The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home provides a good standard of care. Residents have access to healthcare as necessary and are offered a balanced and healthy diet. Residents said that their needs were being met and that the staff were friendly and helpful. Activities and entertainment within the home is well thought out and varied and residents spoken with were happy with the choice available. Regular resident meetings enable residents to air their views, have a say in the running of the home and to be kept informed of issues that may affect them.

What has improved since the last inspection?

Additional ramps and handrails from the fire exits have been provided since the last inspection. This should provide residents with a safe exit from the building in the event of an emergency.

What the care home could do better:

There needs to be an overall improvement in the checks made on staff before they start working at the home, to ensure that residents are protected. The home has been without a manager for sometime. The recruitment of a manager is essential to ensure that the above areas are improved upon.

The home is located in Woolston not far from the local shops and within a short distance from Weston Shore. All rooms for service users are on the ground floor.The staff sleeping in room is located on the first floor. Service users have access to a large lounge/diner and all service users have single rooms with shared WC facilities between rooms. The home is set back from the main road and offers a rear garden for service users. Rosewood Care Home Version 1.10 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This inspection was an unannounced inspection that took place over six and half hours. The purpose of this visit was to carry out an inspection of the home and follow up on requirements made at the last inspection. A tour of the premises took place that included looking at service user’s bedrooms. Staff and care records were inspected. Two visitors, three members of staff and six of the eleven service users currently living at the home were spoken to. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,4 and 5. The home operates a thorough admission and assessment procedure ensuring that they can meet peoples’ needs prior to them moving in. EVIDENCE: As part of the admission process to the home all prospective service users are assessed to determine whether the home can meet their needs. As part of this procedure prospective service users and or their representative have the opportunity to visit the home, look at the type of accommodation on offer and find out about day-to-day life in the home. Service users and their relatives confirmed that they had been given this opportunity and that this assisted them in making their choice of home. Individual records are kept for each service user and from inspection of the records for two recent admissions to the home, full assessment information had been recorded, that included the homes’ own assessment and that of placing agencies such as social services. This demonstrated that the home had ensured that they were able to meet the service user’s needs prior to offering them accommodation. All service users spoken with said that they considered that their needs were being met and that they received the appropriate level of support. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9 and 10. Service users receive adequate and appropriate support with their personal and health care needs. EVIDENCE: Each service user has an individual plan of care detailing their care needs and these provide guidance as to how these needs are to be met. Included in each care plan is risk assessment. Where specific risks are identified such as mobility or falls then measures are put in place to minimise these. In discussion with service users their description of their personal care needs matched with the details recorded in the care plans. Staff were able to demonstrate that they were aware of service user individual care needs. Service users said that they were happy with the level of assistance that they received. All service users are encouraged to be involved in their care planning to ensure that their wishes are taken into consideration and to promote their independence. Service users told the inspector that they had access to a range of healthcare support as and when necessary and written documentation supported this. Rosewood Care Home Version 1.10 Page 11 Service users also confirmed that they were happy with the way that their medication was managed and that they received them regularly and at the correct time of the day. Medication is safely and appropriately managed in line with written procedures. At the time of this inspection staff were undergoing further medication training. Service users told the inspector that staff respected their privacy and that assistance with personal care was carried out in private. This was supported by observations during the inspection. All service users have single rooms, where they can meet visitors in private. Rosewood Care Home Version 1.10 Page 12 Daily Life and Social Activities The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13 and 15 Social activities are well managed, creative and provide variation. The meals in the home are good, offering both choice and variety. EVIDENCE: Service users told the inspector that they were free to please themselves how they spent their day and that they did not have any restrictions placed upon them regarding what time they should get up or go to bed. On the inspector’s arrival to the home a clothes sale was taking place and service users commented on their satisfaction with the level of organised activities such as visiting musicians and quizzes. Service users are consulted about the types of activities that they wish to engage in and have provided. It was evident that thought and planning had gone into providing suitable and appropriate activities based on the needs of the service users. Staff were observed to spend time chatting to service users on an individual basis. All service users spoken with confirmed that they were free to receive visitors as and when they pleased. A visitor to the home said that they could visit their relative as often as they chose and was made to feel welcome. Rosewood Care Home Version 1.10 Page 13 Service users expressed their satisfaction with the food. Service users comments