Conditions of registration: Date of last inspection 21st February 2005 Brief Description of the Service: Oakland Grange is a privately owned care home registered to provide accommodation for up to fourty two service users in the category OP (old age, not falling into any other category). The home is situated in the village of Littlehampton. Littlehampton shopping centre, train and bus services are nearby. The home occupies three floors with a vertical lift servicing each floor, The majority of rooms are for single occupancy, however some rooms are double rooms, which can be shared if requested. All rooms have en-suite facilities. There is a large and well maintained accessible garden at the rear of the home. This is an overview of what the inspector found during the inspection. This was an Announced Inspection which took place over a period of eight hours. In preparation for the inspection the Responsible Individual provided detailed information as requested on a variety of aspects of the services provided. In addition comment cards were received from a number of current residents. The Inspector reviewed previous inspection reports and analysed any correspondence or information received since the last inspection. The Inspector considers that standards of care at the home are high and are consistently maintained. This is supported by positive feedback received from residents during the inspections. The home provides a welcoming and homely atmosphere and residents spoke very highly of the care provided by the staff team. Comments made by residents included, ‘I am blissfully happy’, ‘its like heaven’, ‘I’m glad I moved’ and ‘I’m very happy, the staff are very good’, and ‘I feel safe and secure here.’ What the service does well: What has improved since the last inspection?
The quality assurance procedure and monitoring has been extended and improved. A newsletter for residents has now been introduced and will be produced quarterly. Works to the garden have provided more interest for residents. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 The Registered Manager or senior staff member carries out an assessment of individual needs for all potential residents prior to them moving into the home. EVIDENCE: The records of three residents who have recently moved into the home were seen. These showed that they had all received a pre-admission needs assessment. A standard format is used for this purpose which covers all aspects of personal, health and social care needs. Some of the residents spoken with during the inspection were also able to confirm that they had discussed their care needs prior to moving into the home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10. All residents have a care plan in place which sets out all aspects of their health, personal and social care needs. The health care needs of residents are promoted and maintained. Residents are able to take responsibility for their own medication where appropriate. All residents are protected by the homes policy and procedure for the administration of medication. Residents are treated respectfully and their privacy is upheld. EVIDENCE: A sample of care plans was seen during the inspection. These are kept within the residents’ own rooms. All residents spoken with were aware of their care plans and confirmed that these had been discussed and agreed with them. They are reviewed at least monthly and amended as appropriate. Health care needs are monitored and any changes in health are recorded. Residents are encouraged and supported to make their own decisions as long as they are able to do so.

Appointments with doctors and other health care professionals are recorded along with the outcomes of these. All residents who completed comment cards advised that they felt well cared for in the home. There is a policy and procedure with regards to the administration of medication in place. Named staff only, administer any medication and during the inspection they were observed to do so in accordance with the homes procedure. Residents are able to self-administer their own medication and lockable storage for this, is available within their rooms. During the inspection staff were observed to respect the privacy and dignity of residents. All residents spoken with confirmed that they considered that their privacy was respected by staff and this was supported by the completed comment cards in which 100 of respondents advised that they felt their privacy was respected. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 15 Routines of daily living are flexible and a number of activities are arranged for residents to participate in if they wish to do so. A varied, appealing and nutritious menu is available within the home. EVIDENCE: Social interests and hobbies were seen to be recorded within individual resident care plans or on pre admission assessments. A number of residents spoken with are independent and able to pursue their own interests. Within the home activities such as art and craft, music, bingo, entertainers and some outings are arranged. All residents spoken with during the inspection were happy with the activities provided. Of residents who completed comment cards 63 were satisfied with the provision. However residents are given opportunities through meetings and quality assurance procedures to provide feedback on the services provided within the home. Oakland Gange H60-H11 S14646 Oakland Grange V230678 250705 Stage 4.doc Version 1.30 Page 12 Menus provided show that a variety of nutritional meals are served within the home with a choice of three dishes at both lunchtime and suppertime. Lunch is always a three-course meal. The lunch served on the day of inspection was well presented and looked appetising. All residents confirmed that they had enjoyed the lunch. Of the residents who completed comment cards 63 advised that they liked the food with 27 saying that they liked the food sometimes. One resident advised that her son who had viewed the home prior to her admission had been able to sample the meals as part of this visit. She advised that he had recommended the food to her and that he was quite right. Other residents advised that their relatives and visitors can also stay for meals on request. Discussions with the chef demonstrated that she had a good awareness and understanding of personal likes and dislikes and tried to incorporate these into the daily menus. It was clear through these discussions and observations made that the chef serves fresh fruit and vegetables whenever possible, and special diets can be accommodated. JUDGEMENT – we looked at outcomes for standard(s) 16 There is a clear and simple complaints procedure in place. EVIDENCE: There is a policy and procedure in place with regard to complaints. One complaint has been received within the last 12 months and this was dealt with appropriately. All residents who completed comment cards advised that if they were unhappy with their care that they knew who to speak to. During the inspection residents advised that they did not have any complaints with regards to their care and that they knew that Mrs Greenhill as Registered Manager was available to speak with them if they wished to do so. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 24, 25. The home provides a safe well-maintained environment The home provides a range of both internal and external communal space which is accessible to all residents. Residents are able to furnish their own rooms as they wish. Works are still in progress with regards to heating and hot water provision to fully meet a requirement made at previous inspections. EVIDENCE: The location and layout of the home is suitable for its stated purpose. Both internally and externally it is accessible and well maintained. A new maintenance technician has been employed at the home and a number of residents expressed satisfaction and pleasure at the repairs/extra works he had undertaken in their rooms and elsewhere in the home.

