Conditions of registration: Date of last inspection 16/09/04 Brief Description of the Service: Sandgate Manor is part of the MNP Complete Care Group. It is a residential home providing care and support for up to nineteen people with a range of learning and physical disabilities. It is a large and well-maintained manor house that has been adapted to be wheelchair accessible. It is situated in Sandgate, about half a mile from the local shops and seafront. The house has spacious accommodation and large grounds. The residents all lead busy lives and many attend colleges and adult education centres in nearby Folkestone The registered manager is Mrs Lesley Rudd. a very experienced Home Manager, who has been at Sandgate Manor for a year. This is an overview of what the inspector found during the inspection. This inspection began at 9.15am and took five hours. Mrs Lesley Rudd, the registered manager assisted the inspector throughout and she is thanked for her help. During the course of the inspection the inspector spoke to seven residents and four members of staff. Detailed discussions were held in private with one resident and three members of staff. A pre-inspection questionnaire was sent out to the residents and their relatives. The responses were positive and demonstrated a high level of satisfaction amongst the respondents. A tour of the home was undertaken, key documentation inspected and sample care plans examined in detail. Both direct and indirect observations were made throughout the inspection. The Home continues to maintain a high quality of care that meets the National Minimum Standards. The residents who spoke with inspector said that they are well cared and like living in the home. They are able to put their views forward and participate in the running of the home if they wish to and are able to do so. The accommodation and grounds are spacious and allow for easy access and movement for all wheelchair users. The Home is well maintained and well decorated. Staffing levels are good and staff told the inspector that they really enjoy their work with the residents. Staff training is linked to the needs of the residents and there is a good level of training and development taking place. The Home has excellent quality monitoring systems and a well-structured, long-term improvement plan. What the service does well:
The Home provides an accessible, comfortable and well-decorated environment in which to maximise the residents are able to maximise their independence. Many residents have complex disabilities and require very specialised equipment. The Home is proactive in its approach to accessing the necessary specialist equipment, such as individually adapted wheelchairs and communication equipment, and staff training, in order to minimise these problems. Sandgate Manor provides very spacious and wheelchair accessible living accommodation that is also very tastefully decorated and homely. Outside there are pleasant, well-maintained gardens that are accessible to all the residents. The Home accesses a wide range of activities on behalf of the residents. These activities are tailored to meet individual needs. In addition, it offers good support to relatives and friends who visit the Home. The internal long-term planning and quality assurance systems are excellent.

Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,5 The statement of purpose and the service user guide are comprehensive and good. The residents are aware of their roles and responsibilities and know that their independence will be promoted. They know that their goals and aspirations will be supported by the Home. EVIDENCE: The statement of purpose and the service user guide are well written and enable residents to understand what to expect from the Home. No new residents have joined the Home since the last inspection. Residents said that the Home helps them do the things they want to and helps them live interesting and enjoyable lives. Indirect observation confirmed that staff encourage the residents to be as independent as possible. Individual needs and goals are reflected in the care plans and risk assessments are in place. JUDGEMENT – we looked at outcomes for standard(s) 6,7,8,9&10 The residents know that the Home will respect their confidentiality and value their views. There is a clear and consistent care planning process that the residents and their supporters understand and records are maintained securely. EVIDENCE: There are regular house meetings and residents told the inspector that they can discuss ideas for improving the Home and any other general concerns at these meetings. They also said that they can talk, on a day-to-day basis, to the registered manager or staff about any concerns they may have. They said they feel assured that confidentiality will be maintained. There is a key worker system and staff ensure that they support the needs and decision making of those residents who are unable to communicate those needs for themselves. On the day of inspection some of the residents were participating in household tasks. Although some need a great deal of encouragement and assistance to carry out their duties, they were observed to be given appropriate support by the staff. Relatives said that they were happy with the care at the Home and that the Home fosters good communication. Inspection of care plans confirmed that they are in order, up-to-date, and clearly identify the needs of the residents. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,14,15,16&17, The home offers excellent opportunities for personal development and is proactive in the way it assists and encourages the residents to lead fulfilling lives. The quality of meals is very good and special diets are catered for when indicated. EVIDENCE: The residents and their supporters spoke enthusiastically about the various leisure and educational activities in which they are involved. Since that last inspection one resident has achieved success in GCSE examinations whilst others have further developed their interests in activities such as music and horse racing. Inspection of care plans and discussion with staff suggests that the residents are all involved in appropriate leisure and cultural activities. Since the last inspection both lounge-dining rooms have been tastefully redecorated. The large dining table in one room has been replaced by two smaller tables. This makes the room appear much more homely. Both rooms

