Conditions of registration: Date of last inspection 8th August 2005 Brief Description of the Service: Oakdene is a large detached property providing accommodation on two floors. Oakdene is a residential service provider, providing care and support for up to 26 Older People. The owner manages the home, and she employs a deputy manager, 4 senior carers, 5-day care staff, and 5 nights care staff. In addition to this the home employs two cooks, 2 kitchen assistants, a housekeeper and a cleaner. The Registered Owner/Manager, Mrs Tracy Gobbi has several years experience as a manager and owner of residential care homes for the elderly. The home is located on the main road in Sheerness (Minster on Sea). It is within walking distance of public transport, (bus) and has local shops and facilities nearby. This is an overview of what the inspector found during the inspection. The Unannounced inspection was carried out on the 20th December 2005. The inspection consisted of a discussion with the Manager, looking at finance and medication procedures and the progress made on the Requirements from the announced inspection of the 8th August 2005. The Manager had checked through all of the staff files to ensure that they contained 2 references and all the documentation set out in the Schedule 2 of the Care Home Regulations 2001. The recording and storage of finance was checked and found to minimise the risk of financial abuse. The Inspector was satisfied that the storage and recording of administration of medication met with the National Minimum Standards. The Manager informed the Inspector that the home had accepted a Resident on a 2 week trial period as the assessment was not conclusive that the home could meet the individuals needs, during the trial period it became apparent that the placement was not meeting the individuals needs. The Manager has telephoned and faxed the funding authority asking them to find an alternative placement. The Manager is concerned that she will be in breach of the regulations if the funding authority do not respond to her request. What the service does well: What has improved since the last inspection? There is nothing the home can do about the problems with local doctors surgeries regarding the use of locum Doctors, it is difficult for the Residents continuity of medical care as they are seen by a different doctor on most occasions.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,4, Residents have the information available to make an informed choice. Residents enter the home with on clear agreements and their representatives are informed. EVIDENCE: The home has a clear Statement of Purpose and Service User Guide that is given to the prospective Resident, family and/or their representative. The home carries out an assessment of need and gains information from the funding authority, recently the home carried out an assessment following a referral but felt that the individual care needs may be better met at a nursing home, the funding authority carried out another assessment and claimed the person was suitable for a residential placement. The Manager following a discussion with the funding authority agreed to a 2 week trial placement, it became apparent that the home could not fully meet the needs of the individual as they required nursing care. The Manager has contacted the funding authority about finding an alternative placement as the local nursing home provision is full. The Manager has telephoned and faxed requests for the placement to cease as the individuals needs are not being fully met. The Manager is keeping the Inspector updated of the situation.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8,9,10 Resident health needs are fully met. Residents are treated with respect and their right to privacy upheld. Residents who self medicate are protected by the homes policies and procedures. EVIDENCE: Residents are all registered with a doctor and attend the Community Dentist Department at the local hospital, this also includes a yearly visit to the home by a Dentist. Residents also have access to a local optician. None of the Residents self-administer medication, the home has policies and procedures in place to cover this if it is required in the future. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13,14,15 Resident maintain contact with family and friends. Residents are able to exercise choice and control over their lives. Residents have a healthy and nutritious diet. EVIDENCE: The Manager said she will continue to encourage Residents to join in and organise outings from the home. On the 14th December 88 children from the local Primary school visited and sang Xmas carols for the Residents and on the 21st December a trip out to see the Xmas lights has been arranged. Family and friends are able to visit the home at any time, there is a lounge area where they can have some privacy or if they wish they can use the Residents room. Residents are asked what they would like to do, or if they wish any assistance and support. The dining room tables were well laid out and in pleasant surroundings, and the menu was on display by the main door and was 2 weeks in advance. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Complaints are taken seriously and acted upon. Residents are protected from abuse. EVIDENCE: Since the last inspection the home or the Inspector or the home have not received any complaints. Since the last inspection 2 staff have attended an Adult Protection training course. Medication storage and procedures were looked at and found to be in order, the Medication Administration Records were all signed and up to date including the morning of the inspection. Access to the medication is through the Senior member of staff on duty, there are only 1 set of keys available to the medication cabinets. There are 2 main medication cabinets bolted to the wall, 1 for blister packs and the other for liquids and creams, there is also a lockable drugs fridge, these are all in a lockable room. The Inspector noted that all creams and liquids were individually named with dispensing labels and instructions, they had also got hand written dates of when they were received into the home. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,25,26 Residents live in a safe, comfortable well-maintained, clean and hygienic environment. Residents have access to safe indoor and outdoor communal facilities. Residents have access to sufficient and appropriate baths and lavatories. Residents have specialist equipment that will maximise their independence. EVIDENCE: Residents live in a safe and well-maintained home, there are no en-suites in the home but there are 6 toilets and 3 baths. The Residents have access to specialist equipment to assist with increase independence such as Zimmer frames, wheelchairs, grab rails, hoists and stair lift. The parts of the home seen were clean and free from offensive odours and was safe. Residents live in comfortable surroundings with ample communal space both internally and externally. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 Residents needs are met by the numbers and skill mix of the staff. Residents appear to be in safe hands at all times. Residents are protected by the homes recruitment policy. Trained and competent staff care for residents. EVIDENCE: A team of competent and trained staff meets the needs of the Residents. Training carried out since the last Inspection includes; Manual Handling on the 7th October 2005, 1st Aid on the 20th October 2005 and 2 staff attended Adult Protection training in late August 2005. The home also uses a competency training booklet covering Administration of medicine, Nutrition, Health and Safety, Aggression Management, Basic Care and Dementia, these are certificated courses. The home is in excess of the required 50 of staff with NVQ level 2, one member of staff has started their NVQ level 4 in care. The individual staff member references that had been misplaced had been located and placed in their staff file. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,35,38 Residents live in a home that is managed by a person fit to be in charge. Residents finances are protected by the procedures of the home. The Residents health, welfare and safety are promoted and protected. EVIDENCE: Residents live in a home that is managed by a fit person, it was apparent from the discussion with the Inspector that the Manager was competent and confident in carrying out their responsibilities. The Inspector looked at finance records and procedures, most of the money held had come from individual Residents families. Records showed money coming in the expenditure with receipt and signature, most of the expenses were for hairdressing and newspapers.
