Date of last inspection Brief description of the care home Nyton House is a care establishment registered to provide accommodation for up to twenty-three service users in the category OP (old age not falling in any other category) and one named person in the category DE (E) over sixty five years of age. Care Homes for Older People Page 2 of 10 0 6 0 4 2 0 0 9 Brief description of the care home The establishment is situated in the village of Westergate. Local bus routes are near by. Accommodation is provided on ground, first and second floor level. A vertical lift services each floor. All rooms are generally for single occupancy however there are five rooms that can be used as doubles providing the occupancy levels do not exceed twenty-three. The service is privately owned. The proprietors are Mr and Mrs Davis. The current fees range from #450 to #850. (#850 is for a double room) Care Homes for Older People Page 3 of 10 What we found: We visited the home on 23 June 2009 to check compliance with a notice served around medication concerns. You were required by 17 June 2009 to; Have a system in place that records the date and quantity of all medication received into the Home, which ensures that the same are signed and dated by the person responsible for checking in the new medication. Ensure your records clearly identify, risk assess and manage those service users who are self medicating, including maintaining records of when, who and what medication was passed to the service user to self administer. Have a system in place that accurately records all medication that is administered, the dosage, the times [if variable] refusal or unable to take medication and the signature of the person administering the medication. Ensure there are written instructions provided to care staff that inform them of the protocol for administering all as required medication and medication that is for symptom relief such as pain relief medication. Have a system in place that ensures the medication is administered as prescribed and that there is a safe practice for the changing of prescribed dosages and instructions for medication. This must include the date, two signatures and evidence of who gave the instruction (for example the G.P.) We found that the notice has been met. There is a new medicine supplier who has supplied two trolleys for use in the home. Medicine received is recorded on the medicine administration record chart. This is signed and initialed. People who are self medicating have a care plan which includes a risk assessment and a procedure. The consent form will be signed when a relative is present. Relatives have been invited to a meeting for this purpose. A few gaps were noted on the MAR chart however steps have been taken in the right direction to have true and accurate records around medicine use. There was training for staff on medication in early June. There were written instructions in the care plan on the use of medicines to be used only when needed so that there is guidance for staff. Where the dosage directions were changed, evidence was seen either in the care notes or on a new label giving information of who authorized this change. No. Standard Regulation Requirement Timescale for action 1 8 13 (1)(b) Individuals who have been 15/07/2008 identified with potential risks to their pressure areas or with actual skin breakdown must have a risk assessment in place, be reviewed by the GP and the care plan updated to include how the risks are to be reduced and specify how wound care needs are to be met. Where it has been identified 15/07/2008 that people who use the service are at risk from falls, a risk assessment must be put in place to lessen those risks. 13 (6) The registered manager must ensure staff receive training in safeguarding adults.
