What the care home does well

The home continues to provide a friendly caring homely atmosphere for service users to live. The service users gave very positive comments about the staff, the home and the services provided.

What has improved since the last inspection?

The senior staff have implemented training to develop care practices of staff. A high number of care staff have achieved NVQ 2 and above.

It has the facilities for 34 service users (OP) who require Nursing and Care needs and is situated in a quiet residential close on the northern outskirts of Slough. The home offers views over adjoining school playing fields and residential properties. The establishment was originally an Edwardian family home and has many original features and décor. The visit to the home was a planned unannounced inspection to review the quality and outcomes for the service users living there. The inspection took one day and involved the inspector meeting with service users and staff. Records regarding the employment, management and administration were also reviewed. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 and 10 The medications are not fully administered in accordance to the regulations. Service users are treated with dignity and respect by the staff. EVIDENCE: The recorded medication policy, procedure and processes are in accordance to the regulations and National Minimum Standards. The medications are stored safely and administered appropriately by staff with the exception of the recording of the controlled drugs. The registered nurses have not regularly recorded their full signature as required by the Medicines Act 1968 and the Nursing and Midwifery Council Code of Professional Conduct. Regular audits are carried out on the medications held and administered in the home by the appointed pharmacist. Should a service user wish to administrate their own medications the home as the necessary assessment and storage facilities. Service users confirmed that the staff are polite and respectful to them and they maintain their privacy whilst they carry out personal care. The staff were observed to be friendly and have a good rapport with the service users. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 Service users are provided with a nutritious diet that meets their individual needs and wishes. EVIDENCE: The menus are planned on a two week basis and service users have a choice of main meal or a vegetarian option is available. Service users are consulted on a daily basis of what they wish to eat for that day and should they wish to change their choices the staff accommodate their wishes well. Service users with special dietary needs are catered for and soft diets are provided should they be required. The staff were observed to assist service users with eating their meals sensitively and patiently and were making it a very social occasion for them. Service users gave very positive comments to the meals provided and stated that they are enjoyable and well presented. On service user recently admitted to the home informed the inspector that they had regained their appetite due to the quality of the food provided. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. JUDGEMENT – we looked at outcomes for the following standard(s): 18 The home has the systems and has trained the staff to ensure that service users are protected from abuse. EVIDENCE: Staff have a good understanding of protection for vulnerable adults. They are provided with information and training in the induction programme and attend refresher training when required. The home has suitable policies and procedures in place and senior staff have receive training appropriate to their roles. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 and 30 The staff are trained well to support the health and social needs of the service users. EVIDENCE: The home’s team of twenty- four care workers has a high number of NVQ trained staff. Eighteen carers have achieved NVQ 2 and two carers are currently obtaining level NVQ3. There is a rolling programme of training inclusive of the mandatory health and safety training required and specialist training to support all staff roles. The home provides monthly workshops for staff to provide support and information regarding care topics. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33 and 35 The senior management staff are a skilled experienced team. Service users and others are consulted on a regular basis to ensure that the home is run in their best interest. Systems are in place to support service users control their own money should they wish EVIDENCE: The position of Registered Manager is in the process of changing to the senior nurse taking on the role. The current manager is remaining working in the home as a different role whilst attending further education. An application for this has not yet been received by the CSCI at the time of writing this report. The senior nurse/manager is in the process of undertaking NVQ4 and the Registered Managers Award and is an experienced nurse. The information from these processes are provided for the participants and others to view in the newsletters or a report each year. A recent survey has been carried out for the end of the financial year. The home has policies and procedures in place should service users wish to manage their own monies. Secure facilities are available in each of the service users rooms for the safekeeping of valuables. The home does not hold service users monies on their behalf.