included, “good” and “lovely”. Menus were inspected and found to be varied and balanced. Service users said that they were happy with the quantity and choice on offer and that drinks were available throughout the day. Rosewood Care Home Version 1.10 Page 14 Complaints and Protection The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 Satisfactory systems are in place for service users to address any concerns or complaints that they may have. EVIDENCE: There had not been any complaints made about the home since the last inspection. A complaints procedure is displayed within the home informing service users of their right to complain and how they can go about this. The procedure had been discussed and explained to all service users at a recent meeting and relatives confirmed that they had also been made aware of the their right to make a complaint. Service users commented that if they were unhappy with anything they would inform the deputy manager and they felt confident that she would deal with it. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,22,24,25 and 26 Generally service users live in a safe, clean well-maintained environment. This is at times compromised by poor practice. EVIDENCE: On the day of the inspection the home was clean, tidy, warm and well maintained throughout. A maintenance person is employed to deal with repairs as and when necessary and regular checks are completed to ensure the home is safe and all is in working order. Requirements from the previous inspection regarding the installation of ramps and handrails from the fire exits had been addressed. This should provide service users with a safe exit from the building should the need arise. A call bell system is fitted throughout the building to enable service users to summon staff. Service users reported that these were responded to promptly. Adequate safety precautions are in place to minimise risks to service users. This includes radiator covers, emergency lighting and fire detection equipment. Rosewood Care Home Version 1.10 Page 16 Accommodation is provided on one level and consists of twelve single bedrooms with interconnecting en suite toilets, a large lounge, dining room and bathrooms. All bedrooms contained adequate furnishings and service users had been able to personalise them with their own belongings. The only concern from looking at the physical environment was that plastic bags were being used to protect the mattress on one service user’s bed. This does not promote the dignity of the service user or provide comfort and would seem unnecessary as all other mattresses in the other bedrooms had a fitted mattress protector and there were adequate supplies of them. This has been raised at a previous inspection. Rosewood Care Home Version 1.10 Page 17 Staffing The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 29 Staffing levels are maintained and structured around the needs of those living at the home. However, the procedures for the recruitment of staff are not robust and do not provide the safeguards to offer protection to people living in the home. EVIDENCE: The home maintains a reasonably consistent staff team enabling service users and staff to get to know one another. Adequate staffing levels were in place to ensure that the needs of the service users could be met. The home also employs a part time cleaner. This enables care staff to spend more time attending to the needs of service users. The organisation of the staff rota had recently been reviewed and care staff duties had been reorganised. This had come about following a meeting with service users where they had requested a review of the bathing rota to enable them to have the same carer to assist them with their bathing. Staff demonstrated that they were aware of their role and reported that they thought that the restructuring of their duties was more efficient. Service users and visitors to the home said that staff were helpful, responsive to their needs and all were happy with their attitude and felt well treated. The inspector observed this to be the case. One commented, “ If I wake up in the night they always help me”. The staff files of two members of staff recruited since the last inspection indicated that the home had not undertaken all the necessary recruitment Rosewood Care Home Version 1.10 Page 18 checks to ensure the protection of service users. Criminal Records Bureau checks had not been obtained for one staff member neither had a check against the Protection of Vulnerable Adults list been made. References were not on file, nor was evidence that the staff member had been through an induction period. Although adequate checks had been completed for the other staff member there were discrepancies in the dates on some of the records, suggesting that they commenced employment prior to submitting an application form. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,33 and 38 The absence of a manager results in some practices that do not safeguard the welfare of service users. The home is however responsive to the needs of those that live there. EVIDENCE: The home has not had a registered manager for some time. The current arrangement is that a manager from one of the other homes within the Mycare organisation visits on a regular basis, as does the Area director. A deputy manager is employed at the home and is clearly well thought of by the service users and does oversee the day-to-day running of the home to a good standard. However the support of a full time manager is needed to ensure that all responsibilities such as correct recruitment procedures are followed to safeguard service users. Service users do benefit from having regular meetings to air their views and it was clear that their needs and preferences are listened and responded to. The Rosewood Care Home Version 1.10 Page 20 minutes from a recent meeting demonstrated this. Relatives and other visitors to the home are also asked their opinion about the quality of care and general standards in the home and are invited to complete a questionnaire. The home appeared to be safe and from inspection of the fire log book regular and thorough testing of the homes fire detection equipment had taken place. Fire safety and fire evacuation procedures are discussed with service users during meetings. Evidence was also seen to demonstrate that regular servicing of gas and electrical appliances are carried out and that food hygiene is maintained.
