This inspection was carried out on 8th June 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Feedback from service users and relatives indicated that people now feel generally satisfied with the care provided at Redwell Hills. They felt that the current manager has worked hard to lead and motive that staff team which has resulted in an overall significant improvement within the home. Comments from relatives included ` we are very happy with all aspects. My relative is well cared for and the food is excellent`.

What has improved since the last inspection?

All of the relatives and service users spoken to felt that there has been a general significant improvement within the home in most areas since the last inspection. Comments included ` I previously could not stay away from the home as I was concerned about the care. The current manager has listened to me and I now think things are really good. The home is well managed and the food is really good. If anything the people are over nourished.`

What the care home could do better:

There is currently a temporary manager within the home who has been employed by the company to cover areas where there is a vacancy. The company must appoint a person to be registered with the CSCI as a matter of urgency. Whilst there has been some improvement with regard to the assessment and care planning within the home, there must be further progress in the care planning so that staff know what to do for each resident. There has been some progress with regard to the tidying up of the environment in general and the temporary manager advised that plans are in place to improve this further. All service users and relatives spoken to confirmed that there is a lack of social activities available within the home. However a coordinator has recently been appointed to ensure that a suitable programme is put in place.

Date of last inspection 15th February 2005 Brief Description of the Service: Redwell Hills is a purpose built care home located in a residential area on the outskirts of Leadgate. The home provides accommodation for older persons offering 24 hr general nursing and residential care for service users. The home offers single room accommodation, several of which have en-suite facilities. There are adequate additional toilets, bathrooms and communal sitting/dining areas. The home offers attractive outdoor sitting areas with a pleasant rural outlook. This unannounced inspection took place on 8th June 2005 over a period of 6 hours. During the inspection some of the time was spent talking to the residents, staff and relatives. A selection of comment cards were given to service users with pre-paid envelopes provided. A tour of the building was also undertaken and a number of care plans were examined. Time was spent observing the care provided to residents in the home. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3 &4 Information is given to prospective service users to help them make an informed decision about moving into Redwell Hills. Appropriate assessment procedures are in place but these needs to include more information about peoples mental health, psychological and physical needs to ensure that the home can meet the needs of service users. EVIDENCE: Copies of the Statement of Purpose and Service User Guide were available in the reception area of Redwell Hills, along with copies of the home’s most recent inspection reports. There is still no statement of terms and conditions available to service users. The company are aware that this issue has been outstanding for some considerable time. Discussion with the manager indicated that all service users will be assessed before admission, to ensure that Redwell Hills is able to meet their needs and Redwell Hills Care Home B54 S742 Redwell Hills V232383 080605 Stage4.doc Version 1.30 Page 9 that relatives and people who may wish to move into the home can visit prior to making that decision. Individual records are kept for each service user and those inspected contained care management assessment and an assessment carried out by the home. Examination of an assessment of a person who had recently been admitted to the home, confirmed that comprehensive assessments are now undertaken. Redwell Hills does not provide intermediate care; therefore the assessment of Standard 6 is not necessary. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9&10 Individual care plans are in place, but little progress has been made since the last inspection about the requirement that these should be much more comprehensive to ensure that all care needs are appropriately met. EVIDENCE: Individual plans of care are available but little progress has been made to ensure that all aspects of health, personal and social care needs are planned for. Plans remain basic, most were not up to date and had not been reviewed. One of the records examined did not contain a care plan for each of the problems identified in the assessment and another service user had no care plans at all. Staff confirmed that the needs of these people were being met even though there was a lack of clear plans. This approach is dependant on informal verbal communication systems. Residents are at risk of not having their needs met if these informal systems break down. All staff who give out medication have been trained to do so and the drug administration sheets checked were found to be in order. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,14&15 Service users are encouraged and assisted to make a choice about their care. Throughout the inspection process it was evident that the lack of social stimulation is considered to be a major issue for the people who live there. Meals are nutritious and balanced, offering a varied diet for residents. EVIDENCE: There is now a policy in place to assist service users so that they know where their records are kept and how they can access them. Whilst it was identified that the people who live in the home consider the lack of social activity a problem, a member of staff has recently been employed who will be responsible for organising a suitable programme. A number of people living in the home and their relatives were spoken to throughout the inspection process and all confirmed that the food is excellent, with a varied choice offered. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 29 & 30 The procedure for the recruitment of staff is now considered to be robust and offers protection for the people who live there. Staff training has recently been implemented to ensure that staff have the skills to do their job. EVIDENCE: The staff files reviewed indicated that all of the required recruitment checks are now made before a new member of staff starts to work in the home. After talking to staff and reviewing records it was confirmed that recent traing has taken place in the areas of care planning, nutrition, food hygiene, fire safety, first aid, moving and handling and ‘Protection of Vulnerable Adults. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,35,36,37&38 The home does not currently have a registered manager in place, however the current arrangement is considered to be adequate on a temporary basis. EVIDENCE: Redwell Hills has a peripatetic manager in place. She has previously been a manager in another home within the company, but now covers in homes where there is a vacant post until a new manager is appointed. Service users and relatives spoken to during the inspection process confirmed that there have been significant improvements whilst she has been there and had hoped that she would stay. The company must now seek to appoint a suitable person to be registered with the CSCI. The company have now written to service users advising them of the pooled banking arrangements currently operating within the home. The temporary manager was able to demonstrate that as far as possible the health and safety of the residents, staff and visitor’s to the home is maintained. Health and safety records inspected were up to date and the appropriate staff training had been carried out.
