Conditions of registration: Persons accommodated or provided services because of a physical disability may be aged 40 years and over. Date of last inspection 6 January 2005 Brief Description of the Service: Oakwood is a fifty-bedded nursing home in Colney on the outskirts of Norwich. It is one of BUPA’S homes.The building is of new design and is on two floors. It is set in pleasant grounds, with several very large oak trees being a notable feature. There is a car park at the front of the home. There is good wheel chair access to the home and to the garden.The internal environment, especially the resident’s rooms, are of a very high standard. The Home provides general nursing care for persons over forty, and also cares for persons over forty, who may have a physical disability. There is a large dining room that is furnished with separate tables, seating various numbers of Service Users; it is a most pleasant room that has been tastefully decorated and allows accommodation for wheelchair users. Work has commenced on a ten bedded, extension to the Home that is being managed so that it causes as little disruption to the Service Users as possible. It is also planned to build a conservatory leading on from the dining room, another sitting room, quiet room and nurse’s station. The home has recently undergone an extension and now provides accommodation for ten more service users. These new rooms have ensuite facilities and pation doors leading to extensive landscape gardens. A new small conservatory has been added which leads off the dining room. The new extension also provides a new dining room, stelite kithen, quiet room and a nurses station. This is an overview of what the inspector found during the inspection. This was an announced inspection that took place over five and a quarter hours. Opportunity was taken to tour the premises, inspect care records, staff records and policies. Several service users were spoken with including all the staff on duty. The manager and deputy manager were present for the inspection process. What the service does well:
Oakwood House is an exceptionally well maintained home with a very good internal and external environment for service users. Both service users and staff are well supported by an experienced management team. There is a good team of staff that work hard at supporting each other and there is a friendly atmosphere within the home. The home has a very good training record and all the members of staff feel that they are continually supported in order to enhance their skills and experience. There are very good and detailed care plans that give clear guidelines to all members of the caring team so that they have a good understanding of the care required to meet the assessed needs of the residents. The home has a very good system in place that constantly monitors care and thus ensures that it is always adequate. All staff are encouraged to participate in a ‘Personal Best Tool’ this allows the staff to experience what it like to be a resident, also it gives them the opportunity to sympathise with other’s roles and encourages them to do that little bit more. The home provides a very good milieu for the dying, and also short term care for those persons who are not ready to be discharged after surgery at the main Bupa Hospital. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2,3,4,6 Admission to the home is handled well and prospective residents know that their needs will be met. Those residents who are admitted for intermediate care are helped to maximise their independence before discharge. EVIDENCE: The home has a very good admission procedure that use a recognised tool for assessment that guides the manager and the nurses as to the actions that need to be taken to ensure that they can meet the prospective resident’s needs prior to their entry to the home. Evidence for this was noted in assessment documentation, care plans and confirmed by those residents spoken to. These residents also indicated that they had been given the opportunity to visit the home prior to admission. Specialised interventions are available such as physiotherapy and occupational therapy and these interventions were noted on the plans of care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9 10,11, The care planning system is clear and provides staff with the information needed for them to meet the health, personal and social care needs of the residents. The welfare of the residents is protected by the home’s policies and procedures. EVIDENCE: Individual care plans were inspected, these clearly defined the care that was needed to meet the assessed needs of the individuals. Those care plans examined showed evidence of continual review of care and updating of care when needs changed. The home needs to be commended for its excellent care plans. Medication records and storage were inspected and these were found to be satisfactory; the MAR charts were clear and well documented, a random check of medication tallied with those record on the MAR charts. Policies and procedures were seen and found to be up to date and in line with current legislation; those residents who are able to self medicate and documentation for this practice was inspected and found to be satisfactory.

The Inspector gained the impression that the care of the dying is handled well and many letters of gratitude were seen from grateful relatives expressing their appreciation of the care given to their dying relatives. Two of the registered nurses have done the palliative care course and are able to cascade their knowledge down to other staff members which is obviously reflected in the letters of appreciation. A letter was also seen from the continuing care team expressing their appreciation of the care that their patients receive when living at the home. Care plans indicated the liaison that existed between other health care professionals, they also indicated that risk assessments were carried out in relation to skin integrity and nutritional needs. Observations made during the inspection process showed that staff had an understanding of how to promote dignity and preserve privacy. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13 The home offers a range suitable options to satisfy the residents social, cultural, religious and recreational needs; however the home could consider the provision of stimulation to those residents who are not so active and robust. Visitors are made welcome. EVIDENCE: There is an activity organiser in post and a programme for activities; the activities are very much geared around residents wishes and two residents had recently visited at their request a car boot sale. Those residents spoken with indicated that they chose what activities they joined in and that the staff respected their wishes if they wanted to opt out. The Manger is aware that they could do more to stimulate those less able, consideration is going to be given to this. Residents indicated that they receive visitors when and where they wish and this was noted during the inspection. There are quite areas if required for the purpose of receiving visitors although most residents indicated that they use their own rooms.

JUDGEMENT – we looked at outcomes for standard(s) 16,17 Arrangements for dealing with complaints are satisfactory. Residents legal rights are protected. EVIDENCE: Those residents spoken with indicated that they speak with the manager or with the other care staff if they have a concern about anything. They feel that they are listened to and are aware of the procedure for making complaints and are given this information on entry to the home. The home keeps a record of all complaint and how they have been handled and any action taken as a consequence. A record of a complaint seen showed that it had been responded in the appropriate time and was in line with the complaints procedure. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,23,24,25,26 Residents live in a well maintained safe environment. EVIDENCE: Oakwood House is a purpose built care home that appears safe and well maintained, it is suitable to meet the needs of the persons living in the home. It has recently undergone an extension to provide ten more rooms and a lounge and nurses station. Maintenance records were seen and were in very good order, the home needs to be commended for these very good records that are maintained by the maintenance person employed in the home. The residents have access to well maintained gardens and walkways. The rooms showed evidence of resident possessions and those residents spoken with indicated that they were encouraged to bring in some meaningful possessions. The home appeared very clean and tidy throughout.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,30 The deployment of staff is in sufficient numbers to adequately meet the residents needs. The home runs a very good training programme and staff have an understanding of resident support needs. EVIDENCE: Examination of the duty rotas show that the home constantly provides sufficient staff above the acceptable standard, this was found to be so during discussion with staff; however a resident did express concern about the length of time it takes for staff to answer bells. On discussion with staff and the Manager it was ascertained that one resident constantly rings the bell and it is difficult for staff to respond when they are involved in delivery of care with other residents. The staff acknowledged that there was a problem and were trying to overcome it. The Manager admitted that there had been a staffing problem due to sickness but this had not compromised the amount of staff on duty at any given time. Residents spoken with felt they were well looked after and were handled safely. The good training programme of induction, mandatory training and the facilitation of NVQ training supports this. The home comfortably exceeds the fifty per cent requirement for NVQ training, this was evidenced by good training records and discussion with staff. The home needs to be commended for its training programme.

Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32,33,34,38 Residents benefit from the ethos, leadership and management of the home. The home is run in the best interests of the residents. The health welfare and safety is protected by the homes policies and procedures. A quality monitoring process is in place that constantly monitors the quality of care given; this is done by numerous surveys the results of which were seen. The results from these surveys are audited and practices updated to correspond with the results. The areas looked at encompasses all practices which includes, hose keeping, laundry, management, clinical aspects of care, and nursing. The home seeks to promote the health, safety and welfare of the residents and this was evidenced by all records examined during the inspection.
