attend further training, and there are currently five staff studying for the NVQ qualification. A clearly written training record is maintained for each staff member. Training courses recently attended have included: deaf blind person, challenging behaviour, adult protection, first aid, autism, and health and safety. The home has monthly staff meetings where policies and care practices are reviewed. Staff are provided with regular supervision. Service users are confident their views underpin all self-monitoring, review and development by the home. JUDGEMENT – we looked at outcomes for standard(s) 37,38,39,40, 41, & 42 The Registered Manager is an experienced manager. She is mainly office based, and should aim to increase time spent within the home. There are appropriate strategies in place to obtain feedback from service users and their families. Records relating to service users are stored securely. Appropriate action has been taken to promote the health and safety of staff and service users at the home. EVIDENCE: The Registered Manager is Pippa Greed. She has several years experience of managing a care home, and is close to completing the NVQ level 4 qualification in Management. Due to the needs of the service user group, feedback is sought from service users on an individual basis. SENSE, the organisation that owns the home has recently organised a family weekend, where relatives were able to comment on the service provided. The home has appropriate policies and procedures in place to safeguard vulnerable service users. Fire safety records were examined. Fire equipment had been serviced and tested as required. There are regular fire drills and appropriate actions have been taken to meet the requirements from the Fire Officers visit. Portable appliances have been tested as necessary and the home has a satisfactory electrical hardwiring certificate. Weekly checks are made on the homes’ vehicle and hot water temperatures. Due to concerns regarding the health and safety of service users, the two pet cats have recently been re-homed. Accidents recorded and reported as required.
