Conditions of registration: 1. 1 person under the age of 65. Date of last inspection 16/11/05 Brief Description of the Service: Normanton Lodge is a care home able to provide personal care and support for up to 26 residents over 65 years of age. It may also offer care to 1 resident who may have dementia and 1 resident who may be under 65 years of age, but the maximum persons living at the home must not exceed 26 Normanton Lodge is located in a quiet residential area of Bognor Regis. Local amenities are within walking distance of the home and Bognor Regis seafront and promenade is a short drive away.There is a small car park at the front of the home and gardens to the rear which are accessible to residents. Communal areas consist of a main lounge with a small dining area and a separate dining room on the ground floor. There is an additional quiet sitting room on the upper floor.There are twenty-four single bedrooms and one double bedroom. Nine bedrooms have en suite facilities. This is an overview of what the inspector found during the inspection. This announced inspection took place over 5.5 hours on the 9th August 2005. The inspector arrived at 9.20 am and was welcomed by the manager of Normanton Lodge A full tour of the home took place and the majority of the 23 residents living at the home were met. Five residents were spoken with in depth to find out if they felt that their needs were being met. The inspector joined residents for lunch to sample the meal and meet other residents who were not spoken with privately. The care records of four residents were inspected during the visit along with other records, which showed how care needs are to be met. The records of four staff were also inspected and staff were spoken with informally during the visit, Eight service user comment cards were returned during the visit to the home all indicating that residents were satisfied with the facilities at the home and the standard of care provided. The inspector met with two visitors, visiting the home and gained positive feedback about the service provided. What the service does well:
Residents, who are able, are encouraged to retain an independent lifestyle, accessing local communal facilities and maintaining relationships. Care plans are clear and provide staff with the information, which they need to meet the needs of the residents. The staff team treat residents as individuals and are able to assist them to maintain their dignity. Meals are varied, nicely presented, offer choice and are served in pleasant surroundings. Visitors are made to feel welcome and find communication with the management and staff of the home open and friendly. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,4,5,6 A Service User Guide provides information about the home and the service, which it offers to enable prospective residents to make an informed choice about moving into the home. Residents are assessed prior to moving into the home to make sure that the home can provide a care plan which residents or their families know will meet their needs. Prospective residents, their relatives and friends have an opportunity to visit the home to look at the facilities available and assess its suitability before moving in. EVIDENCE: There have been some changes made to the Service User Guide, which provides information about the care home. Prospective residents can use this information to ensure that Normanton Lodge can offer the service, which they require. Residents spoken with during the visit explained how they had come to live at Normanton Lodge. Usually relatives had visited on the behalf of residents. Two residents having a period of respite care at the home were hoping to stay permanently. One resident said that they were “trying it out to see if they wanted to stay”.

Care records viewed at this visit showed that an initial assessment is carried out prior to moving into the home and then reviewed after moving into the home, to ensure that the residents needs can be met. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9 and10 Systems for care planning give clear information to assist with all aspects of health, personal and social care needs. Procedures for recording accidents are not always adhered to, which could result in residents risk assessments not being reviewed when not all falls are recorded. Medication at the home is well managed and ensures that residents are protected by the homes procedures for dealing with medication. Residents are treated with respect and their rights to privacy are recognised. EVIDENCE: Care plans examined were clear and gave staff the information, which they need to meet the needs of the residents in all aspects of heath, personal and social care. They had been reviewed monthly to show the changing needs of the residents. Daily progress notes are in use to monitor the heath and well being of residents and records examined showed significant events in residents daily lives. These records were used in a case tracking process for four residents to ensure that all accidents and falls sustained by residents were being recorded in the homes accident book. This process identified a shortfall in the homes procedures for reporting and recording accidents to residents by staff. A statutory requirement has been made to improve the practice so that

Visits to residents by other heath care professionals are recorded and outcomes detailed. One resident spoke about visits by the Doctor and Nurses whilst she had been ill. One residents care records viewed showed regular visits by a District Nurses to dress wounds. During the visit staff were observed to treat residents in a respectful and courteous manner, providing comfort and assurance to confused residents. Medication records were observed to have clear recording systems and medication was stored in a locked trolley and cupboards. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14and15 The home provides a lifestyle that respects privacy, dignity and choice so that resident’s expectations and individual preferences are met. Activities provided are varied to suit resident’s preferences and abilities. Residents are able to have visitors and are encouraged to be part of the local community. Meals are well managed and provide choice and variation. EVIDENCE: All residents spoken with during the visit were pleased to be living at the home as an alternative to their own home. One resident who had lived at Normanton Lodge for over three years commented that they “love it here its my second home.” Another resident said that they had moved to this home because they had not liked it at a previous care home. The resident showed the inspector their bedroom and talked about the personal items, which they had been able to bring to the home. This resident explained that they liked this home because it was “homely and everyone is friendly”. Routines at the home were observed to be flexible. When the inspector arrived at 9.20 am some residents were already up and dressed others had chosen to have a lie in. The atmosphere was noted to be relaxed. Residents are encouraged to maintain contact with their families and friends. The visitors book records regular visitors to the home and trips out by residents with relatives and friends. During the visit one resident went out for lunch with a visitor and two other visitors spoke with the inspector about their views of the

