What the care home does well

Residents benefit from living in a well maintained, spacious and nicely decorated Home, which is clean and free from offensive odours throughout. On the whole residents benefit from staff who have the skills and training to meet their needs and who are appropriately supervised and supported by an effective management team. The Home now has a new intermediate nursing care unit, which is providing an effective rehabilitation service with very positive feedback from service users. The Home has a day centre onsite and offers respite care as part of its linked service provision. Several residents felt reassured and privileged to have been able to take advantage of the continuity provided by such service models. The inspector received very positive feedback regarding the food provided in the Home.

What has improved since the last inspection?

The Home now keeps individual records of all financial transactions undertaken on behalf of service users.

What the care home could do better:

The safety of residents could be improved by ensuring that the remaining radiators have low temperature surfaces or are appropriately covered to prevent burning. The management of residents` finances could be improved by ensuring that their money is held in an individual named account and not pooled. Residents would benefit if all staff involved in supporting service users in the rehabilitation unit receive rehab training appropriate to their role. Residents in the rehabilitation unit would benefit if the service ensured that their progress is reviewed after 1 month and an exit plan is put in place. The assessment process for prospective intermediate care residents could be improved by adopting a single assessment procedure. The rehabilitation of residents could be made more effective if residents had Occupational Therapy input on admission to the Home and delays of 2 to 3 days, as is sometimes experienced, was avoided. Residents` access to a telephone could be improved.

