CSCI found this care home to be providing an Good service.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 6 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The service user guide has been updated to reflect the current management arrangements in the home, the kinds of training staff have received and the contact details for the Commission for Social Care Inspection. Progress has been made in meeting the recommendation made at the key inspection to improve staff`s knowledge of multidisciplinary safeguarding procedures. Suitable training has been arranged for both the acting manager and the staff team. A greater variety of fresh foods (such as vegetables) is being offered to people to try and encourage them to eat a more varied diet. A referral has been made to a dietitian for advice on how to improve the diet of one person living in the home.

What the care home could do better:

The way that the home assesses new people who want to come and live at Rosedale must be reviewed to ensure that the service is able to meet the needs. Clear written guidance must be in place for staff members on how to meet the needs of people who come to live in the home, to ensure that staff know how to meet their needs. It is strongly recommended that all staff with responsibility for administering medication have their competence in this area reviewed. An application form must be completed and two written references obtained for all people who work in the home. Staff records must accurately record when a person started working in their current post. The staff rota must accurately reflect which staff work at the home on which days together with their hours of work.Any disciplinary action taken against a staff member must be accurately recorded in their staff file.

Date of last inspection Brief description of the care home Rosedale is a registered care home for up to six people who have learning disabilities. It is in Whitwick, and is close to shops and other places for people to use. There are bus stops nearby, for buses to Coalville and other local towns. There are six bedrooms in the home, downstairs and upstairs. The bedrooms have toilets next to them. There are shared lounge and eating areas, and bathrooms for people to use. The basic fee for staying in the home is #900 per week. Additional one-to-one support is charged at an Care Homes for Adults (18-65 years) Page 2 of 10 hourly rate. Extra charges on top of fees are made for hairdressing, chiropody, dental and other healthcare costs, and magazines and papers. Care Homes for Adults (18-65 years) Page 3 of 10 What we found: An unannounced visit was made to Rosedale between 11:20 and 13:50 on 1st October 2008. The visit was made because of issues raised in a safeguarding alert from Leicestershire Social Services concerning a recent admission to the home. The visit was also used to check on the services progress in meeting the one requirement and three recommendations made at the key inspection of the service on the 3rd June 2008. A social worker’s community care assessment was in place for the most recently admitted service user but there was no documentary evidence that a representative from the home had completed an assessment of the persons needs. Certain issues were highlighted in the social worker’s assessment but there were no risk assessments in place at the home or written guidance to staff on how to manage these. The acting manager said that she had not met the person before they moved in, as the responsible individual had dealt with the admission process. Other members of staff had met the person when they came to visit the home. A requirement was made at the key inspection to ensure that suitable storage was available for controlled medication. This was to ensure that the service complied with recent changes to Regulations under the Misuse of Drugs Act. A suitable cabinet has been ordered and the acting manager said that she would inform the Commission when this had been installed. The recent incident at the home reported under safeguarding, alleged that medication had been administered at the wrong time. The daily records at the home indicated that medication had been given to the person concerned 1 hour too early. Four staff have authority to administer medication and have undertaken suitable training in this area. This includes the person who administered the medication too early. None of the people currently living in the home take any prescribed medication. Staff rotas do not accurately reflect the shifts worked by members of staff. Of the four staff members who were known to be present when the safeguarding incident took place, two were not identified as having worked on that night/day. The staff rota did not accurately reflect the staff working at the home on the day of the inspection. The acting manager said that a staff meeting was scheduled for that afternoon and all staff would be reminded that shift changes must be approved by her first and then accurately recorded on the rota. We discussed the safeguarding incident (in which a person who had come to live at Rosedale left the home without staff being aware) with the staff member who had been on night duty at the time of the incident and with the acting manager. We also read statements made by all four staff that were on duty during the night/day when the incident occurred. The information given does not account for a period of time between 07:30 and 09:15 am when the incident occurred. It appears that neither of the two staff members on duty checked that the person was safe or offered them any assistance to meet their needs during this period. The acting manager said that the senior support worker on duty at the time had been Care Homes for Adults (18-65 years) Page 4 of 10 disciplined and demoted to the post of support worker. However there was no written evidence of any disciplinary action taken on the staff members file. A completed application form and two written references were in place for all staff currently working at the home as support workers. All support workers names had been checked against the vulnerable adults register and all except one staff member had an enhanced Criminal Records Bureau (CRB) check. This person does not work unsupervised in the home and their CRB has been applied for. One person employed as a maintenance worker had not completed an application form and references had not been obtained. A CRB was in place for this person and their name had been checked against the vulnerable adults register. There was no start date recorded for one support worker and the date when the acting manager started her position was also not recorded.
