What the care home does well

What has improved since the last inspection?

The registered provider did not have any statutory requirements made at the last inspection. Relatives and residents consistently speak highly of this service. Comments from relatives and residents questionnaires are used by the provider to ensure that the service continues to meet peoples needs.

What the care home could do better:

Some attention is required to ensure that risk assessments are updated. However, this did not in any way show that care is poor and all staff were very aware of their duties and responsibilities and particular needs of residents.

It is a victorian house situated in a residential area near to Horsham town centre. It has fourteen bedrooms of which three are double and four have ensuite facilities. The home is registered to provide care to people over the age of sixty five who are not in need of nursing care. This unannounced inspection took place during the morning and early afternoon. The inspector had lunch with residents. The inspector walked around the home, met with three care staff, the deputy manager and spoke with residents. Records were checked to validate responses from staff and residents and the service was found to be working to a good standard. The registered owner who is also the manager was away on holiday at the time of the inspection and so this was undertaken with the deputy manager. The overall quality of care provided is very good with all residents expressing satisfaction with the care given. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3 and 6, The home provides clear information to prospective residents and relatives about what the service will be able to provide and ensures it is able to meet peoples needs before they are admitted. EVIDENCE: The Statement of Purpose has been reviewed and updated in the last year and gives accurate details of what services the home can provide. One resident said she didn’t have a Service Users Guide but “Mrs P takes everybody through things about the home when you arrive”. Residents are assessed for suitability using an assessment plan called Minimal Data Set in Care. This is completed with relatives, care manager and the resident. All people move into the home for a two month trial period to allow time to decide if it is the right placement. The home does not admit people in an emergency. The home does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 10 All residents have a plan of care that assists staff to ensure that people wishes and needs are met and they are treated with dignity and respect. EVIDENCE: All service users have a care plan that contains details about health personal and social care needs and has the correct details to enable staff to provide appropriate care. The inspector asked residents about visiting medical staff and all confirmed that they saw doctors, chiropodists, opticians and were assisted to go to appointments at hospital. Staff are required to sign a code of conduct about health procedures. Care plans were seen and the inspector talked to residents about their plan of care. Some care plans need to be updated to show that risk assessments have been updated and reviewed. A resident said that staff were ‘very nice and helpful and that she could go to bed and get up when she wanted and get whatever help she needed’. This type of response is consistently received from residents in this home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 and 15 Residents are able to retain links with friends, family and the local community and are assisted by staff to do so. Residents enjoy a wide range of food and are involved in the choice of menus. EVIDENCE: Individual wishes of residents are detailed in the pre admission assessment and when people move into the home records are updated on the activity charts each month. Records seen showed that people enjoy a good range of activities. A resident said she had lived in the home for six months and that her visitors were made welcome at any time. The inspector spent time with a large number of residents and all confirmed that visitors were made welcome. The home employs an activities organiser and this person arranges activities that take place both in and outside the home. Residents discuss activities at the Forum they have every three months. The notice board is used to display notices about activities. A resident said that she was able to go to her local church, she had a daily paper delivered and enjoyed lots of outside contact. The inspector had lunch with residents and comments from them included ‘lovely food,’ ‘the cook is very good’ and ‘we are never hungry’. Residents said there is always an alternative on the menu and records seen showed a good choice of food was available. Residents take part in a forum to discuss food. The home has two cooks who are both qualified. The Environmental Health Department inspected the kitchens in May and did not make any requirements. Breakfast is served in residents rooms and they can chose where they have other meals. The dining room was comfortable and well decorated. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Procedures are in place to ensure action required by staff to protect residents from abuse are complied with by staff. The Complaints Procedure is understood by residents. EVIDENCE: The Complaints Procedure was on display and the record seen showed there had been one complaint since January 05 which the registered manager had satisfactorily dealt with. The Commission has received no complaints about this service. Residents told the inspector what they would do if they had a concern. The procedure is also in the Statement of Purpose and Service Users Guide. The home has a policy on ‘The Protection of Vulnerable People’ and the provider is up to date on training with regard to Adult Protection. There have been no referrals to the West Sussex Adult Protection team in the last inspection year. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,21 24 and 26, The environment provides a safe place for people to live in which remains homely and comfortable. The home provides a wide range of activities for residents to participate in if they wish to. EVIDENCE: Rosedale is an adapted property and an audit has been taken to ensure that residents are safe in all parts of the building. This includes a ramp to provide a level throughway on the ground floor. The home has a stair lift to the first floor that is maintained in good order. Work is currently being undertaken to cover all radiators to ensure that a resident could not be burned if they accidentally fell against one. The home has window restrictors in all rooms on the first floor. All water outlets have been fitted with a thermostatic mixer valve with the exception of one bathroom where it is not possible to do so. A risk assessment is in place for this bathroom. All bedrooms were furnished comfortably and people are encouraged to have their own possessions. The three double bedrooms are fitted with privacy screens. The premises were very clean and procedures in place to ensure that good hygiene and infection control measures are in place. The inspector advised the deputy manager who was on duty at the time of this inspection to ensure that the fire service were satisfied that the risk assessments with regard to fire safety were up to date as the record available showed this to have last been done in 2003 Rosedale H60-H11 S14688 Rosedale V227558 120705 Stage 4.doc Version 1.30 Page 17 Staffing The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 and 30 All staff are well trained and carry out their duties with commitment and a good level of knowledge of individual residents needs. EVIDENCE: Four staff records were checked and all contained the correct details to show that the home was following a recruitment procedure that ensured the safety of people living in the home. Staff and residents felt the number of staff on duty was good and that they were trained to be able to do the job well. Records seen showed that the staffing levels were sufficient. Staff talked to the inspector about their training and all were encouraged to undertake NVQ training and attend short courses and in-house training. All staff receive induction training that meets the guidelines given by the National Training Organisation. Residents who spoke to the inspector were all very satisfied with the care provided and the way staff treated them. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32, 33,35,36,37and 38 The leadership and management of the home is good with staff and residents feeling confident with the management. EVIDENCE: The manager who is also the registered owner has many years experience in caring for older people. She has a social care qualification and a National Vocational Qualification Level 5 in Management. Mrs Pavoni also holds an approved assessor award for NVQ. Mrs Pavoni is an active member of the Care Training Consortium, the multi agency Adult Protection Group and the NVQ Partnership and is committed to keeping up to date with training. Supervision is given to staff in a variety of forms including group meetings, questionnaires and individual meetings. The home has a quality assurance programme that involves relatives and the results of these questionnaires are used to develop the service. Inspectors have seen copies of these.
