This inspection was carried out on 17th October 2005.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

There was a warm and friendly atmosphere in the home. Staff were observed to be speaking to residents and providing support when required. Residents spoken to said they were happy in the home. One resident said, "The staff are caring". Another resident said "The manager is marvellous, she will do anything for you". Progress has been made in the further development of care plans. Although training was not looked at in depth during this inspection, staff spoken to confirmed that they have attended a range of training and development. Staff confirmed that they received appraisals and regular supervision and stated that they felt supported by the manager who has made some positive changes in the home since she commenced. Investors In People have visited the home and a recommendation was made that the home is put forward to receive the award.

What has improved since the last inspection?

Care plans were sampled and progress has been made in completing action plans in response to any risks identified during the assessment process. Since the previous inspection a meeting was held between the responsible individual, registered manager and the Commission for Social Care Inspection to discuss a previous requirement in respect of staffing levels provided in the home at nighttime. The outcome was that the home may continue with its present staffing levels but this is to be continuously monitored and reviewed. Progress has been made in updating staff personnel files to ensure that the required documents are in place. All police checks have been updated. The home still needs to complete all files to ensure that this standard is fully met. Safety devices have been replaced and fitted for bedroom windows.

Total number of beds not to exceed 57. Date of last inspection 21st June 2005 Brief Description of the Service: Oakhurst Court is a large detached property that has been converted to provide accommodation for fifty-seven service users. The home is located in a rural part of South Godstone. Access to shops, church, public transport and other local amenities are a distance away, however transport is provided for service users when required. Part of the accommodation is provided in the original building, with the rest in a number of annexes surrounding the main building. These have been added during a continuous upgrading programme. Accommodation is provided on the ground and first floors and most bedrooms have en-suite facilities. There are two passenger lifts accessing the first floor. The home has a number of sitting areas including a large main lounge area. There is a large dining area which is used for activities as well as serving meals. The home is located in substantial grounds with some of the gardens landscaped and a river runs close to the main entrance. All areas have wheelchair access. There is ample parking available to the front of the home. This was the homes second inspection carried out in 2005/2006.The unannounced inspection took place over six hours and was carried out by two inspectors. The focus of the inspection was to look at any requirements at the last inspection and to check other required minimum standards. A full tour of the premises was undertaken and care plans, staff files and policies and procedures were sampled. The inspectors spoke to a number of residents and three members of staff in the home. The inspectors would like to thank the residents and staff for their cooperation during this inspection. What the service does well: What has improved since the last inspection? What they could do better: Care plans were sampled and a requirement was made that plans need to be signed and dated by staff who are completing the evaluation. On the day of the inspection it was seen that a number of residents had stair gates installed in their bedroom doorways. A requirement was made that risk assessments are completed and the permission is to be gained from residents and the outcome recorded in individual plans. A recommendation was made that the unused medication in the controlled medicines cupboard should be returned back to the pharmacy. The home presently has vacancy for an activities coordinator, which has been advertised it, was recommended that the appointment to this post be continued to be actively pursued. A complaint procedure was available in the home but a recommendation was made that this updated. Written procedures were in place for the protection of vulnerable adults. However one incident that has occurred was not reported appropriately following the local authority protection of vulnerable adult procedures and was not brought to the attention of the Commission for Social Care Inspection. An immediate requirement was made that all incidences must be reported promptly following the local authority procedure and that notice must be given without delay to the Commission for Social Care Inspection of any event in the care home which seriously affects the well being and safety of any residents. During a tour of the home some environmental issues were identified as requiring action. An upstairs bathroom needs to have an internal door replaced to separate the toilet and bathroom to maintain privacy, as there were two entrances into the bathroom. Adequate storage is needed for the storage of bedpans. Some of the bathrooms were found to have lotions, Oakhurst Court Nursing Home DS0000013343.V257183.R01.S.doc Version 5.0 Page 7 creams, and toiletries in the bathroom cabinets and these need to be removed and creams are to be discarded if they are no longer used and individual toiletries belonging to residents should be maintained in their bedrooms. In one lounge a resident had disposable pads placed on the floor under her chair. A requirement was made that this practice is stopped