This inspection was carried out on 16th January 2006.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home has a homely and friendly atmosphere, and is furnished and decorated to a high standard. The resident`s praised the care they received and said the staff were kind and caring. On the day of the inspection some of the residents were participating in painting mugs and all said they enjoyed creating their own design and using the mug afterwards. The staff receive the appropriate training for them to carry out their work and many of the staff have worked in the home for several years.

What has improved since the last inspection?

All the required information with regard to references has been obtained for the member of staff who was employed from Latvia. Formal staff supervision has commenced for all the care staff and is being carried out at least six times a year. Controlled Drugs are recorded in a separate register. The staff had received training in food hygiene and infection control. The water temperatures were monitored and recorded each month. The home had made progress in contacting a training company about an induction and foundation programme for new staff.

What the care home could do better:

Window restrictors should be fitted to all windows above ground level. Written evidence should be available that all new members of staff have completed an induction into the home in their first few weeks of commencing work, and on completion, signed by the member of staff. The Responsible Individual for the care home should visit the home unannounced at least once a month and produce a written report on the findings of the visit, and provide the Commission of Social Care Inspection with a copy.

The total number of persons accommodated should not exceed 19. Date of last inspection 12th August 2005 Brief Description of the Service: Rowfant Care Home provides personal care and accommodation for nineteen older people over the age of 65 years. It is owned by Rowfant House Limited a non-profit making organisation which is affiliated to the Latvian Church. The Registered Manager responsible for the day-to –day running of the home is Mr Viesturs Dude and the Responsible Individual is Mr Mike Ward. The home mainly accommodates elderly Latvian people some of whom have lived in self catering accommodation in a large community/country club (Rowfant House) in the same grounds prior to moving into the care home but also admits residents who are not Latvian. The two-storey detached building was a converted stable block, which has been extended and added to during the last five years. All the resident’s bedrooms are single and three offer en-suite facilities and there is a passenger lift giving access to all floors. The home is set in extensive grounds, which include woodland and a river. There is a large patio area to the front of the property with a fishpond and raised flowerbeds, which are accessible to the residents. The unannounced inspection took place over 4 hours and the Registered Manager Mr Viesturs and the Deputy Manager Irene Cherry were present at the inspection and provided the information required. Records were examined with regard to the resident’s care plans and assessments, complaints, maintenance checks, medication and staff training. A tour of the premises was made and eight residents and two members of staff were spoken with. Not all the standards were assessed on this inspection and those not assessed were fully met at the last inspection in July 2005. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2,3 All residents have a copy of the terms and conditions of residency. EVIDENCE: All the residents have a contract and Statement of Terms and Conditions that are signed by the resident or their relative on moving into the home. The contract explains the aims and objectives of the home, the fees, termination of residency, accommodation, personal possessions, care and services, medication, health and safety and complaints. The Registered Manager or Deputy Manager carry out an assessment of all the residents prior to any agreement being made for admission to the home. However, in the past the pre-admission assessment was not always recorded, and it was by verbal agreement that the resident was admitted to the home. However an assessment form has been obtained and will be completed for all future admissions. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 The residents are well cared for. The medication procedures for the home ensure safe practice. EVIDENCE: Three of the resident’s care plans were examined that identified the health, social and personal care needs of the residents. However, one of the care plans identified the resident required assistance when mobilising, but did not indicate in detail the level of assistance required. The risk assessments and moving and handling assessments were incomplete due to the fact that they were in the process of being reviewed and re-written. All the resident’s spoken with, were more than happy with their care and said the staff were attentive and caring. The medication administration records were examined and accurate records were maintained. The Controlled Drugs administered were recorded in a separate register and stored in a locked box in the medicine trolley. None of the present residents wish to handle or take their own medicines. The residents spoken with felt that their privacy and dignity was respected by the staff and said the staff were courteous in their approach. