Conditions of registration: 1. The home must comply with the Staffing Notice as issued by the Shropshire Area Health Authority dated 8th May 1990. 2. The home may accommodate a maximum of 43 service users. 3. The home may accommodate any balance of older persons requiring nursing care and younger physically disabled persons to the total of 43, including 3 terminally ill persons. Date of last inspection 24th September 2004 Brief Description of the Service: Radbrook Care Home is registered with the Commission for Social Care Inspection to provide accommodation with en-suite facilities for 43 people needing nursing care. Radbrook is one of a group of nursing homes run by Morris Company Care Division. The home is situated on the outskirts of Shrewsbury with local bus services close by. The home provides 5 sitting areas, which are located at the end of each wing, 1 dining room/conservatory, which is on the ground level and situated at the front of the building. There is a large sitting room upstairs. There is access to a garden and car parking is available. This is an overview of what the inspector found during the inspection. This unannounced inspection took place on the 10th August 2005 and commenced at 10.00am. The National Minimum Standards for Care Homes for Older People focus on achievable outcomes for service users – that is the impact on the individual of the facilities and services of the home. Evidence was looked for that the standards were being met and a good quality of life enjoyed by service users through: • Discussions with service users, families and friends, staff and managers. • Observation of daily life in the home • Scrutiny of written records (including care plans for 4 service users). The statement of purpose was used to assess how far the home’s objectives to be able to meet service user requirements and expectations were being met. Reports regarding an overview of the conduct of the home are sent to CSCI on a monthly basis by a representative of Morris Care. These, as well as the risk assessment from the last inspection were taken into account to determine the core standards focused on and depth of inspection. The commission does not have any concerns with this home. What the service does well:
The staff provide appropriate health and personal care very professionally and do it well. Communication between staff and service users about their care needs is very good. After appropriate risk assessment, service users are not discouraged or unduly restrained from undertaking activities solely for fear that for example, they may hurt themselves. Through discussions with the manager and staff, the principles that should govern life as stated in the statement of purpose, focus on the importance of promoting service users’ independence through enabling them to make their own decisions and fostering their individuality.

What has improved since the last inspection? What they could do better:
There is evidence of service user involvement in the reviews of care at the formal meetings held initially 6 weeks after admission and annually thereafter. The home could improve this further by demonstrating involvement of service users in their care at an earlier stage and following any changes. Care plans should be signed by the service user where able or by any supporter involved. The manager agreed to address this at the team meeting and explore a method that would work in the home. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,4 The homes statement of purpose and service user guide is good providing service users and prospective users with details of the services the home provides enabling an informed decision about admission to the home. Pre-admission needs assessments are thorough ensuring that service users who move into the home are assured that their needs will be met. EVIDENCE: The statement of purpose and service users guide have been updated for 2005. The information provides details of needs assessment to be conducted prior to admission and examination of care plans in the home confirmed that this process had been carried out. In carrying out pre admission assessments whether the service user is privately funded or not the margin for error in meeting needs will be less than just relying on the care management referral process. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 There is a clear, consistent care planning system in place to adequately provide staff with the information they need to satisfactorily meet service users needs. Personal and healthcare support in this home is offered in such a way as to promote and protect service users’ dignity and privacy in their daily life and at the end of life. EVIDENCE: There is a clear, consistent care planning system in place to adequately provide staff with the information they need to satisfactorily meet service users needs. Care plans looked at were consistently completed and had all the useful information in them that is necessary to enable staff to provide the right care for that individual in a way that meets their wishes and preferences. The information that was read showed that individuals receive the level of care which their own situation requires.

This aspect would be enhanced further if service user involvement could be clearly demonstrated on the initial plan. This does happen at the formal review. Appropriate assessment of bed rails is made where they are used. Service users spoken to stated that they were happy with their care. Staff were very knowledgeable about the client group they were caring for on this shift. On both floors staff were seen to approach service users respectfully and speak to them in a dignified manner. Service users showed signs of well being by their relaxed manner and being able to assert their own wishes e.g. what time to get up and where to sit. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 Staff have an excellent understanding of the service users support and leisure needs and use this to assist them to exercise choice and control in their lives. EVIDENCE: There is no strict imposition of rules or routines within the home. Service users were seen in their rooms or in the communal lounge areas. Call bells were within reach. Individuality is maintained as service users have their own rooms, places to withdraw to from busy active communal areas and choice over when they get up and go to bed. Activities are displayed on notice boards and are varied e.g. ‘extend’ exercise class, flower arranging, art therapy. A shop trolley is available. Menus are on display in the foyer rotating between summer and winter fayre. Exit surveys carried out, mainly with service users on respite stay, recorded favourable comments about the food provided. JUDGEMENT – we looked at outcomes for standard(s) 16 The home has a complaint procedure that ensures complaints are dealt with promptly and effectively. Service users and their friends can be assured that what they say will be heard and dealt with. EVIDENCE: The complaint log was seen. There has been one complaint since the last inspection, made to the home, unrelated to care provision. This had been dealt with in the time scales of the homes’ procedure and responded to in full. The quality team carries out an audit trail of all complaints in the home to identify any trends and to address any failings. The Chief Executive of Morris Care records a comment on each complaint seen. Compliment letters were seen attached to the notice boards and in a folder in the foyer. JUDGEMENT – we looked at outcomes for standard(s) 19 The standard of the environment within this home is very good providing service users with an attractive and homely place to live. EVIDENCE: A general tour of some bedrooms and communal spaces demonstrated that the home is purposely designed each with its own lounge/kitchenette and dining area. These were all in a good state of repair and comfortably furnished. Individual rooms are personalised. Service users stated that their rooms are kept clean and that they can spend their time where they wish. People tend to congregate in the large comfortable lounge or in the foyer to watch the coming and goings of staff and visitors in the home. The monthly report on the overview of the home details routine decoration and maintenance of the building due to be carried out.

JUDGEMENT – we looked at outcomes for standard(s) 27,30 The arrangements for the induction and training of staff are good with the staff demonstrating a clear understanding of their roles. There is a good match of well-qualified staff offering consistency of care within the home. EVIDENCE: Relevant training has been provided and staff spoken with confirmed this. Staff observed carrying out their duties were seen to be responsive and understanding of individuals wishes and needs. The nursing and care staff deployed today gave the cover required to meet the home’s stated aims. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32,33,38 The manager identifies and understands the areas in which the home aims to improve. Planning is in place to set out how this improvement will be achieved and resourced. Service users views have been sought and they perceive them as having an effect in changing how the home is run. Quality assurance takes place in the home through a variety of audits ensuring service users that the management seek out any failings and put systems in place to address them. EVIDENCE: The monthly report into the conduct of the home stated that all records and health and safety checks are in place and up to date.

The home has carried out an annual satisfaction survey for 2005. Service users commented on this and said that they had enjoyed the opportunity to have their say. The management is in the process of devising an action plan to inform service users how they intend to respond to the survey. The survey concludes that the majority of service users are ‘very’ or ‘quite satisfied’ with their personal care. Discussions demonstrated that the manager continues to strive for excellence and find innovative ways to provided the service to the ‘community’ that is involved with the home be it service users, visitors, day care, relatives, inhouse staff and outside health care professionals, etc. Future ideas include plans to enhance the excellent palliative care service in the home. The home already has very close liaisons with Social Services and Health Professionals. The manager and a member of staff are members of the Shropshire Tissue Viability Group and have had input into to November 2004 wound formulary. Comments received during the inspection included “the quality of care is good. Staff are professional and very caring”.
