Conditions of registration: None Date of last inspection 2.03.05 Brief Description of the Service: Russell Green is a privately owned and managed care home, registered for 18 older people. The home is situated in the large village of Woodall Spa and within close proximity of local shops and community services.The proprietors live on the second floor and run the home as a family type business. Because of this they are readily available day and night and they keep in touch with service users and staff on a daily basis. The home is well maintained internally and externally. Since taking responsibility for the home the proprietors have made extensive improvements in both the environment and the care practise. This is an overview of what the inspector found during the inspection. This unannounced inspection took place with the assistance of the manager and co-proprietor. One inspector undertook the inspection spending six hours in the home. The inspector toured the building, spoke with many of the residents living in the home. The main method of inspection used was called ‘case tracking’ which involved selecting three residents and tacking the care that they receive through the checking of their records, discussion with them, the care staff and observation of care practices. What the service does well: What has improved since the last inspection?
The proprietors have purchased equipment for the home and garden and have also purchased a vehicle, which allows them to transport residents, one of whom can be in a wheelchair, to follow leisure pursuits or to attend medical appointments. The home has an ongoing maintenance programme. New windows have been fitted. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10 and 11 EVIDENCE: Each resident has a care plan which is reviewed on a regular basis. Care plans seen, recorded residents’ choices, likes and dislikes and gave a good picture of each individual person. There was evidence that when necessary, staff in the home seek the advice of specialist health professionals. A visiting community nurse said that she visits residents and offers care in the privacy of their own rooms, saying the home has a ‘lovely family atmosphere’. The nurse said that there are good working relationships with the home and the medical team. Medication was seen to be offered to residents appropriately and staff administering medication were given training on the management of medicines in June 2005. One resident prefers to look after their own medication and has been provided with lockable storage. Six comment cards were received from relatives residents all of them saying they were pleased with the overall care their relatives receive in the home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15 Residents are encouraged and supported in continuing their leisure pursuits. The food provided in the home is of good quality with plenty of choice made available. EVIDENCE: The home produces a regular newsletter, with June’s newsletter advertising a garden party with a barbeque and live music for residents and their friends and relatives to attend. Each resident is asked about the activities that they would like to participate in which is recorded, with the emphasis very much on individual preference rather than communal activities. A new vehicle has been purchased for the home that is suitable to offer wheelchair transport and the manager takes individual residents out on shopping trips, trips to the seaside or trips out for lunch. A communion service is held in the home on a monthly basis. The midday meal was well presented, offered choice, and residents said they had very much enjoyed it. EVIDENCE: Information received prior to the inspection, indicated that the home had had one complaint, which was dealt with within 28 days. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20 and 24 Residents benefit from living in a comfortable and homely home and are able to make their own room suit their needs. EVIDENCE: The home was comfortable and homely, cleaned and maintained to a high standard both inside and out. The proprietors are committed to an on going maintenance programme which allows a budget for new equipment to be purchased and improvements made to the home when necessary. Residents are able to bring items of their own furniture and belongings if they choose. Two residents had brought their small dogs with them. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 and 30 Residents’ benefit from consistency of care from staff who know them well. Before new staff are employed, the views of residents are sought and taken seriously. EVIDENCE: The staff rota is organised so that there are two senior care staff on duty and a carer, plus the manager during the day. There is one staff member awake during the night, with one member of staff sleeping, but on call when needed. The staff group have worked in the home for some time, and the manager does not use agency staff. Recruitment files examined were found to be in order. Residents living in the home are also involved in the recruitment process for new staff. The managers are committed in seeking opportunities for staff to train to National Vocational Qualification Levels at two, three and four for senior members of staff. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 35 and 38 Residents benefit from a well run and managed home with the proprietors ensuring the safety of residents and staff by following good working practices. EVIDENCE: The owners/manager live in an apartment in the home and undertake the main management duties, with staff saying ‘it is a lovely home, I enjoy working here’. Another member of staff said, ‘the manager is supportive, kind and is always there for the staff’. Residents living in the home look after their own financial affairs. Information received by the Commission prior to the inspection showed that health and safety checks are undertaken on equipment used in the home, including fire equipment, electrical equipment, water temperature checks and gas installations checked.