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 The residents are able to have visitors at any reasonable time and participate in any local events of their choice. EVIDENCE: The home has an open policy on visitors, and welcomes visitors to the home at any reasonable times. The residents confirmed that their visitors are always made welcome and if pre-arranged can have a meal with them. The residents are able to get involved in events in the local community if they wish. The home has a strong link with the Latvian Church and visit the home on occasions. Rowfant House that is situated in the grounds of the care home is a social and country club, and the residents are able to participate in the social events held there, and can use the restaurant and bar facilities. The home does not have any volunteers working in the home. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,17 The residents are given clear information on how to make a complaint. The home ensures the resident’s are able to exercise their legal rights. EVIDENCE: The home has a clear complaints procedure giving details on how and to whom to make a complaint. The complaints log has no recorded complaints for over two years. Any minor complaints or concerns that can be resolved at the time are not recorded in the complaints log. The residents spoken with said they would not hesitate to speak with the manager or deputy manager should they have any concerns. Arrangements for the residents to vote in local elections are provided either by postal voting system, or the resident can visit the local polling station. Arrangements are also made for the residents from Latvia that wish to continue to vote in Latvian elections. Information on advocacy services would be obtained for the resident on request. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 21,26 The bathroom and toilet facilities in the home meet the needs of the residents. The home is kept clean and hygienic. EVIDENCE: The home has sufficient bathroom and toilet facilities to meet the needs of the residents. There are two assisted bathrooms and six toilets. Three of the resident’s bedrooms offer en-suite facilities and a further bedroom is in the process of having an en-suite fitted. The home was fresh and clean on the day of the inspection and the sluicing and laundry facilities were appropriate for the number of residents accommodated. The home does not employ a laundry assistant, and the care staff handle and deal with the resident’s laundry. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,29,30 The number of staff on duty is sufficient to care for the residents. The recruitment procedure for the home ensures all the appropriate checks are carried out on new staff. An induction programme for new staff needs to be developed. EVIDENCE: The number of staff on duty on each shift appears appropriate to meet the needs of the residents accommodated. There are three care assistants working during the day and either the Registered Manager or Deputy Manager are on duty, except on Saturday, when the on-call system is in place. During the night there is one care assistant plus one sleeping-in. The home has not employed any new staff since the last inspection in July 2005. However at the last inspection it was found that references were not available for a new member of staff from Latvia. One reference had since been obtained for that member of staff that is kept on file. The home carries out an induction for new staff during the first few weeks of their employment that includes fire procedures, the aims and objectives of the home and an understanding of some of the home’s policies and procedures. However there was no written evidence available, as to the content of the induction and that all the staff had completed it. A training company has produced a workbook that is in the process of being completed by the staff. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 34,36,37,38 The staff are provided with the training necessary for their job and are given regular supervision to ensure safe working practices. The Registered Manager must ensure the health and safety of the residents and staff is protected. EVIDENCE: A valid insurance certificate for the premises and home was displayed in the entrance of the home. The home produces a development plan at the beginning of each year and is in the process of completing one for 2006. The plan indicates what needs to be done to improve and develop services taken from the comments and feedback from the residents, relatives and staff surveys. Rowfant Residential Home DS0000014695.V273225.R01.S.doc Version 5.0 Page 17 The home has commenced formal staff supervision that takes place at least six times a year and is carried out by the deputy manager. The resident’s and staff records are kept secure in a locked filing cabinet in the main office. The maintenance records showed that the systems and equipment in the home are serviced and maintained within the required timescales. The Registered Manager records and monitors the water temperatures around the home on a monthly basis. It was noted that the windows above ground level still did not have restrictors fitted. However the home did have one window fitted with a type of restrictor that proved unsatisfactory, as stopped the window from opening wide enough to get any fresh air. On touring the building it was found that one of the resident’s bedroom door was held open by a door wedge, although the bedroom door had a door guard fitted for this purpose. All the staff had attended training in manual handling, first aid, food hygiene, infection control and fire safety. The Responsible Individual for the home is required under Regulation 26 to do an unannounced visit to the home at least once a month, and prepare a written report on the visit and send a copy to the Commission of Social Care Inspection. The last visit carried out was in August 2005. This requirement is outstanding from the last inspection in July 2005.
