The Home must comply with the Staffing Notice as issued by the Shropshire Area Health Authority dated 8th May 1990. The home may accommodate a maximum of 43 service users. The home may accommodate any balance of older persons requiring nursing care and younger physically disabled persons to the total of 43, including 3 terminally ill persons. 10th August 2005 Date of last inspection Brief Description of the Service: Radbrook Care Home is registered with the Commission for Social Care Inspection to provide accommodation with en-suite facilities for 43 people needing nursing care. Radbrook is one of a group of nursing homes run by Morris Company Care Division. The home is situated on the outskirts of Shrewsbury with local bus services close by. The home provides 5 sitting areas, which are located at the end of each wing, 1 dining room/conservatory, which is on the ground level and situated at the front of the building. There is a large sitting room upstairs. There is access to a garden and car parking is available. This unannounced inspection took place on the 10th January 2006 commencing at 09.10am for a duration of one and a half hours by one inspector. The National Minimum Standards for Care Homes for Older People focus on achievable outcomes for service users – that is the impact on the individual of the facilities and services of the home. Evidence was looked for that the standards were being met and a good quality of life enjoyed by service users through: • Discussions with service users, families and friends, staff and managers. • Observation of daily life in the home • Scrutiny of written records The statement of purpose was used to assess how far the home’s objectives to be able to meet service user requirements and expectations were being met. Reports regarding an overview of the conduct of the home are sent to CSCI on a monthly basis by a representative of Morris Care. These, as well as the risk assessment from the last inspection were taken into account to determine the core standards focused on and depth of inspection. The Commission does not currently have any concerns regarding this home. What the service does well: Only qualified staff are involved in giving medication to service users. Procedures are in place to ensure the wrong drug is not administered. The manager recognises the importance of providing palliative care for service users in the terminal phase of their illness and aims to develop staff skills and expertise within this area. The manager and staff in working with other health care professionals have provided sensitive and professional palliative care for service users at the end of their life. It is the manager’s intention to participate in the nationally recognized approaches to palliative care as below, which would further enhance the quality of end of life care provided. • The Liverpool Care Pathway (LCP); and • Gold Standard Framework. The LCP format transfers the hospice model of care into the care home setting and will facilitate multi-professional communication. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,9,11 Through appropriate research, training and development the manager and staff at Radbrook are improving their skills to provide for service users’ needs for healthcare and support. EVIDENCE: The manager demonstrated her knowledge and intention to be more involved in the End of Life Initiative. A discussion was held regarding national frameworks as detailed in the summary. The home ensures that service users and their families have access to support through all stages of treatment. Medicines are kept safely with full records of their receipt, administration and disposal. Reviews of medication are conducted by the GP on a regular basis. The home stocks some non-prescribed remedies that are authorised by the GP. Wherever possible and depending on their capabilities, service users are enabled to take responsibility for their own medicines. Care plans regarding medication issues with individuals are fully recorded and regularly reviewed. Service users stated that they receive their medication from staff at the correct times of the day. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Staff are provided with induction and on-going training regarding adult protection. This provides staff with the relevant knowledge to safeguard service users from many types of abuse. The home has a complaint procedure that ensures complaints are dealt with promptly and effectively. Service users and their friends can be assured that what they say will be heard and dealt with. EVIDENCE: There have been two complaints since the last inspection, made to the home, one of which was related to medication administration. This had been dealt with in the time scales of the homes’ procedure and responded to in full. The quality team carries out an audit trail of all complaints in the home to identify any trends and to address any failings. The Chief Executive of Morris Care records a comment on each complaint seen. Compliment letters were seen in a folder in the foyer. On the whole, all service users spoken with were very happy with life at Radbrook. They said they knew whom they could speak to if they were worried about anything. Staff induction and training contains information regarding adult protection. A member of staff was receiving her first day of induction during the inspection. Regulation 37 incident reports are sent to the CSCI as required by legislation. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26 The domestic services are organised to ensure that the home is hygienic and that service users clothes and bed linen are always clean and fresh. EVIDENCE: Domestic staff were on duty during the inspection and all areas had been maintained to a high degree of cleanliness and hygiene. Service users stated that their clothes are washed nicely and are always returned to them. The manager is developing a post of infection control link trainer to liaise with the infection control advisor post at Shropshire Partners in Care to share information and teaching expertise. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 The home’s procedures provide a safe framework for the recruitment of staff that is followed consistently in order to protect service users. EVIDENCE: The staff recruitment files were seen of two the most recent employees. All required checks were in place that demonstrated the home’s vetting procedures were thorough. However, the two references recorded as having been received for one employee were not on file. CRB checks had been received after the start date but the induction process is very thorough ensuring that no member of staff is left to work unsupervised. The actual CRB forms are held at head office and could not be examined but the disclosure number is held on file. It is recommended that the home holds all CRBs for the purposes of inspection. Staff observed carrying out their duties were seen to be responsive and understanding of individuals wishes and needs. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,35 The manager is experienced and competent and management systems and practices ensure that the home is well run for the benefit of the residents. Service users personal monies are well managed so that their financial interests are safeguarded. EVIDENCE: Discussions with the manager demonstrated that she continues to strive for excellence and find innovative ways to provide the service at Radbrook. She is committed to service improvement and does this in a manner that involves all levels of staff, encouraging them to work as a team to improve outcomes for service users. Service users were complimentary about her style of Radbrook Nursing Home DS0000022270.V268717.R01.S.doc Version 5.1 Page 16 management, being one of involving and encouraging according to their abilities. One service user stated that the staff were “top class”. Residents’ valuables and sums of money lodged with the home for safe keeping were accounted for satisfactorily.
