CSCI found this care home to be providing an Good service.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home has a pleasant and calm atmosphere. Residents spoken with said that they are well looked after and that staff are kind. "Staff are good and look after me." "I have no complaints" were comments made. The acting manager promotes Equality and Diversity through staff recruitment, training and staff meetings. The acting manager takes residents` complaints seriously and acts upon them without delay. Residents said they knew how to make a complaint and felt confident that it would be dealt with. Through staff training and robust recruitment procedures, residents are assured that they are protected from abuse. The home looked clean and well maintained. No unpleasant odours were detected. A resident said she liked her newly decorated room with a new carpet and wallpaper. Residents enjoyed a music and movement event in the conservatory, which overlooks a pleasant garden. The home is accredited to serve as a base for student nurses. Such staff would work in the home for a designated period during which they learn under supervision. Staff spoken with said that Philbeach is a good place to work. They feel supported and are provided with many training opportunities. The great majority of staff have an NVQ qualification or are in the process of achieving this. Several staff have obtained an NVQ 3 and one a NVQ level 4. Staff are provided with safeguarding vulnerable adults training. Mandatory training includes infection control, moving and handling, food hygiene, fire safety and First Aid. The acting manager and deputy manager keep the CSCI informed of any reportable event. This ensures a good dialogue between the home and the inspector. Auditing systems have been introduced to ensure that policies are adhered to and care practices reviewed. Residents` and relatives` views are sought on a regular basis to ensure that these views are included in the home`s improvement plan. Monthly residents` meetings are held.

What the care home could do better:

Staff need to ensure that all handwritten transcriptions on medication charts are double signed. Whilst care plans are being upgraded and information transferred, staff must ensure that a current care plan is available to them at all times. The programme of upgrading staff files should continue to ensure that every file contains two written references.The provider must ensure that a registered manager be appointed without delay.

Date of last inspection Brief description of the care home Philbeach Care Centre is a large detached three storey building set in 9 acres of attractive grounds. The home has a terrace with good views over well-maintained gardens. It is situated on the outskirts of Hythe close to shops and other amenities. A refurbishing programme of decorating and installing en-suite facilities in bedrooms, in the long term, is in progress. The home provides residential and nursing care. The Company owns other care homes, the bungalows in the grounds and two Domiciliary Care Homes for Older People Page 2 of 9 Care Agencies. Weekly fees range from £337.23 to £412.76 for residents with residential needs and £ 550 to £750 for residents with nursing needs. Care Homes for Older People Page 3 of 9 What we found: This unannounced random inspection was carried out on 16th October 2008. The inspector was assisted by deputy manager Sue Rodgers and other staff throughout in a helpful and professional manner. The purpose of the random inspection was to examine those standards that relate to management. The home has been without a registered manager for some considerable time. The home is currently managed by a part-time acting manager assisted by senior staff. At this inspection it was stated that interviews for the post of manager are currently taking place. During a tour through the home, 6 residents were spoken with. They expressed their satisfaction with the service. Discussions were held with a nurse in charge and a number of care staff. Records were examined in relation to care planning and risk assessments, recruitment, staff training, duty rotas, complaints, medication and accidents. The inspection was further informed by an AQAA (annual quality and audit) carried out by the acting manager on 25th July 2008. This had been completed in great detail for which the manager is to be commended. Comment cards were sent to residents and staff. Two residents responded and answered all questions positively. Three members of staff responded. Two responses confirmed that regular staff meetings take place to discuss any care issues. There is good communication between management and staff. Management is approachable. Regular handovers are useful. These are robust and explicit. New staff are provided with a good induction. One member of staff said that staffing levels are low which can be a problem when needing to use hoisting equipment. Since the previous inspection an investigation under adult protection procedures was carried out. This was satisfactorily concluded. The CSCI has received 3 complaints since the previous inspection. These have been dealt with in an appropriate manner. Occupation at the time of the inspection comprised 6 residents with residential and 34 residents with nursing needs. Since 31st of March 2008, the home has been managed by an acting manager currently on a part-time basis. The acting manager and her deputy work well together in an effort to continue to raise the standards particularly in respect of care planning and staff training. They reported good communication with GP’s and district nursing staff. District nurses are based at Philbeach and readily available for treatment and advice. The acting manager is further supported by other managers in the Company. Care plans have much improved and are person centred. Risk assessments and wound care records have been reviewed and included in the care plan. Following staff changeover, the manager reported that apart from one full-time nurse vacancy, the home is fully staffed and staffing levels are adequate for the needs of the residents. Staff files are more comprehensive but could be further improved.
