The inspector found no outstanding requirements from the previous inspection report, but made 3 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

This home for adults with learning difficulties looks after the service users well with some of the residents saying that this is also their view. The staff continue to develop and follow the individuals` care plans encouraging them to improve their current levels of skill and take part in a variety of activities that they clearly enjoy. The building provides a pleasant environment for the service users to live in with the bedrooms being personalised to different levels depending on the wishes of each of the occupants.

What has improved since the last inspection?

Since the last inspection service user plans for all of the residents have been completed enabling the staff to work towards meeting their needs. Records of hot water temperatures are now being regularly maintained showing that the safety of the service users is being taken into account. All of the staff records required to be kept are now in place as part of the process that ensures that people working with vulnerable adults are fit to do so.

What the care home could do better:

Although this is overall a safe and positive home for the service users to live more attention must be given to such things as CoSHH risk assessments to reduce the hazards to service users and staff and the development of a contract between the home and the service users that specifies who should be meeting which need.

It consists of two neighbouring bungalows which were purpose built in a residential estate in Parkfields which is to the south of Wolverhampton City centre. This gives good transport access to the local facilities for the residents. The location of the home also makes it easily accessible to the relatives and friends of those living there. Jaffray Care is the organisation that provides the care at this home. Each bungalow is designed to accommodate four people and are managed by Lian Hargreaves with each of her two deputy managers having particular responsibility for one of the buildings. Information about how care is delivered is given in a service user guide that is being developed in a format that is designed for easy understanding. The fees range from£1721 to £2159 per week. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 and 5 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Prospective users’ individual aspirations and needs are assessed. The service users expectations about how much the home would be involved in meeting these would be clearer if there were a contract in place outlining each party’s responsibilities towards the other. EVIDENCE: There have been two service users admitted to the home in recent months and looking at the records of one of them showed that assessment records were in place that covered all of the areas required to identify the full range of needs of that person. The manager described the process that had been followed to ascertain the information and other staff confirmed that this had been the case. There was also a care plan in place for this person and the manager explained that this had been developed from the information obtained during the assessment and the contents of the plan was entirely consistent with this. The manager confirmed that there were no contracts outlining the service users rights and responsibilities in place. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7 and 9 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The service user plans are comprehensive and updated at appropriate intervals ensuring that the individuals’ needs are met. Where possible service users are consulted and do make decisions about their lives. EVIDENCE: The care plans and the assessments of activities contained the information that enable staff to work towards the service users’ goals. Talking with some of the service users it was clear that they could make their wishes understood and that they were happy to undertake the activities that had been organised for them that day. The records contained evidence that these plans had been reviewed by staff on a regular basis. In talking to and watching the service users it was obvious that they were encouraged to make as many decisions for themselves as possible and their records showed if this could not happen for safety reasons. Those that could spoke of times when they had received the support of staff an example of this Parkfields (Jaffray Care Society) DS0000064571.V294729.R01.S.doc Version 5.2 Page 9 being that one of the residents, whose birthday it was, went out for a birthday lunch with staff at his request. A second service user’s plan had also been seen by a number of people such as social services staff and community nursing staff and had previously communicated to Commission for Social Care Inspection that this had been developed appropriately to meet what are thought to meet the needs of the person concerned. Various elements identified within the Individual Plans had also been risk assessed and these documents were regularly reviewed protecting the safety of the individual concerned. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15, 16 and 17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users are helped and encouraged to keep in contact with their families and make good use of the local facilities. The home provides good quality food in appropriate quantities. EVIDENCE: The regular weekly activity time-table for both of the service users whose files were looked at were found in their individual files. Activities ranged from attendance at a local day centre to helping with some of the household duties. During the visit the one service user was collected by the day centre staff and he appeared enthusiastic about going with them. The residents living in the home come from a variety of ethnic backgrounds and through talking with and listening to both the service users and the staff as well as people from other agencies and organisations ways in which the home attempts to meet their needs could be identified. Parkfields (Jaffray