Conditions of registration: The 4 beds registered for Learning Disability are for Respite use only Date of last inspection 6 September 2004 Brief Description of the Service: Robert Bean Lodge is a purpose built home which includes a community day centre. All of the accommodation is on the ground floor in single rooms. The accommodation is divided into four units, each containing ten bedrooms and separate day space. Medway council runs the establishment, which is registered for forty older people. The home is on a bus route and has off road parking available. The home employs care staff, working a roster, which gives 24-hour cover. The home also employs other staff for catering and domestic duties. In addition the home is Temporarily accommodating a Learning Disability respite care service. This is registered for 4 persons and is self-contained with its own staff team. This is an overview of what the inspector found during the inspection. This was an announced inspection, the first in the year running from April 1st 2005 to March 31st 2006. The visit lasted from 10.00am until 15.00pm. The visit was spent talking directly with residents privately and collectively ; 6 care staff, 3 relatives, a community nurse and the registered manager. Some judgements about quality of life and choices were taken from direct conversation with residents and their relatives and observation followed by discussion with support staff and evidencing records held at the home. A tour of the premises was undertaken. Residents and relatives spoken to during the inspection and comment cards received gave positive feedback about the staff and services the Home provides. Many residents and staff commented that things have improved since the current manager took up post, but it was also clear that some staffing issues still remain. What the service does well:
Residents benefit from living in a well maintained and spacious Home, which is clean and free from offensive odours throughout. On the whole residents benefit from staff who have the skills and training to meet their needs and who are appropriately supervised and supported by an effective management team. The Home has a day centre onsite and offers respite care as part of its linked service provision. The inspector received very positive feedback regarding the food provided in the Home. What they could do better: The safety of residents could be improved by ensuring that all radiators have low temperature surfaces or are appropriately covered to prevent burning. The management of residents’ finances could be improved by ensuring that their money is held in an individual named account and not pooled. The quality of the environment in some residents rooms could be improved by having carpets fitted and replacing institutionalized hospital beds with ones that are domestic in character. The quality of the environment and safety and independence of residents could be improved if the Home actioned the recommendations made in the Kent Association for the Blinds assessment of the premises and facilities. The quality of life and health and safety of residents could be improved by providing opportunity for residents to undertake regular physical exercise. The quality of life and health and safety of some residents could be improved by ensuring that the Home receives feedback from residents Fall Assessments undertaken by the NHS Falls Clinic and that any recommendations are included in residents care plans. The health of residents could be improved by ensuring that opportunities for regular eye and dental check ups are provided and that the importance of maintaining good mouth and eye care is promoted by the Home.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,5 Prospective residents benefit from being able to visit the Home and know that the Home will meet their needs before they agree to stay. EVIDENCE: The inspector spoke at length with one resident that had recently moved into the Home. She confirmed that she had received a visit from staff and had been able to see the Home before agreeing to move there. Records also showed that a full Care manager Assessment was in place. The manager confirmed that all prospective residents are visited and a full assessment is undertaken before a place in the Home is offered. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 One the whole residents benefit from having their needs assessed and met but some improvements are needed to ensure that the assessment and care planning system is entirely effective. Residents benefit from being supported by a sensitive and caring staff team. EVIDENCE: A sample of care plans and assessments seen showed that reasonably comprehensive plans were in place. However it was noted that although residents are regularly referred to the NHS Falls Clinic as appropriate, the Home does not receive feedback from these assessments and are therefore not able to incorporate any recommendations into residents care plans. A requirement has been made that the Home seek a copy of the full assessment for the benefit of residents. Discussions with residents and the manager indicated that access to dental and eye care was dealt with on request or by families. The Home did not seem to properly monitor or promote access to these services, consequently many residents spoken with had not used these services since moving into the