All residents have been advised of the fire regulations and a Fire Officer has visited and confirmed compliance. There is a large lounge area with conservatory. The layout of which allows a number of seating areas. In addition there is a further separate lounge and a dining room. All of these rooms are furnished appropriately using good quality furniture and fittings. All residents who completed comment cards advised that they liked living at the home and that they felt safe. A number of comments were received during the inspection including, ‘it’s a very homely place’, ‘I am very happy to be here’, ‘it’s a home from home.’ There is a large garden to the rear which is accessible to residents and provides a number of seating areas. A new addition has been the reintroduction of a fishpond which a number of residents advised that they enjoyed going to see. Individual residents rooms are furnished appropriately. All residents confirmed that they were able to bring in items of their own furniture. One resident advised that she was particularly grateful to have been able to bring her own furniture with her, as it ‘was nice to see friendly and familiar things each morning when I wake up’. The home is centrally heated throughout and it was evident during the inspection that works are continuing to ensure that all radiators are guarded in line with national minimum standards. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28,29,30 Resident’s needs are met by the numbers and abilities of staff. Residents are in safe hands. Residents are protected by the recruitment procedures in place at the home. Staff receive training in areas appropriate to their work. EVIDENCE: Rotas and other records seen demonstrated that staffing levels at the home are appropriate to meet the care needs of the current service users, in line with department of health recommendations. Of the current staff team 18 of them have achieved a national vocational qualification at level 2 or above. This figure is lower than at previous inspections due to changes in the staff team, however other records seen show a commitment to provide this type of training to staff. A sample of staff records were seen for staff employed since the last inspection and these showed that a thorough recruitment procedure was followed thus ensuring the protection of residents. All staff were seen to have a statement of terms and conditions in place. This showed that a variety of training has already been undertaken this year and further training is planned. Staff spoken with were able to confirm attendance at training courses this year. All residents who completed comment cards were satisfied that staff treated them well. The majority of residents spoken with during the inspection were also very happy with the staff team with comments such as, ‘staff are always there to help if needed’, ‘the staff are very good’. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 38 The Registered Manager of the home is competent and is able to demonstrate that she is able to discharge her responsibilities fully. The management approach within the home encourages an open and positive environment Systems are in place to monitor the homes performance in meeting the aims and objectives set out in its Statement of Purpose. The Registered Manager ensures as far as is possible the health and welfare of residents and staff. She has also now successfully achieved the Registered Manager Award. Residents advised that they regularly see the Registered Manager and that she is available to speak with on request. Residents and staff meetings are held giving both the opportunity to contribute ideas and feedback on the services provided. In addition the home does have a quality assurance and monitoring system in place which seeks feedback from residents as to aspects of the services offered within the home. Since the last inspection a further questionnaire has been developed for potential residents and/or their representatives to use. Through records seen and information provided by the Responsible Individual within the pre inspection questionnaire the Inspector was satisfied that the home is run in such a way as to ensure the health safety and welfare of both residents and staff. The home is well maintained, training is provided within safe working practices and equipment and services within the home are checked and maintained.