The home provides care and accommodation for a maximum of 43 older people. It has a new intermediate care unit, which is registered to provide intermediate nursing care. A new six-bedded en-suite facility has also been built for long stay residents and is now in use. The Home is situated in a residential area of Rainham and within walking distance of a bus route. The home is approximately ¾ of a mile from Rainham Town Centre and 1 mile from the main line railway station in Rainham. There is off road parking on site and garden areas around the Home that service users can use and enjoy. The home has a hairdresser on site during weekdays and an activities coordinator who works in the home Monday to Friday. The Home also benefits from its own transport. This was an announced inspection, the first in the year running from April 1st 2005 to March 31st 2006. The visit lasted from 10.00am until 18.00pm. The visit was spent talking directly with residents privately and collectively ; 5 care staff, 2 relatives, O.T /Physio staff, the nurse in charge of the intermediate care unit and the registered manager. Some judgements about quality of life and choices were taken from direct conversation with residents and their relatives and observation followed by discussion with support staff and evidencing records held at the home. A tour of the premises was undertaken. Residents and relatives spoken to during the inspection and comment cards received gave positive feedback about the staff and services the Home provides. The inspector is confident that, overall the outcomes for residents in this Home are good. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,6 Whilst residents benefit from effective assessment and care planning systems, improvements in this area are needed for residents receiving intermediate care. EVIDENCE: Residents spoken to and records seen demonstrated that residents’ needs are assessed prior to entering the service. Assessments and care plans of residents receiving rehabilitation, although in place, did not include 4-week reviews and exit plans as is considered good practice. Examples were also given of residents inappropriately remaining in the Home after successful rehabilitation has been completed. To ensure an effective and efficient transition through the service a requirement has been made regarding this. The inspector has recommended that the Home and professionals involved in the rehabilitation of residents move to a single assessment. It is also recommended that the Home look at ways of ensuring that rehabilitation programmes can commence on admission to the service and delays of 2 to 3 days are avoided. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 10 Residents benefit from being cared for by staff who are sensitive to their needs and treat them with respect and dignity. Not all residents have easy access to a telephone. EVIDENCE: Feedback from residents and observation on the day evidenced that staff treated residents with respect and dignity. Comments such as “They are very kind nothing is too much trouble”. “ they always ask me what I want, I would not want to be any where else”. “ The carers, you couldn’t get nicer people”. Were expressed at the inspection. It was noted that there is only one pay phone for residents and one resident told the inspector they have to walk a long way to use it. The manager confirmed that some residents have to walk to another unit to use the phone and a recommendation has been made that the Home look at ways of ensuring residents have easy access to a telephone. It is acknowledged that all residents rooms have telephone points and it was evident that some residents have had their own phone installed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13,15 Residents are encouraged to maintain family contacts and staff communicate with families and friends in the best interests of residents. Residents benefit from balanced and nutritious meals which are served in a congenial setting. EVIDENCE: Residents and their relatives spoke positively about their relationships with the Home. Relatives said they were made to feel welcome and could visit as they wished at any reasonable time. They also complimented the Home for maintaining good communications with them. They were able to raise any concerns they had and felt that their concerns were listened to. Residents also confirmed that their views were sort individually and at regular residents meetings. Most residents spoke positively about the food provided and it was evident that special dietary needs were catered. Residents confirmed that alternative meals are offered if they wish and they are consulted about the menu. Residents also confirmed that drinks and snacks were available at regular intervals throughout the day. JUDGEMENT – we looked at outcomes for standard(s) 19,26 Residents benefit from living in a reasonably well maintained, nicely decorated and furnished environment, which is spacious and free from offensive odours throughout. EVIDENCE: A tour of the premises, information taken from the homes pre inspection questionnaire and feedback from residents evidenced that the Home was Maintained to a good standard. Residents repeatedly commented on how clean the Home was kept. The manager confirmed that a budget has now been allocated to ensure the remaining radiators are replaced or covered to prevent risk of burning. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 27,28 Residents benefit from having sufficient and well trained staff to meet their needs. EVIDENCE: Based on information provided by the manager the Home exceeds the minimum staffing levels as recommended by the Residential Staffing Guidelines and the Department of Health. Discussion with the manager and staff and information taken from the homes pre inspection questionnaire evidenced that 61 of staff are qualified to NVQ level 2 or above and have undertaken a range of mandatory training. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 31,32,35,36,38 Residents benefit from a Home that is managed in their best interests with an appropriately supervised staff team. EVIDENCE: Platters Farm Lodge H56-H06 S36837 Platters Farm Lodge V239353 250805 Stage 4.doc Version 1.40 Page 18 The Manager has NVQ 4 and Diploma in Management Qualification and is also a First Level Qualified Nurse. She has 9 years experience of managing services and undertakes a range of short courses to update her skills and knowledge. One team leader also has the NVQ 4 In Management and Care. The inspector is satisfied that there are clear lines of accountability within the Home and external management. It was evident from discussion with the manager and staff that she communicates a clear sense of direction and leadership, which staff and service users can understand. The Home creates an enabling environment for service users both in the rehabilitation and long-term residential service. The Home sets good standards of care and innovative practice particularly in regards to partnership working with health services for the benefit of residents. Discussion with residents and families indicated that a range of quality monitoring systems are in place and that action is taken to address any issues raised. Records seen and discussion with staff evidenced that formal staff supervision was being undertaken every 2 months. A supervision format is used which covers all aspects of practice and staff training and development needs. Team leaders who are in tern supervised by the manager undertake supervision. The Home also has staff appraisal, which is undertaken every 6 months. Records viewed, a tour of the premises and information taken from the homes pre inspection questionnaire evidenced that the homes facilities and equipment were appropriately maintained. Refer to Standard 35 Good Practice Recommendations The following is recommended:Southampton City Council have managed to find a good solution to the issue around reg 20, and the need to avoid pooling service user monies. The system theyre adopting is called Clients Monies Service (CMS) from Nat West. It involves one account, with separate sub-accounts within it. The sub-accounts carry their own number and are interest bearing, therefore ensuring interest is calculated on the amount in each account etc. Monies can be managed in relation to each sub-account by the authoritys finance department through a modem link to the bank. It is recommended that, in the case of prospective intermediate care residents, the home and appropriate professionals move to a single assessment system. It is strongly recommended that the Home look at ways of ensuring that rehabilitation programmes can commence on admission to the service and delays of 2 to 3 days are avoided.