A requirement has been made that the home ensure that opportunities for regular eye and dental check ups are provided and that the Home evidence that the importance of maintaining good mouth and eye care is promoted by the Home. The manager confirmed that the Home does not currently offer opportunities for residents to receive regular physical exercise and a requirement has been made regarding this. Feedback from health professionals indicated that feedback and communication systems between themselves and the Home could be improved and a recommendation has been made that the Home look at ways of doing this. Feedback from residents and their relatives was unequivocally positive regarding the care and consideration showed by staff. All residents spoken with and feedback on comment cards demonstrated that residents were happy with staff and felt safe in the Home. One resident said “ Staff are very good, no one gets angry with you”. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 Residents know that their concerns and complaints will be listened to and acted upon. Residents benefit from being supported by staff who are appropriately trained to identify and protect them from abuse. EVIDENCE: Residents and relatives spoken to and information taken from feedback cards confirmed that they felt able to raise concerns and complaints and were aware of the homes complaints procedure. The homes pre inspection questionnaire states that the Home has had one complaint in the last 12 months that was partially substantiated and was dealt with within 28 days. The Home has An Adult Protection Policy and procedure which includes whistle Blowing and staff spoken to confirmed that they had received Adult Protection training and demonstrated knowledge and understanding in this area. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,23,24,,26 Residents benefit from living in a clean, bright and spacious Home but improvements are needed to the environment in order to promote the safety and independence of residence and to create a more homely look to the place. EVIDENCE: A tour of the premises was undertaken, this was clean bright and free from offensive odours throughout and the garden areas were safe, attractive and accessible. Each unit has its own lounge and dining area and in addition the Home has a visitors room. There were adequate washing and bathing facilities and the bathrooms were in the process of being upgraded. Some of the residents’ beds were institutional hospital beds and several rooms did not have carpets. Some residents and their relatives said that they would like a carpet and a bed that was more domestic in character. A requirement has been made that the Home assess residents needs and wishes in this regard and take action to provide carpets and upgrade beds as appropriate.

An assessment of the premises and facilities has been undertaken by the Kent Association for the Blind, although none of the recommendations have yet been acted upon. A requirement has been made that the Home produce an action plan detailing how it will address the recommendations raised in the report. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29,30 On the whole the homes training and recruitment procedures ensures the safety and wellbeing of service users is promoted, but some minor improvements are needed to ensure this is entirely effective. EVIDENCE: A sample of staff files were inspected including the most recently employed member of staff. Records and discussion with staff evidenced that appropriate recruitment procedures are followed. Staff also confirmed that a range of mandatory training was provided by the Home and the homes pre inspection questionnaire stated that 95 of staff have completed NVQ level 2 or above. It was noted that the Home does not offer residents the opportunity to undertake physical exercise and the manager informed the inspector that the homes activities coordinator is not trained to do this. A requirement has been made regarding providing physical exercise and a recommendation has been made that the homes activities coordinator is trained to undertake this task and other activities that she may be required to facilitate. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,35,38 Residents benefit from a Home that is managed in their best interests with an appropriately supervised staff team but some improvements are needed to improve the health and safety of residents. Financial systems to safeguard residents interests need to be improved. EVIDENCE: The Manager has NVQ 4 and Diploma in Management Qualification and has several years experience of managing services. The inspector is satisfied that there are clear lines of accountability within the Home and external management. Discussion with residents and families indicated that any concerns and issues raised would be addressed appropriately. Refer to Standard 35 Good Practice Recommendations The following is recommended:Southampton City Council have managed to find a good solution to the issue around reg 20, and the need to avoid pooling service user monies. The system theyre adopting is called Clients Monies Service (CMS) from Nat West. It involves one account, with separate sub-accounts within it. The sub-accounts carry their own number and are interest bearing, therefore ensuring interest is calculated on the amount in each account etc. Monies can be managed in relation to each sub-account by the authoritys finance department through a modem link to the bank. It is recommended that the look at ways of improving communication and feedback systems from health professionals.
