Conditions of registration: Date of last inspection 18th May 2005 Brief Description of the Service: Saxby Lodge is a privately run establishment providing board, accommodation and personal care for up to nineteen people over the age of 65. It is a large detached property set in its own grounds close to local shops and railway station and about two miles from the seafront. Accommodation is provided in single rooms, four of which have an ensuite toilet. The home does not have a vertical lift so only residents that can manage the stairs can be accommodated in any of the four rooms on the first floor. Facilities include a lounge/diner and a conservatory on the ground floor and a lounge on the first floor. There is a small garden for the residents to enjoy. This is an overview of what the inspector found during the inspection. The inspection was conducted between the hours of 9-30am and 3-45pm. Saxby Lodge has performed well over the past years and at the last inspection no requirements were made. During the course of this inspection the inspector spoke to residents and staff. The owner/manager was present throughout the inspection. Records and Care Plans were looked at and a tour of the home was conducted. The inspector had received a large amount of comment cards from residents, relatives and professionals to the home. These comments will be included in this report. One resident told the inspector, “I have always found the staff very nice, and always very helpful”. Another said, “ The staff do everything they can to provide a homely atmosphere.” These sentiments were expressed by a number of residents. A relative wrote, “Having been in many homes visiting I feel that I am very lucky to have my mother in Saxby Lodge. Everything about it is so good and I am delighted with the service they give to my mum and me. It is one of the best!” All records inspected were found to be in order and up to date. At the last inspection, the inspector felt that some of the records kept for staff could be better. There were some omissions in CVs and Application Forms were, in two cases, not fully completed. These records have now been completed and were up to date. The Care Plans were well written and contained all of the information needed to look after the residents. They were reviewed regularly. Staff training was ongoing and it was noted that the majority of staff were either National Vocational Qualification (NVQ) trained or were awaiting to start the training. The training records of all staff were well documented and up to date. Staff were being supervised by the owner/manager and detailed notes had been kept of these sessions. The building was warm, clean and welcoming. There was three care staff on duty with one domestic, a cook and an administrator. At night there were two staff on duty, one of whom was awake. The rotas showed this to be the case. The inspector discussed with the owner/manager whether both staff should be awake on duty. The home met the requirements of the local fire and environmental health services. There were no requirements made at the time of this inspection. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3, 4, 5, 6 Residents and their families have the information they need to make an informed choice about where they live and are suitably assessed before placing. EVIDENCE: Each resident is given a contract when he/she enters the home. Most of the residents had family or friends who were able to act upon their behalf. The owner/manager assesses all residents before they join the home either in their own home or in hospital. These completed assessments are kept in the Care Plan. The owner/manager told the inspector of a recent admission to the home. The prospective resident moved from a hospital outside of the area into the home to be nearer to relatives. Due to the distance involved the assessment was carried out over the telephone talking to the hospital, the social worker and the family. The resident was fully assessed when arriving and the home feels it is meeting her needs. The home does not take anyone in for intermediate care.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10, 11 The resident’s health, personal and social care needs are well met through training and supervision of the staff. EVIDENCE: Care Plans were well written and contained all the information necessary to look after the residents. They were updated regularly and reviewed at least once a month. The inspector looked at the Care plans of some of the residents he had spoken with and felt that the care given was accurately recorded. District nurses visit the home when requested and records are kept of all such visits. A district nurse commented, “ I can always rely on the manager and her staff to liaise with the district nurse team if they have any concerns and carry out relevant instructions.” Medication was stored safely and records were well kept in regard to the administering and disposing of them. Staff were well trained and some had attended an outside course on ‘Care and Control of Medicines’. When looking around the home the inspector noticed staff knocking before entering rooms. Residents were called by their chosen name; others preferred to be addressed as Mrs.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15 Residents say that they are happy with their lifestyle, are able to keep in contact with their family and friends and able to have control over their daily tasks. Good home cooked food is provided. EVIDENCE: The home does provide some activity for the residents if they wish to join in. There is a fortnightly ‘Movement to Music’ session, a weekly singsong, monthly visits from outside entertainers, weekly visits from a local catholic church and a quarterly visit by a Church of England vicar. The majority of residents are able to go out with their families or friends and one lady told the inspector that she goes to stay with her family every fortnight. One lady regularly goes out to the cinema or theatre with her family, another loves her trips to a garden centre. One lady had written that she wanted “a visit from someone with a dog”. On the day of the inspection the inspector noticed that someone from ‘Pat-a-dog’ had arrived and the lady was seen playing with the dog. The owner/manager knew of this lady’s request and had organized it for regular visits. There had been a recent outing when nine residents went out with carers for a trip in a minibus. Visitors are made welcome and one wrote, “I feel so welcome when I arrive, every one has a friendly word and look so happy in their work.”

There was also a choice of salad. One resident needed help with feeding so a member of staff sat with them whilst they ate. This was done in an unhurried manner. JUDGEMENT – we looked at outcomes for standard(s) 16, 17, 18 Any complaint is taken seriously and staff and residents know how to complain. Staff are well aware of the Adult Protection Procedures and a Whistle Blowing Policy is in evidence. EVIDENCE: All staff have attended the Adult Protection Training and the procedures are in the office. There have been no complaints made about the home to the CSCI. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 23, 24, 25, 26 The home is well maintained, safe and clean with no nasty smells. EVIDENCE: The home is well kept and is clean and tidy. The inspector observed the cleaner doing her rounds and every room was left clean and tidy. The home has undergone considerable refurbishment since the present owners took over. This is still on going and rooms are redecorated when they become available. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29, 30 There is usually sufficient staff employed to ensure that residents’ basic needs are met. The home has a sound recruitment policy and all checks are made. There has been considerable input into staff training. EVIDENCE: All staff have undergone Criminal Records Bureau (CRB) checks and copies of these were kept in the office. Prospective staff are invited to complete an application form and attend an interview. Staff training had been undertaken in all areas and the evidence showed that all staff were offered training relevant to their work. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 34, 35, 36, 37, 38 It is apparent that the needs of the residents are paramount and that things happen for their benefit. The home has no dealings with any personal finances preferring to leave that to families or other professionals. The home is managed by a well qualified and competent manager. EVIDENCE: The home was constantly updating their practices to ensure that residents received the best care that they could. The home does not have any dealings with any money belonging to residents. Fire and Environmental Health Requirements had all been met and all equipment is regularly serviced.
