Date of last inspection 23rd August 2005 Brief Description of the Service: Oulton Abbey Care Home is a grade 2 listed building converted to a nursing home in 1989. Prior to this the home had been a retreat house and a boarding school. St Marys Church was built in 1853 and forms part of the home. The building has been home to a community of Benedictine nuns since 1853 (St Marys Abbey, also known as Oulton Abbey). The home is situated within the village of Oulton near to Stone in Staffordshire and can be accessed by public transport. The bus stops about 200 yards away and the train station is in Stone (approximately one mile away). Oulton Abbey is registered to take twenty-six elderly service users over the age of 60 years with care and nursing needs. This includes individuals referred through care management either from social services or hospital as well as privately funded individuals. This is an overview of what the inspector found during the inspection. This unannounced visit was made on the 30th December 2005 at 9.30 am. The inspection was carried out by one inspector who used the National Minimum Standards for Older People as the basis for the inspection. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 and 4 A written contract was in place for each service user and confirmation that needs could be met was provided. EVIDENCE: The standard contract between the home and the individual service user was inspected and evidenced that all required elements were contained. Contracts were seen as signed by the service user and the proprietor, Dame Benedicta. Confirmation of meeting needs was provided following an initial assessment of needs. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 and 11 Care planning processes within the home were clear and consistent to adequately provide staff with the information they needed to meet service user’s needs satisfactorily. Health care needs were met very well and service user’s needs at the time of their death were treated with dignity and respect. EVIDENCE: Two service user care plans were inspected and found to be detailed and well organised to provide staff with information to understand and meet individual needs. Care plans were reviewed monthly and risk assessments were completed as required and also reviewed monthly. Health care needs were met very well and service users had access to a range of other health care professionals. The resuscitation status of each service user, contained within the care plans, was signed as endorsed by the relevant service user to ensure clarity of their needs at the end of life. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Service users were enabled and supported to make their own decisions and choices about their own lives. EVIDENCE: Observation and comments from service users evidenced that they were enabled and supported to make their own decisions and choices about their day-to-day lives. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17 and 18 A robust complaints procedure was in place and service users were listened to with their views taken into account. Service user’s legal rights were protected and the homes’ Adult Protection procedure and staff training ensured they were protected from abuse. EVIDENCE: Neither the home nor the Commission had received any complaints since the last inspection. The Nurse In Charge confirmed that service users were enabled to exercise their legal rights directly and were supported by staff to participate in the electoral process if they so wished. The senior management of Oulton Abbey Nursing Home had reported one incident of alleged abuse which had been investigated prior to the inspection via the Protection of Vulnerable Adults procedure. All key agencies, including CSCI had been involved in this investigation and appropriate action taken to maintain the on-going protection of the service users. The provider and the management within the home were to be commended for their professional, prompt handling of this incident, which had ensured the protection of the service users. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 21 and 23 Sufficient toilets and bathing facilities were in place for the service users and the layout and size of rooms were satisfactory to suit the needs of the service users. EVIDENCE: Toilets and washing facilities were satisfactory throughout the home and very clean. The dimensions and layout of the rooms within the home was suitable to meet the needs of the service users. Rooms were homely, comfortable and individually personalised. Repairs to the heating and water system within the home remained outstanding. The registered provider confirmed that numerous efforts had been made to encourage a local plumber to progress the remedial work. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 and 30 Service users were cared for by skilled and trained staff that were competent to carry out their roles. EVIDENCE: There was 20 care staff working within the home, including bank staff. The care manager confirmed that a number of care staff was in the process of achieving NVQ Level 2 or above and the home would meet the requirement for 50 of all care staff to be qualified within the home in the near future. The inspector examined the records of the type and frequency of training undertaken by the staff within the home. Training records included the following: venepuncture 30.09.05, moving and handling 18.10.05 and planned for 09.01.06, fire safety training 01.07.05, chiropody planned for 11.01.06, dementia awareness 08.06.05, PEG 15.02.05 and infection control 06.10.05. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 34, 35, 37 and 38 Financial accounting and recording within the home was robust to safeguard service user’s financial interests. Records held were accurate and secure. The health, safety and welfare of service users were upheld. EVIDENCE: Suitable accounting and financial procedures were in place and individual service user financial records were inspected. All written records of transactions were maintained and appropriate records and receipts were kept. Individual records and home records were secure, up-to-date and in good order. The inspector examined a range of records and documentation, which evidenced that the health, safety and welfare of service users and staff were protected. These included fire safety records, which showed that fire safety training took place in addition to weekly fire alarm testing and fire doors were

The inspector was not able to examine records regarding the water tanks due to the ongoing delay with the plumber and the heating system.