The re-decoration and re-arrangement of furniture now gives a more comfortable atmosphere in which meals can be taken. Inspection of the kitchen and conversation with the chef confirmed that there is a plentiful supply of good quality produce. There are regular deliveries of fresh fruit, vegetables and meat. The Home offers a choice of meals to the residents. Residents said that they mostly enjoy their meals. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19&20 Personal support is offered in a way that protects the privacy and dignity of the residents and promotes their independence. There are clear and comprehensive systems for the management and administration of medicines. EVIDENCE: The residents said that the staff respect their confidentiality and that they are happy to discuss personal issues with the staff. Indirect observation confirmed that personal care, when needed, was offered in a discreet and sensitive manner. Inspection of the systems for ordering, recording and administration of medication, and discussion with a senior team leader, confirmed that improvements have been made. There is now an improved system for ordering and monitoring the supply of medicines for the Home. Two members of staff now ensure that the medicines are ordered and recorded in a timely way. The storage of medicines has also been made more secure. There have been no recorded medication errors since the last inspection. JUDGEMENT – we looked at outcomes for standard(s) 22&23 Staff have good knowledge and understanding of Adult Protection issues and how to protect the residents from allb forms of abuse. The residents and their supporters know that their concerns will be listened to and acted upon. EVIDENCE: The residents said that they can talk to the staff if they are worried. They know that staff will only share important information with the appropriate people. Mrs Rudd said that day-to-day comments and concerns are dealt with as they arise. There have been no formal complaints since the last inspection. Induction training is now more structures and a new member of staff confirmed that she had undergone initial adult protection training. She was knowledgeable about the action she should take if she were to witness any form of abuse. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25,26,27,28&29 The standard of the environment within the Home is very good and provides the residents with an attractive and homely place to live. The layout of the Home promotes the independence of the residents. EVIDENCE: A tour of the Home was undertaken. An internal re-decoration and refurbishment programme has recently been undertaken. This is nearly complete and the kitchen was due to be replaced in the week following the inspection. The internal décor is tasteful and fresh. Residents and relatives said that they have been involved in the choice of colour schemes for their rooms. Inspection of individual rooms showed that they reflect the different personalities and interests of the residents. The communual areas are spacious enough to allow for easy wheelchair access and movement. The Home has made these areas appear light and spacious whilst maintaining a comfortable and homely feel. There are sufficient toilet and bathing facilities, several of which are adapted for use by disabled people. There is a spacious and luxurious lift that residents find easy to use. Where indicated, ramps have been fitted. Specialist

Carpeting in the Home is of good quality and new carpets have been fitted to some areas. These give the Home a bright and comfortable feel. All areas of the Home were very clean and free from all offensive odours on the day of inspection. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,35&36 There is an enthusiastic workforce that positively promotes improvement in the quality of life of the residents. Staffing levels are good. The arrangements for staff induction are very good and staff demonstrate clear understanding of their roles and responsibilities. There is a good training and development programme. EVIDENCE: Staff to whom the inspector spoke were clear about their roles and responsibilities. They spoke enthusiastically about their work in the Home and about the way in which they support the residents to achieve their goals. They said that they receive both statutory and specialist training and that there are clear reporting mechanisms and good team working. Staff said that they had noticed improvements in the Home since the last inspection and that their ideas for those improvements had been taken into consideration. They were pleased that the workforce is now more stable and that there has been no need to use agency staff. Inspection of staff files and discussion with Mrs Rudd, confirmed that all appropriate checks and made on staff before they are offered work at the Home. There is a staff training and development programme that is linked to the specialist needs of the residents, however, one-to-one staff supervision has not yet been fully established. It is recommended that the Home give this priority over the coming months. The Home should ensure that training and development programmes are linked to staff needs identified at one-to-one supervision sessions. JUDGEMENT – we looked at outcomes for standard(s) 37,38,39,40,41&42 The Home is well managed and provides a caring and supportive service that promotes the independence, health, safety and welfare of the residents. There are excellent quality assurance systems. EVIDENCE: Mrs Rudd, the registered manager, has many years experience in care and in managing care homes. She has completed all the necessary work for the NVQIV in Management and Care earlier on this year, however, there is a delay in the assessment process. The local college has admitted full responsibility for this delay. Staff said that they have great respect for Mrs Rudd and that she both supports them and promotes their training and development. They said that they believe that she listens to their ideas and gives them credit for their achievements.

The Providers visit the Home frequently. The Home is well maintained and there is a long-term development programme. There is clear evidence of improvements to the fabric of the building, both inside and out. The CSCI has received regular, comprehensive quality reports in accordance with Regulation 26.
