Conditions of registration: 1. Service users must be aged 65 years and over on admission 2. The maximum number of service users to be accommodated is twenty eight (28) Date of last inspection 15 February 2005 Brief Description of the Service: Pinewood Manor Residential Retreat is a large, pivately owned care home registered to provide care and accomodation for 28 older people. The curent owners registered the home in June 2004. The detached property is set in its own mature gardens that overlook Ashdown Forest. It is about one mile from the amenities of Crowborough town centre. Service users accomodation is situated on two floors with a shaft lift and two staircases providing access to the first floor. The home was extended in 1997 to provide additional ground floor accomodation and lounge. There is level access to the grounds. This is an overview of what the inspector found during the inspection. The inspection was undertaken on the 25th July 2005 by two Inspectors between 10am and 4pm. During the inspection the following documents were examined; 3 recruitment files, 3 care plans, the homes statement of purpose, the service user guide, a selection of the homes’ policies and procedures and the staff rotas. Discussions took place with the manager of the home and with three members of staff. The homes compliance with the requirements made at the last inspection were checked. What the service does well: What has improved since the last inspection?
Since the last inspection a service users guide and a statement of purpose have been drafted although they still require amendment and the addition of a copy of the terms and conditions of residency. Terms and conditions of residency have been reviewed but have still not been given out and signed by the service users. Drains have been cleared. A review of lighting took place and all lights are now fully functional. Staffing levels over the evening mealtime period were reviewed. The outcome of this was that, should the number of residents rise to 23, additional catering staff will be provided to prepare the evening meal. A policy for emergency admissions has been implemented. All recommendations as advised by an occupational therapist, have been implemented, apart from the replacement of the tables and chairs in the dining room. Two members of staff have enrolled on NVQ level 2 in Care. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4. Standard 6 is not applicable. Pre admission assessments are carried out. The home is not providing prospective service users with the information required, to assess if the home can meet their needs or if the terms and conditions are acceptable. EVIDENCE: The home has a statement of purpose and separate service user guide. These documents provide useful information, however, the service user guide does not include a copy of the terms and conditions of residency or refer to the availability of the last inspection report. The statement of purpose needs amending to include the size of all rooms. The service users guide needs amending to include the views of the service users. The home was purchased by Ampersand Care in May 2004. The terms and conditions of residency for residents are currently being updated. The Inspectors were concerned to note that service users admitted since the new ownership had not been provided with a written contract/statement and terms of residency. Records confirmed that the manager or another senior carer assesses all prospective service users prior to admission.

Although the specialist care needs of residents are assessed appropriate specific guidance for staff is not well documented in the plans of care. During the inspection it was noted that a skin complaint suffered by 2 residents had not been reported to the public health department for advice on treatment. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,10 Care plans are informative and contain the information required for staff to support service users in their daily living but some lack specialist guidance. Service users are treated with dignity and respect. EVIDENCE: The care documentation relating to three residents were reviewed in depth and identified that full assessments based on the activities of daily living are completed following admission to the home. Plans of care are drawn up following the assessments. However, specialist care needs are not fully identified in the plans of care e.g. the treatment of scabies, and specific guidance must be included for staff to follow. A review of the care plans confirmed that these were up to date and signed by the resident or their representative. Residents spoken to stated that they felt that their privacy was respected even though not all staff knocked on their doors before entering. All residents have their own individual room. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 The home does not provide residents with an adequate level of activities or entertainment. Residents’ links with family, friends and the community are promoted, as are autonomy and choice. EVIDENCE: All of the residents spoken to expressed dissatisfaction with the activities and entertainment provided in the home. One resident said that ‘she spent her time sitting and watching the wall paper’ another said ‘its boring not much to do’. One resident said that her only complaint about the home was that ‘there was a lack of occupation’. On the day of this inspection it was noted that most residents were sitting in one of the lounges, the television was on but most residents were not watching it, some were sitting in a position where the television could be heard but not seen. The residents confirmed that there are some activities that included bingo and skittles, and that the home has a mini bus but this is not used. Although no visitors were seen at the time of this inspection residents confirmed that they were always well received by staff and that there was no restrictions on visiting times. Discussion with residents confirmed that choice on meals is given and those residents who are independent said they were able to spend time how and where they wanted.

