Conditions of registration: Of the 50 (fifty) service users accommodated under the category of OP, up to 7 (seven) may fall within the category DE(E): Dementia (older persons). Of the 50 (fifty) service users accommodated under the category OP, 3 (three) may fall within the category PD(E): Physical disability (older persons). The age/age range of the persons to be accommodated will be: OVER 65 YEARS OF AGE. Date of last inspection 10/11/04 Brief Description of the Service: Nower Care is a care home for fifty service users located on the outskirts of Dorking Town. The home provides personal care in a homely and comfortable atmosphere. Service users accommodation is offered in single bedrooms some of which have en-suite facilities. There is ample communal space available which includes six lounge areas. The home has been undergoing extensive refurbishment in recent years to enhance service users accommodation. This building project has reduced the garden facilities on a tempory basis. Leisure activities and trips out to the local amenities and places of interest are also provided. This is an overview of what the inspector found during the inspection. This inspection was announced and the first in The Commission for Social care Inspection programme year 2005/2006. The inspection was undertaken by Mary Williamson who is the lead inspector for the service. The registered manager Wendy Sharples was present throughout the inspection and Chris Horwood the general manager was present for the latter part of the inspection. Approximately 60 of the service users and 15 of the staff were spoken to during the inspection. Five service user comment cards and two relative comment cards were also received. A tour of the premises was undertaken and records relating to the care of the service users and the management of the home were examined. The home was functioning well and the service users were engaged in individual and group activities overseen by the leisure activities co-ordinator. Interaction between the service users and the staff was observed to be respectful and positive. The staff recruitment procedures and staff training programmes were not inspected during this visit. The inspector would like to thank the service users, staff and the management team for their helpful and positive approach to the inspection process. What the service does well:
The home provides good quality care to service users as identified in individual care plans. The interaction between the service users and staff is professional, respectful and caring. The management team are currently working to keep disruption to a minimum while the refurbishment of the home continues. Social and recreational activities are varied and are provided on a regular basis. Service users records are generally well maintained and computerised. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 4, and5. The home was found to be operating effectively in respect of these standards. The quality of the information available to prospective service users was good and indicated that it would help service users and their families make an informed choice about the home. EVIDENCE: The home has a statement of purpose and service users guide in place, providing prospective service users, their families and designated representatives with the information necessary to make an informed decision about living in the home. The service user guide also includes an application form to be completed prior to admission. This provides the home with the initial information, which includes the diagnosis of the service user, enabling the home to determine if their needs can be met. Contracts of terms and conditions of occupancy are in place and outlines all the relevant charges and care to be provided. These contracts are signed by the service or their designated and a copy is retained on file. The home has developed it’s own assessment tool for this purpose. The manager stated that trial visits are encouraged whenever possible, which may be for coffee or a meal. Relatives may visit on behalf of the prospective service user if they are unable to do so for themselves. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, and 10 Evidence gathered during the inspection confirmed that the home meets each of the assessed standards, with the exception of standard 7. The home was able to demonstrate that the service users health and personal care needs were being appropriately met. EVIDENCE: Individual care plans are in place. These are written with the input from service users, their relatives, information gathered from the pre admission needs assessment, and any other information relevant during the admission process. All the care plans are computerised, and are updated daily. The information is then cascaded to the care staff during daily handovers. The printed copy of the care plan must be signed by the service user, or a designated representative. There was evidence that risk assessments are also carried out, for example moving and handling, risk of developing pressure sores, and nutrition assessment, which are also included in the care plan. Service users have access to appropriate health care services. They are all registered with a local GP and are seen on a regular basis. There is also input

Visits from the chiropodist, dentist and optician are also arranged. Staff were observed to interact with service users in a positive and respectful manner. They were seen to address service users politely and explain their actions when giving care. The staff were observed to knock on bedroom and bathroom doors prior to entry. The home has a policy in place relating to privacy and dignity and staff have training in this policy during induction training. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, and 15 Evidence gathered during the inspection confirmed that the home meets each of the assessed standards. This indicated that service users social, recreational and dietary needs are being met. EVIDENCE: The home employs an activities co-ordinator who organises activities on an individual and group basis. Art and craft, aromatherapy, music and movement, general knowledge quiz, board games, and card games are an example of what is on offer. Outside entertainment is also provided at least once a month. Trips to the local theatre and shopping outings are also arranged. Service users spiritual needs are also supported and various local clergy are made welcome in the home. Relatives and friends are encouraged into the home at any reasonable time. They are also involved in the care planning process and informed of any changes that occur. The kitchen was visited during the inspection. The cook explained that the menus are planned over a six-week period. There was a list of service users dislikes kept in the kitchen for information.

It was only when the inspector asked that enquiries were made to the kitchen and the lunch menu was written on the board in the dining room. Lunch consisted of chicken pie, a selection of fresh vegetables, boiled and mashed potatoes, followed by fresh fruit salad and ice cream. The food offered was nutritious, wholesome and appetising. Two service users commented that the food was adequate but unimaginative while others commented that the food was very good. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, and 18 Evidence gathered during the inspection confirmed that the service users are appropriately protected and that views are listened to but not recorded. EVIDENCE: The home has a complaints procedure in place and is available to all service users and their families on admission to the home. The home does not keep written records of complaints received. Currently the home is dealing with two complaints from service users who are unhappy about having been moved to other rooms to accommodate the building project in progress. The management have been requested to keep The Commission for Social Care Inspection informed of the outcomes and a requirement has been made accordingly. There is an abuse awareness policy available and both the registered manager and general manager confirmed that all staff have training in abuse awareness during their induction period. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, and 24 The evidence gathered during the inspection indicates that standards 21, 22, and 24 were met and there are shortfalls relating to standards 19, and 20. EVIDENCE: The home is suitable for its stated purpose. It has been developed and improved in recent years and the current building project has linked both buildings by a central reception area. New en-suite bedrooms have been constructed to enhance the space standards. The home provides ample communal facilities, which includes six lounge/quiet rooms. The hallway at the main entrance needs redecorating. Due to the ongoing building work, access to the garden is limited and this was a negative comment by several service users. It is recommended that an action plan outlining timescales for the provision of suitable garden facilities is sent to The Commission for Social Care Inspection within the next month.

Some have been adapted to meet the mobility needs of the service users. There is access to a physiotherapist and occupational therapist to provide specialist advice for mobility equipment to meet individual and collective assessed needs. Ramp access is available from the new reception area to the front of the home. Service user accommodation is provided in single rooms. The standard of decoration and facilities varies from been refurbished with en-suite facilities to very basic accommodation on the Dyson Wing. The staff and relatives are committed to ensuring that the bedrooms are kept as individual and personalised as possible. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 37, and 38 Evidence gathered during the inspection indicates that the home is well managed in the best interests of the service users. The standard of record keeping is generally good, with three shortfalls highlighted in previous standards. EVIDENCE: The home is well managed by a registered manager with considerable experience in the provision of care to older people. She also has achieved her NVQ level four award in management. There is also a general manager in post who oversees the administration and current building project in the home. The standard of record keeping is generally good. There was however no menus available to service users for information on the day of the inspection, there was no printed copy of a care plan in place for one service user, and the home does not keep a written record of complaints received. There is a wide range of health and safety policies and procedures available and these were seen throughout the inspection. Risk assessments are in place for safe working practice. The fire safety records were seen and are well maintained. There are contracts in place for regular servicing of the fire fighting equipment and fire alarm systems.