and this should be replaced with extra cleaning to ensure the dignity of residents. Furniture in one bedroom needs replacing to ensure that residents have a comfortable room to stay in. A further recommendation was made that the registered manager should consider arranging for an occupational therapist to visit the home to carry out an updated assessment of equipment provided in the home. The manager is still relatively new and a strong recommendation was made that she is able to work extra to the staffing establishment to enable her to complete her management tasks and responsibilities. A strong recommendation was made that all staff should attend training in dealing with violence and aggression to ensure that they have the acquired knowledge to support residents appropriately. During the inspection some health and safety issues were identified and action was required by the Commission for Social Care Inspection to ensure these issues are responded to and completed. The water was found to be hot and an immediate requirement was made that the temperature is checked and that regular monitoring of the water takes place in order to reduce the risk of scalding. The staff toilet should be locked at all times to prevent residents being able to access the taps to ensure safety. Boiler cupboards should be kept locked at all times to ensure the health and safety of residents. The fire logbook was sampled and records were maintained appropriately. However an updated fire drill should be carried out and a storeroom was found in need of clearing. A further requirement was made that an updated electrical test is carried out on one television to ensure the health and safety of residents. Accident records were sampled and records were adequately maintained. A recommendation was made that the time and the occurrence of the incident is recorded in the logbook. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 & 9 Each resident is provided with an individual care plan, which details the individual’s health, personal, emotional and social, needs. Further work is required in ensuring that risk assessments and the need for stair gates are completed. Residents are protected by the homes policies and procedures for dealing with medicines. EVIDENCE: Care plans were sampled and were detailed and comprehensive, however plans were not signed or dated by staff when reviews take place. While touring the home it was evident that some residents had stair gates installed in their bedroom doorways. A requirement was made that risk assessments and the outcome of the need for the use of gates should be clearly recorded on the individual plan. Medication and administration records were maintained adequately. Photographs of individuals were available with records and a staff signature list was in place for staff that are trained to administer medication. Medicines were stored appropriately. A recommendation was made that some medicine was found in the controlled medicine cupboard should to be sent back to the pharmacy. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 & 15 The home is able to demonstrate that residents have access to a range of recreational activities but need to appoint to the vacant activities post. Residents are offered a well balanced diet. EVIDENCE: There is presently a vacancy for a day activities coordinator and this post has been advertised. Care staff are currently supporting individuals with activities. Games are organised and manicures are carried out. A lady visits the home to play the piano and a sing-along is held. A garden party was held in the summer and this was evident with a range of photographs on display. A Christmas function is being arranged. It is recommended that the activities coordinators post be filled to ensure that this standard is fully met and that the quality of provision for residents is enhanced. The lunchtime meal was seen which was relaxed and unhurried. The meal was well presented and was nutritious. A choice of meals is available and alternatives can be offered. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17 & 18 Residents and relatives are aware of the homes complaint procedure. The legal rights of residents is protected The staff team need to ensure that it responds to the protection of vulnerable adult policies to ensure that residents are protected from abuse. EVIDENCE: A complaints procedure is in place. A recommendation was made that this needs to be updated. There has been one complaint received since the previous inspection, which was documented. However due to the nature of this incident the requirement to bring this to the attention of the Commission for Social Care Inspection was not followed. Residents were supported to take part in voting in the political elections. Adult protection procedures were in place. However there had been a delay in reporting an incident or seeking advice following the local authority protection of vulnerable adult policy. A requirement was made that the registered manager and staff should report any incidents or allegations that fall under the protection of vulnerable adults immediately to the local authority and to the Commission for Social Care Inspection. