What the care home does well

There are good assessment and care planning arrangements in place, and plenty of information is available to help people make a choice of where to live. A caring staff team who know the needs and preferences of the residents, and an effective management approach ensures a high standard of care is provided at the home. There are lots of things to do at the home, and visits from relatives and friends are encouraged. Residents can make choices about what they want to do and how they want things to be done for them. There is a tasty and varied menu available.

What has improved since the last inspection?

Since the last inspection the kitchen has been completely refitted, including provision of a new water heater. Television aerials have been replaced and there is a new hoist available. Several bedrooms have been decorated and new fire doors have been fitted throughout the first floor. There are new bay windows and curtains on the ground floor. Outside the building a large tree has been removed and fencing and paths have been replaced.

There are no requirements made at this visit, and there are none outstanding from previous inspections.

The home is situated in Woodhall Spa and within walking distance of the centre of the village. The home is owned and managed by Mr and Mrs D Bright who live on the premises. Mrs Bright manages the home daily and has an assistant manager and senior staff. The home has a warm and friendly atmosphere and offers a high standard of care. The current fees for care vary between £336.00 and £429.00 per week. This key unannounced inspection took place during January 2007 and the visit to the home was carried out on one day. The care received by two residents was followed in detail. Feedback was obtained from these people and others who live at the home about their experiences of living there. Individual resident’s records and general house records were looked at; staff and the assistant manager were spoken to and observation of the care being provided was made. Feedback was also received prior to the visit from surveys completed by residents and a questionnaire completed by the registered manager. Feedback from residents and relatives was generally very positive. A resident said that they are ‘very comfortable’ living at the home; and a relative said that there is a ‘very good atmosphere’ in the home. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 2, 3 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The manager continues to assess prospective residents and they have the information they need to make a choice. EVIDENCE: Pre inspection information shows that there is a statement of purpose in place, which has been updated to include new fees and staff changes, and this was seen on the day of the visit. Surveys received prior to the visit indicate that residents have contracts for their stay, and they received enough information about the home prior to going to live there. Pre admission assessments are in place, and are carried out by the placing authority. Records show that the provider has met these standards at previous inspections. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The resident’s benefit from a staff team that knows all of their needs and their preferences, and ensures the care is of a high standard. EVIDENCE: Care plans are in place, and they are very detailed. They provide the staff with clear information about the needs of each resident. The staff described the care they give and how they pass on changes to other staff. They said that they meet daily to discuss the care they give and they frequently consult the residents to ensure they are meeting their individual needs. Records show that staff write clear notes about the care that they give. The staff were observed to be friendly, helpful and caring towards residents. One resident said she is helped to bathe and the staff always protect her dignity. Records show that care plans are regularly reviewed with the involvement of the residents and their relatives. It was observed that the registered manager Russell Green Care Home Limited DS0000060706.V323882.R01.S.doc Version 5.2 Page 10 works closely with all members of the staff team, and uses care situations as an opportunity to train staff and ensure a high standard of care. It was also observed that residents can make choices about their personal care, and one relative said that residents are encouraged to maintain their independence. The staff also said they try to encourage the residents to remain as independent as they can. Health care needs are included in the care plans and the staff could describe the involvement of doctors and district nurses. The residents said they could access healthcare and that the staff helped them with this. Staff said that they are due to undertake training in medication administration in the near future, and medicines were stored and administered appropriately. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The residents are able to choose their lifestyle and routines, and there are many opportunities for activity and social interaction. EVIDENCE: Pre inspection information show that a range of activities are provided such as keep fit, regular entertainers, musical bingo, manicures garden parties and trips out. During the visit three residents were enjoying wrapping up the Christmas decorations to put away, whilst having a glass of sherry and a chat with a member of staff. The manager and the staff consult the residents about the activities they would like and they then choose whether to participate. One relative said that the resident prefers not to join in and that is respected, although they are always asked their opinion. Another resident said that the staff have the time to care for the residents and that activities regularly take place. The staff produce an interesting newsletter and the residents can and do contribute articles. Relatives are welcome to visit and one person said there is an open and approachable atmosphere in the home, where they can speak to any of the staff. Residents said they could choose all aspects of their lifestyle. The registered manager ensures that the staff spend time with each resident gaining their views and preferences. A relative said that the staff always work for the benefit of the residents, and one member of staff described the cultural needs of a resident and how these were accommodated. This demonstrated the flexibility of the staff team and their efforts to meet each person’s holistic needs. Previous inspections have commented on the resident’s positive attitude to the food and this was confirmed during this visit. A sample menu was sent as part of the pre inspection questionnaire and this was varied and suitable. During the visit three residents said how much they liked the food, as they were eating lunch. They said that it was always good and they could choose what they wanted. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The residents feel safe in the home, and the staff have the skills to raise concerns and deal effectively with complaints. EVIDENCE: Pre inspection information shows that that no complaints have been received by the home since the last inspection. A relative said they were happy to raise issues with any of the staff, and they were confident that they would be resolved. The registered manager has distributed new safeguarding adults guidance to staff and has requested training. Most of the staff have completed or are currently undertaking a nationally recognised training course at various levels, and these courses include elements of safeguarding adults awareness. One staff member said they were confident that they would approach senior staff if they suspected abuse. The residents live in a clean, comfortable home, which the providers continue to improve and maintain. EVIDENCE: There is evidence that the provider continues to improve the home. The kitchen has recently been completely refitted, and a new water boiler provided. Television aerials have been replaced and there is a new hoist available. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The residents are cared for by a well-trained and safely recruited staff team. EVIDENCE: The staff demonstrated that they are clear about the care they give to residents. A relative said that the staff are very good and they have enough time to offer quality care in a friendly, almost family atmosphere. Records show the lists of staff that have completed or were doing nationally recognised qualifications, and one member of staff said that they had enjoyed doing their course. Records also show that staff have received training in areas such as moving and handling, fire safety, infection control and basic food hygiene. The registered manager also does in house training in practical care issues. No staff files were seen during this visit, however previous recruitment checks have shown that the provider ensures that the standards are met. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The manager ensures that the home is run effectively and safely for the benefit of the residents. The residents are given plenty of opportunities to contribute to the running of the home. EVIDENCE: The providers continue their daily involvement with the home and monitoring of the standards, and they have contact with every staff shift. The assistant manager is to apply for registration with the commission in the near future, with the ongoing support of the providers. A relative said that the staff and the registered manager have an approachable and open attitude. The residents said they can always speak to any of the staff and the staff were observed asking the residents opinions. The residents have given positive written statements about the home and these are included in the statement of purpose. One member of staff produces a monthly newsletter called ‘The Gazette’ and the residents and staff are asked if they would like to contribute. Each resident has a key worker who organises a three monthly review, this meeting includes the resident and they may also invite family or friends. One relative said they could talk about all aspects of the residents care and they had noticed that changes had been made after the meeting. The provider conducts a monthly inspection of the home and this covers all safety checks and maintenance, as well as other elements of the care and the environment. This ensures that all checks are completed. New fire doors have been fitted upstairs and the fire officer has booked a date to visit the home. The staff are trained in health and safety and lifting techniques. Pre inspection information shows that nine residents have power of attorney and all the residents or their relatives manage their own finances.
