Conditions of registration: Date of last inspection 14th July 2005 Brief Description of the Service: Russell Green is a home for up to 18 residents who require personal acre and are over 65 years of age. The home is situated in the village of Woodhall Spa and within walking distance of the centre. The home is owned and managed by Mr and Mrs D Bright who live on the premises. Mrs Bright manages the home daily and has senior staff who deputies. The home has a warm and friendly atmosphere and offers a high standard of care. This is an overview of what the inspector found during the inspection. The inspection took place over 4 hours and involved talking with the providers, the assistant manager and 9 of the residents. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4,5 The homes assessment and admission procedures ensure that residents are assured that the home can meet their needs and they have the information they need to make an informed choice. EVIDENCE: The manager talked about the process of assessing new residents and how they would fit in with the existing residents. New residents receive the home’s brochures and the manager assesses them to determine if the home can meet their needs. The assessment is detailed and it continues to be reviewed up to 6 weeks after a resident has moved in, and then at regular intervals. The home provides a statement of purpose and a service users guide along with a contract and fire regulations. The providers write to each prospective resident assuring them that the home can meet their needs before they move in for a trial period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10,11 The care plans meet and exceed the standards required and the home is well able to meet each resident’s health and social needs. Each person is treated with respect and dignity and genuine care. EVIDENCE: 3 care plans were seen and they contain all the information that staff need to care for each residents. They include information on resident’s normal preferred routines, their likes and dislikes and their hobbies or interests. The manager stated that the staff have the daily opportunity to review the care plans and discuss the care they offer. The staff also carry out 3 monthly reviews with the residents and their invited family or representatives. The minutes were seen and they offer a good chance for everyone involved to keep up to date and share information. The manager discussed the resident’s health care needs and how these are met and this was confirmed by the care plans and by the residents. One resident was attending a doctor’s appointment. All the residents had received flu inoculations at the home except one person who chose not to. District nurses provide regular health care for one resident and the staff carry this out. The manager also said the district nurses were very good and that they would

One resident prefers to sit in their wheelchair during the day and their pressure area care was discussed with the manager. The staff are able to help this person move during the day and to observe the health of their skin. They are well nourished and have plenty to drink to prevent dehydration. The manager discussed the care and arrangements that had been carried out at the end of a resident’s life and this demonstrated a thoughtful and caring approach. Each resident is asked about his or her wishes and these are recorded along with their religion (if any) in their care plans. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15 The home provides activities that suit the residents and they were all positive about their lifestyle at the home and the contact they have with their families and friends. EVIDENCE: The residents talked about the activities in the home and the manager showed photographs of recent events. Each resident expressed their opinions and said whether they joined in or choose not to. The staff maintain a list of activities and of participants. Residents also have the opportunity to go to the local village or on trips out. The home now has a wheelchair accessible vehicle, which has proved useful. The residents talked about visits from their family and friends and the manager spoke about keeping people informed. The home produces a newsletter and family are welcome to events. The staff and residents are looking forward to the staffs annual pantomime which they said is ‘always good fun and gives them a good laugh’. They also spoke about a singing duo that visits and sings hymns and carols. The daily menu is displayed and some of the residents knew what was being served. They said the food was lovely and they had plenty to eat and drink. Tea is served early at about 4.45p.m and two residents said this suited them well. One resident said they liked to have something later and so this has

Some of the residents said they also have supplies of food in their rooms that they can have at any time. The residents also said that if they didn’t like some food on the menu they could ask for an alternative although the staff also know peoples individual preferences. The policies include a diet and nutrition policy and this states how specialist diets must be catered for, for both health and religious preferences. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,17,18 The home does protect and promote the rights of each resident. EVIDENCE: On admission each resident is told about the advocacy service that they can access and the information is also displayed in the hall. All the staff have signed the complaints and comments procedure and there have not been any complaints since the last inspection. Staff are informed about ‘ protection form abuse’ during their induction and this is also part of their N.V.Q training. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,22,26 This home offers a safe and comfortable environment. EVIDENCE: The home is well decorated and furnished. It is well maintained and the gardens are an asset. Each resident’s mobility needs are assessed and it was observed that they have the equipment they need to maintain their independence. The providers carry out regular maintenance and repairs. The residents said how much they like the home and that it ‘is always warm and well lit’. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,36,37,38 The home is run in the best interests of the residents by a very competent manager who is dedicated to providing a quality service and prides themselves on maintaining that quality. EVIDENCE: The manager is qualified through their experience to run the home. They stated that they are training senior staff to eventually manage the home but they will maintain daily contact and support. The two senior staff are near to completing their N.V.Q 4 qualifications and they will then undertake their managers awards. The manager stated that they think carefully about their responsibilities and this is conformed by the quality of the organisation and the care of the residents. The residents said they ‘like all the staff and that the manager is very kind and they couldn’t wish for any better care’.

The homes policies were seen and they contain safety information. Safety notices for both the residents and the staff are displayed on the notice board in the hall. No accidents involving the residents have occurred since the last inspection. The manager holds regular seniors meetings and staff meetings and the staff are key workers to different residents. The staff are supervised and supported to carry out their roles and they are happy to approach the manager to discuss any issues or to seek advice.