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20,22,24,25 & 26 The home was generally well maintained but some items were identified as requiring action. Residents have bedrooms that suit their needs. One bedroom was identified as requiring action. The home was clean and hygienic. EVIDENCE: The home has a rolling refurbishment programme in place. A full tour of the premises took place. The home was generally, adequately maintained and furnished. However an upstairs bathroom needs to have an internal door replaced to separate the toilet and bathroom as it was seen that this bathroom had two entrances. Adequate storage is required for bedpans as these were not stored appropriately. Some of the bathrooms were found to have lotions, creams, and toiletries in the bathroom cabinets. A requirement was made that these are removed and creams are to be discarded if they are no longer used and individual toiletries belonging to residents should be maintained in their bedrooms. It was observed in one of the lounges that a resident had disposable pads placed on the floor under her chair. Resident’s bedrooms viewed on the day of the inspection were decorated with individual’s belongings, which were seen on display. Residents are able to bring their own furniture into the home. However a requirement was made in respect of one bedroom to replace the bed and furniture. This is to ensure that residents have comfortable and well-presented bedrooms to stay in. A recommendation was made that an updated assessment is carried out by an occupational therapist to look at the equipment provided for residents. A visit has been undertaken by environmental health and this report was sampled with evidence to support that actions required have been responded to. The home was clean and hygienic. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28 & 29 The staffing levels in the home were sufficient to meet the needs of the current residents. The home needs to continue to update the staff personal files to ensure that residents are protected by the homes recruitment procedures. Staff are trained to carry out their jobs. EVIDENCE: On the day of the inspection there were two registered nurses and nine carers as well as the registered manager. It was concluded that the staffing levels in the home were satisfactory. However it was strongly recommended that the manager works supernummary to the staffing establishment to ensure that she is given the opportunity to carry out her management responsibilities. Since the previous inspection a meeting was held with the responsible individual, registered manager and the Commission for Social Care Inspection to discuss the requirement in relation to staffing levels on night duty. It was agreed that the present arrangements of one registered nurse and four carers can remain but with monitoring in place. The registered manager has made progress in ensuring that the information required as set out in the Care Homes Regulations 2001 is being completed. Proof of police checks was available. Further action has been required by the Commission for Social Care Inspection to ensure that the outstanding files are completed. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37 & 38 Residents and staff were supported by the manager. Staff receive regular formal supervision. Policies and procedures are in place but the registered manager needs to ensure that all staff are aware of their responsibilities in relation to the policies and procedures in the home to ensure that residents rights and interests are safeguarded. The registered manager needs to address health and safety issues identified to ensure that the health, safety and welfare of residents is protected. EVIDENCE: Staff and residents spoken to stated they liked the manager and that she was supportive. One resident said “The manager is marvellous and she will do anything for you”. The manager stated that a visit has taken place by the Investors in People and a recommendation has been made that the home is put forward to receive the award. Oakhurst Court Nursing Home DS0000013343.V257183.R01.S.doc Version 5.0 Page 18 A recommendation was made at the last inspection that the registered manager should consider implementing a relatives survey questionnaire this has not been completed. A further recommendation was made that this should be given consideration. Although staff supervision was not looked at in detail during this inspection staff spoken to confirmed that they receive regular supervision with their line manager or a supervisor. There was a range of policies and procedures in place. The registered manager needs to ensure that all staff are aware of the policies and procedures in relation to the protection of vulnerable adults and incident reporting to protect the health, safety and welfare of residents. A recommendation was made that the registered manager needs to continue updating the homes policies and procedures. Accident and incident records were sampled and were adequately maintained. A recommendation was made that the time and the place of occurrence should be recorded on the logbook. One incident in the home had not been reported to the Commission for Social Inspection. A requirement was made that the Commission for Social Care Inspection must be informed without delay any event in the home, which affects the health, wellbeing or safety of any resident. The water was found to be too hot and an immediate requirement was made that the temperature is checked and that regular monitoring of the water takes place to reduce the risk of scalding. STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1 Standard OP7 Regulation 15(2)(b) Requirement Risk assessments must be completed in respect of the use of stair gates in resident’s bedrooms. All decisions reached must be agreed with the resident, their relatives/representatives and significant others including (care manager, GP, district nurse where appropriate). And all decisions reached documented in the residents care plan. The registered manager must ensure that all care plans are regularly reviewed and all care plans must be signed and dated at the time of each review. The registered manager must ensure that a bed and furniture is replaced in one bedroom to ensure that residents have a comfortable and pleasant rooms to stay in. The registered manager must ensure that the outstanding staff personnel files are completed. (Previous timescale of 21st July 2005 not met.). This is to ensure that residents are protected by the homes recruitment procedure.