Care Society) DS0000064571.V294729.R01.S.doc Version 5.2 Page 11 Although this visit took place during the early morning it was evident from talking with the service users that they are given a choice of meals. Each was given or obtained for themselves a different breakfast and they clearly enjoyed what they had. Those service users who were spoken to confirmed that all of the meals that they had been given were to their liking and looking at the menus showed that a wide variety of meals are offered. On the day of the visit one of the service users went out for lunch as it was his birthday. He was also looking forward to his parents visiting for tea. At recent adult protection meetings the manager and the responsible individual nominated by the provider have expressed their views around how residents must keep in contact with their families and how this must be balanced within the context of their personal safety and welfare. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 and 20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The healthcare needs of the service users are met. EVIDENCE: The residents care plans showed how each individual should be approached and how different types of help should be given. It was seen during the visit that when personal support was needed the person concerned was discreetly taken to a bathroom or their room making certain that their personal care needs are met whilst their dignity is maintained. Numerous entries in the individuals’ records that were confirmed by either the staff or the service users showed that the help of various healthcare professionals was obtained when necessary. Further information that had been provided by local healthcare and social services professionals also confirmed this to be the case. The storage and recording of medication was looked at and was seen to be appropriate. Records showed and the staff on duty confirmed that they all receive training in the safe handling of medication. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The service users are protected from abuse and have access to a complaints procedure should they need it. EVIDENCE: The manager confirmed that there had been no complaints received. A copy of the complaints procedure was seen to be contained within the service user guide. The home has recently been involved in the local processes designed for the protection of vulnerable adults around allegations against a third party. They have carried out all of the action points agreed during the meetings as well as providing feedback to the meetings in a professional way thereby helping to ensure that actions are taken that are in the best interest of the person concerned. Looking at the staff training records and talking with staff established that they receive training in the identification of abuse and the procedures for reporting it so that should it be occurring it will be identified and action taken to prevent it. JUDGEMENT – we looked at outcomes for the following standard(s): 24 and 30 The standard of the environment is good providing service users with an attractive and homely place to live. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The home is situated on a housing estate in Parkfields and is two single storey purpose built buildings designed in a sensitive and practical manner. Both the houses and the gardens are well maintained with all décor and furnishings being clean and bright. Each house has a laundry area that is clean and contains the equipment necessary to keep service users’ clothes and bedding clean and hygienic. The staff training records showed that they receive training in infection control to further enable this. This judgement has been made using available evidence including a visit to this service. The home ensures that staff are available in adequate numbers and with appropriate training to meet the needs of the service users. EVIDENCE: Observing and talking with the staff on duty at the time of the inspection confirmed that enough staff are present at all times to meet the current needs of the service users. Looking at the records and talking with the staff and the manager confirmed that they receive all of the mandatory safety training and over 50 were qualified to NVQ2 in care. Looking at the records also confirmed that all of the required background checks had been carried out before each new employee had been allowed to start working with the service users so as to ensure that they were suitable to do so. Each personnel file contained a photograph enabling individuals to be recognised from their records. Talking with the manager and newly recruited members of staff who were receiving training at the time of the inspection as well as by looking at their Parkfields (Jaffray Care Society) DS0000064571.V294729.R01.S.doc Version 5.2 Page 17 records confirmed that an appropriate induction programme was available and implemented ensuring that new staff are given a good understanding of what their job entails. This judgement has been made using available evidence including a visit to this service. The home is a safe environment for the service users to live in. EVIDENCE: Talking with the registered manager established that she has some qualifications that are appropriate to the managing of this type of service, however, it has not been verified that these are the equivalent of NVQ 4 in care and management which are considered to be the appropriate qualification for managing a care home. The Commission for Social Care Inspection have been receiving copies of the reports on visits made by the responsible individual required by Regulation 26 of the Care Homes Regulations 2001 and these show that important areas of the home’s performance are reviewed on a regular basis. All of these records were found to be appropriately maintained and action taken where necessary. The exception to this was that although product data sheets were available for each of the dangerous substances used for activities, such as cleaning, were available the products had not been risk assessed by the home to ensure that they were used in a safe manner.
