This inspection was carried out on 2nd March 2006.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

This is a warm and friendly home that cares for the residents with a management team who have the experience and training to ensure that the needs of those people living there are identified and where possible met. The building has a homely atmosphere but it also equipped with the facilities necessary to provide the appropriate care such as a fully equipped and secure medication room.

What has improved since the last inspection?

Since the last inspection the home has found more appropriate ways in which to hold the doors that need to be kept open. Further steps have been taken to ensure the safe use of bedrails should they be needed again.

What the care home could do better:

The comfort of the service users will be further enhanced when the heating systems in some of the rooms has been modified to make them more controllable by the occupants.

The home has been converted from a large country house and there are double and single rooms some of which have en-suite toilet facilities. The accommodation is on two floors, the upper being accessed by stairs or via a shaft lift. On the ground floor are pleasant lounges and dining areas, which are well decorated and comfortably furnished, the well-maintained grounds are also available for residents use. Care is planned in consultation with individual residents and is reviewed regularly. The home also offers a variety of in-house activities, which are optional, outings and special events are arranged throughout the year. This inspection was unannounced and took place over three hours during the late morning and early afternoon. Information was gained from documentation made available by the home, by discussions with the proprietor, the staff and some service users as well as by looking around the building. Inspection work undertaken by CSCI is proportionate in relation to how a home has performed in the past. As this home has a history of providing a good service for people this inspection was brief and focused only on a small number of “key” areas of work. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8, 9 and 10 The health care needs of the service users are met in a manner that maintains their privacy and dignity. EVIDENCE: This section should be read in conjunction with the report of the last visit made on 16th August 2005. The proprietor reported that the manager of the home has now been trained in risk assessing the need for and use of bedrails which further enhances the safety and wellbeing of those service users who may need to use them. At the time of this visit there was no need for them to be used. The home has a secure medication room that is fully equipped with secure storage for the medication held and administered by the home. The refrigerator used to store the medications that must be kept cool was seen to be equipped with an appropriate type of thermometer and a regular record was seen to have been kept of the temperature inside it. A range of other records Sambrook Care Limited DS0000063772.V275933.R01.S.doc Version 5.1 Page 9 were seen relating to the issue and disposal of medication and these were seen to be accurately maintained. At the time of the visit there were no Controlled Drugs stored at the home but appropriate recording and storage facilities were seen to be available should the need arise. The senior carer on duty at the time of the inspection was able to quickly make available a range of policies and procedures relating to the maintenance of the privacy and dignity of the service users demonstrating her familiarity with them and throughout the inspection she and the other care staff were observed treating the service users in a polite, friendly and dignified manner. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 Service users are able to maintain links with their families and friends. EVIDENCE: During this inspection a local minister visited the home and the staff confirmed that this is a regular occurrence. Talking to residents and looking at the visitors book showed that the service users relatives can and do visit regularly. JUDGEMENT – we looked at outcomes for the following standard(s): 25 The attractive, clean and homely environment of this home will be further enhance once the planned work on the heating system is carried out. EVIDENCE: Only one issue remains outstanding from the review of the standards at the last inspection, that being the ability to control the temperature of some of the room independently. Documents showing how this situation is to be rectified were seen but this work will, understandably, not take place until the warmer weather. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 and 38 The home is managed in a safe manner by a qualified management team. EVIDENCE: The proprietor confirmed that the manager of the home has achieved the qualifications that are required by the National Minimum Standards for people employed in her position. He and the senior carer both confirmed that he spends a good deal of his time in the home and, in fact, lives in an adjacent house. The senior carer also confirmed that she is about to start working towards her NVQ4 in care which will enable her to be more involved in the management of the care of those who live in the home. On this visit it was not happening and a notice was seen instructing the staff that it should not happen. Approved devices had been fitted to some of the doors where this had caused distress for the service user involved.