The meals provided are focussed on home like cooking, and fresh vegetables and meat are used from local suppliers. All residents confirmed a high satisfaction with the food provided saying it was ‘good’ and ‘excellent’. This was confirmed when one of the Inspectors joined the residents for lunch. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 The home has a satisfactory complaints procedure. The adult protection procedure needs amending to include the need to contact the Social Services department if abuse is suspected and to notify the Commission for Social Care Inspection if an adult protection alert is made. EVIDENCE: The homes complaints policy and procedure are satisfactory, though it was noted that since it’s introduction earlier this year, there had been no complaints logged. Upon examining records of residents meetings, it became apparent that residents had raised complaints and issues of concern on a regular basis and staff had addressed them. Although staff are responding to complaints they are not recognising them as such when they are made in the setting of a residents meeting. Staff need to be given guidance on how to use the complaints procedure. The home has an adult protection policy and procedure and a copy of the local guidance that they are required to follow. However the procedure for making an adult protection alert needs amending. It must include contacting Social Services when abuse is suspected and informing CSCI when an adult protection alert is made. It is important that the home keeps up to date with local guidance and procedures and that the manager ensures that all staff are familiar with this procedure. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,23,24,25. The décor of the home is poor. The communal space provided for service users is not homely. Service users own rooms meet their assessed needs. EVIDENCE: The hallways on the ground and first floors are in need of redecoration. The wallpaper is peeling from the walls and is unsightly. Handrails that have recently been purchased need to be varnished or painted to give a nonporous and easily cleaned surface. The dining room, corridor in the new extension, and lounge all had no curtains making them look cold and bare. The lack of pictures etc to adorn the walls again gives the home a very cold and ‘unlived’ in feel. The hatch from the kitchen to the dining room is badly stained and needs repainting as does many of the doors. The tables and chairs in the dining room need to be replaced and an occupational therapists advice should be sought as to suitable alternatives. This is a requirement that has been made at several previous inspections. The wall in one of the bedrooms is badly stained and in need of redecoration. Radiator covers that have been installed in service users rooms must be painted.

A programme for redecoration of these rooms needs to be devised and implemented within agreed timescales. The first floor landing is dark in parts and needs lighting during the day. There are sufficient lavatories and washing facilities. The bathroom that is being updated needs to be completed as soon as possible and should not be accessible to service users until such time as the work is completed unless it has been assessed that it is safe for them to do so. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29,30 Albeit the policy and procedures for the recruitment of staff are robust they are not consistently followed. The staff team are trained to do their jobs. The deployment and number of staff is not always sufficient to meet the service users’ needs. EVIDENCE: Of the three recruitment files inspected, one contained all the required documentation and two did not contain any proof of identity. It is a requirement that proof of identity is obtained, prior to staff working in the home. The induction programme for new staff is comprehensive. The completed induction records for two members of staff were examined they had been completed within the first 6 months of employment. The home has a training programme in place to address the gaps in skills identified by the manager. This includes training in specialist areas e.g. dementia. However, this must be continuously monitored to ensure staff can meet service users changing needs. The home employs two trained nurses and 3 members of staff that have obtained a National Vocational Qualification in Care (NVQ) at level 2 or above. Two members of staff are currently working towards NVQ Level 2. However, they are not meeting the minimum requirement of 50 of the workforce be trained to level 2 in care or above. In light of a recent concern brought to the attention of the CSCI, before the inspection that the home was left with an insufficient level of staffing in the afternoon, staff rotas were examined. It is evident that there is a shortage of staff between the hours of 3pm and 4pm due to the fact that most of the morning staff leave at 3pm and the next shift does not start until 4pm.

A shift leader must possess the relevant experience and qualifications and have the level of seniority required to carry out the roles and responsibilities of the position. The staff rota must be revised to ensure there is adequate staff cover for this period of the afternoon. This should also allow enough staff to be present at a handover to ensure that all relevant information is passed on to the next shift. The manager must be aware at all times of the staffing levels. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,33,35,36 The process of gaining service users views needs to be further developed. Financial arrangements, weekly charges and additional costs are not documented. Staff are appropriately inducted and supervised. EVIDENCE: Residents meetings are held on a weekly basis although some times an activity is held rather than a meeting. It is at these meetings that service users views are sought. Issues raised at the residents’ meetings are often in relation to the running of the home and some changes have been made as a result. The manager is developing a service user satisfaction survey and is hoping to implement this in the near future. There must be more consultation with service users as to their views on the running of the home and things they would like to see change or be implemented, including activities. Staff receive supervision on a regular basis and this is documented. One recruitment file examined showed that the member of staff was receiving supervision weekly. Supervision covers relevant issues e.g. clarifying the roles and responsibilities specified in their job description.

The home does not take responsibility for any of the service users finances. Where purchases are made on behalf of a service user or a service is provided the cost of which is not included in the weekly charge e.g. hairdressing, the service users next of kin or advocate is billed accordingly. The homes statement of purpose and the service users’ terms and conditions must specify what is included in the weekly charge and what the financial arrangements are for paying for services that are not included. The owners must do an unannounced visit to the home on a monthly basis. A report of which should be sent to the CSCI and be made available to the acting manager. The owners were made aware of this at the last two inspections when requirements were made within the CSCI inspection reports. The home does not have a registered manager but an application has been submitted to the CSCI by the acting manager.