They said that they had been very impressed by the care and attention from staff at the home. Comments about food at Normanton Lodge both during the visit and in comments cards returned to the inspector was good. Residents spoken with said that the food is of a good standard and if they didn’t want what was on the main menu for the day they could always have an omelette or ask for something different. For tea there is also a choice. Staff were observed to ask residents during morning coffee what they would like to eat at teatime. A resident told the inspector that this was the normal procedure. The inspector joined a group of residents for the mid-day meal, which was ham, egg and chips followed by apple pie and custard or a yogurt. Everyone enjoyed the meal and there were compliments for the cook when she came to collect the empty plates. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 There is a clear complaints procedure, which enables those using the service to have the confidence that their complaint will be responded to within a maximum of 28 days. Residents are protected by the homes adult protection procedure and staff have attended training in how to protect vulnerable adults. EVIDENCE: The home has a complaints procedure included in the Service User Guide and on display on the notice board in the entrance hall. There have been no complaints made to the home or to the Commission for Social Care and Inspection (CSCI) since the last visit to the home. Information provided prior to the visit to the home confirmed that Normanton Lodge has its own policies and procedures for responding to suspicion or evidence of abuse or neglect in addition to the West Sussex Multi-Agency Policy for protecting vulnerable adults from abuse. Staff training records viewed showed that training has been provided for some staff in February 2005 to ensure that those in daily contact with residents are aware of their responsibilities in recognising and reporting abuse. Care staff that have completed an NVQ in care have completed Unit Z1 (contribute to the protection of individuals from abuse) as part of their qualification. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25 and 26 The home is clean and residents have a comfortable, homely environment to live in. Bedrooms are comfortable and meet the needs of the residents. Residents are encouraged to contribute to making their bedrooms their own by having their own personal possessions around them. Vulnerable residents are at risk from excessive hot water temperatures, which could scald them. EVIDENCE: The inspector visited all of the communal and private accommodation during the visit to the home. The home was observed to be clean and well maintained. There is a patio/garden to the rear of the building, which residents can access. Communal areas are nicely decorated, comfortable and have many homely touches. Residents visited in their rooms commented upon the personal items, which they had been able to bring to the home and have in their bedrooms. Some resident had brought small items of furniture from home. There is specialist equipment in bathrooms and toilets and a new hoist.

Hot water temperatures were sampled during the tour of the home. Five bedroom wash basins and one of the baths sampled, had water leaving the hot water taps in excess of 50 degrees centigrade, this places vulnerable people at risk. Records viewed by the inspector showed that the maintenance person had been regular checking hot water temperature since the last inspection at the home because of a problem identified at that visit. The most recent check has been the day prior to this inspection. Comment was made that the water had been tuned off since the check, for boiler maintenance. A requirement has been made that steps are taken to avoid residents been at risk of scalding from excessive hot water temperatures. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29 and 30 Staffing numbers are set at a level, which allows residents assessed needs to be met. Procedures for the recruitment of staff are not robust and do not fully safeguard and protect residents at the home. Staff receive training to support them in carrying out their jobs and meeting the needs of residents. EVIDENCE: Staffing rotas observed prior to the visit show that sufficient staff are employed with the appropriate skill mix to meet the needs of residents over the 24-hour period. Residents and staff confirmed that there are two staff on duty at night. Residents felt there are usually enough staff on duty. Staff records show that 60 percent of care staff now have an NVQ level 2 or above which meets the 2005 target set. The staff files of four members of staff were fully inspected during the visit and other records were viewed to make sure that Criminal Record Bureau (CRB) and Protection of Vulnerable Adult (POVA) clearance is sought for all staff. Records showed that two staff did not have the results of checks on file. The CRB for one person had been sought by the person’s previous employer and this did not include a POVA check. There was no identification for this person available to the inspector. A CRB application had been made for the other person but there were no written references on file. The manger was reminded that there is a statutory requirement on care providers to check if workers, which they want to appoint, are included on the POVA list and that this can only be done by a new CRB application. This process should be concluded prior to staff starting work at the home to protect residents.

Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,37and 38 The home is well managed providing leadership and guidance for staff. Some practices do not promote and safeguard the health and safety of residents living at the home EVIDENCE: Residents feel that the home is well managed. One resident said, “the manager is very kind and very special”. The registered manager has more than two years experience as a manager and has worked at the home for many years. She has now completed an NVQ Level 4 in Care Management. There needs to be some improvement in recruitment practice at the home. Not all staff records sampled had sufficient evidence to show that residents are protected by the homes recruitment practices. Staff are not entering all accidents sustained by residents a in the homes accident book, although there is evidence that they may be being recorded in care records and other daily records kept by the care staff. Vulnerable residents are at risk from excessive hot water temperatures in some hot water outlets.
