Conditions of registration: Date of last inspection 14th July 2005 Brief Description of the Service: Pinewood is a care home registered with the Commission for Social Care to provide Nursing care for up to twenty one older people. The service is owned by Mr and Mrs Marsh and Mrs Deryl Marsh, a first level nurse, is the registered manager. Nineteen rooms provide accommodation of which seventeen are singles and two are doubles. Nine of the rooms have ensuite facilities. All first floor rooms are serviced by a passenger lift. The home is situated in the village of Chidham, some four miles west of Chichester with its shops, pubs, banks and other amenities. The detached home stands in beautiful grounds with stunning views of the sea and countryside. There is a large car park at the front of the house to accommodate staff and visitors. This is an overview of what the inspector found during the inspection. This inspection was announced and took place from 11am to 4-15pm. Pinewood has performed well over the past years and at the last inspection no requirements were made. During the course of this inspection the inspector spoke to residents and staff. The inspector had received a large number of Comment Cards from relatives and residents and some of the comments made will be included in this report. Records and Care Plans were looked at and a tour of the home was conducted. One relative wrote, “If all homes were run as well as Pinewood Nursing Home then there would be no need for inspection.” Another commented, “I looked around many nursing homes and found Pinewood to be one of the best!” Residents were very happy with the care that they received and several commented, “staff are very kind and take great care of us.” One said, “I could not think of a better place to be.” Another said, “Nothing is too much trouble.” One letter was sent to the inspector from a family who had recently lost their loved one. It talks about the care that their relative received at Pinewood and finishes, “…..I can’t praise the staff and home enough.” Staff training was ongoing and it was noted that the majority of staff were either National Vocational Qualification (NVQ) trained or were on training. The training records of all staff were well documented and up to date. The manager had recently gained her Registered Managers Award (RMA) and should be congratulated for doing so. The building was warm, clean and welcoming. There was one registered nurse, four care staff, two domestics, one cook, an administrator and the manager on duty at the time of the inspection. At night there were two staff awake on duty with backup to the manager. The rotas showed this to be the case. The home had had inspections from the Fire Officer and the Environmental Health Officer. No adverse comments were left. No requirements were made at this inspection. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4, 5, 6 Residents and their families have the information they need to make an informed choice about where they live and are suitably assessed before placing. EVIDENCE: Each resident is given a contract when he/she enters the home. Most of the residents had family or friends who were able to act upon their behalf. The manager assesses all residents before they join the home either in their own home or in hospital. These completed assessments are kept in the Care Plan. As the home is registered for nursing, it does take people who have left hospital and need some care before they are able to return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10, 11 The resident’s health, personal and social care needs are well met through training and supervision of the staff. EVIDENCE: Care Plans were well written and contained all the information necessary to look after the residents. They were updated regularly and reviewed at least once a month. The inspector looked at the Care Plans of some of the residents and felt that the care given was accurately recorded. The care plans had been updated and were now contained in proper folders ensuring that some paperwork was not misfiled or misplaced. Medication was stored safely and records were well kept in regard to the administering and disposing of them. A number of residents were receiving ‘Controlled Medication’ and these were appropriately stored and administered. Only trained nurses administer the medicines. All unused medication was safely disposed of as required by new regulations. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 Residents say that they are happy with their lifestyle, are able to keep in contact with their family and friends and able to have some control over their daily tasks. Good home cooked food is provided. EVIDENCE: Some of the residents are very poorly and were in bed at the time of the inspection. Others were sitting in their room or in the main lounge. One lady was listening to some music, whilst others were sitting watching television. Others were sitting in their rooms talking to relatives. The home employs an administrator who the owners call their “Girl Friday”. She currently organises activities for the residents and was busy trying to put all the Christmas decorations up. All the residents able to go out enjoy the trips to the local pub or for a ride around the area. A local vicar visits at least once a fortnight. The extensive gardens are well used in the summer months. Family visits are important and the inspector was told that all visitors were offered refreshments. One visitor always spent the whole day in the home having lunch and tea with his loved one. Five people have their food liquidised and the inspector was delighted to see that the cook was liquidising each part of the meal separately. Staff assist people to feed and the inspector observed one member of staff feeding a resident in the dining room. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17, 18 Any complaint is taken seriously and staff and residents know how to complain. Staff are well aware of the Adult Protection Procedures and a Whistle Blowing Policy is in evidence. EVIDENCE: All staff have attended an in-house Adult Protection Training and the procedures are in the office. There have been no complaints made about the home to the CSCI. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25, 26 The home is well maintained, comfortably furnished with no unpleasant smells. EVIDENCE: The home is constantly updating and the inspector noted on his walk around that several rooms had been recarpeted and decorated since the last inspection. Residents’ rooms are comfortably furnished and contain many of their own personal ‘treasures’. All rooms are fitted with safety covers for radiators and mixer valves on the washbasins to deliver water at a safe temperature. Some doors have individual fire closures fitted so that doors are not propped open. The home is kept very clean and tidy and there were no nasty smells. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 There is usually sufficient staff employed to ensure that residents’ basic needs are met. The home has a sound recruitment policy and all checks are made. There has been considerable input into staff training. EVIDENCE: All staff have undergone Criminal Records Bureau (CRB) checks and copies of these were kept in the office. The inspector was present when a prospective member of staff brought in documents that were needed for this process. Prospective staff are invited to complete an application form and attend an interview. Staff training had been undertaken in all areas and the evidence showed that all staff are offered training relevant to their work. The inspector was shown a workbook that new staff were being encouraged to use. Four staff have a National Vocational Qualification (NVQ) at Level 2 or above and a further four are studying for the Level 2 at this time. The home employs ten registered nurses. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 34, 35, 36, 37, 38 The home is well managed by a competent and qualified manager. It is apparent that the needs of the residents are paramount and that things happen for their benefit. The home has no dealings with any personal finances preferring to leave that to families or other professionals. EVIDENCE: The manager has recently qualified for her Registered Managers Award. Staff told the inspector that she is very approachable and offers support and encouragement when needed. The home was constantly updating their practices to ensure that residents received the best care that they could. The home does not have any dealings with any money belonging to residents.
