This is an overview of what the inspector found during the inspection. Severn Oaks meets a range of needs for service users with learning disability. These needs are often complex and sensitive involving multiple agencies and multiple issues. The inspector concludes the service meets these complex needs well. However, the inspector observes that as an organisation (Cintre), needs to clarify the range of needs which can be met at Severn Oaks. The home offers a good service. However, the inspector notes that on at least one occasion the service has allowed its practice to be dictated by external agencies. Whilst the inspector understands that pressures may be placed on the service by outside agencies, it is neither in the service users interests, nor the service providers to allow care to be dictated by funders. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,and 5 Severn Oaks offers a structured and comprehensive range of activities to its service users, this is based on an in depth assessment of their needs. The needs of some service users may be quite complex and require a sensitive balance between support and encouraging independence. Severn Oaks meets these needs appropriately and responsibly. EVIDENCE: The inspector sampled 2 care files offering a range of particularly sensitive and complex issues. The inspector found evidence from both staff and documentation outlining the process of boundary setting and negotiation. The inspector noted the care outlined within care files. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 8, and 9 Service users needs were clearly and comprehensively assessed, part of this assessment involves service users active participation in both assessing how their needs have been met and how they need to be met in the future. Service users are actively involved in all aspects of their care, particularly risk assessment and risk management. EVIDENCE: The inspector reviewed documentary evidence for 2 service users. This process involved review of care plan, care assessments, risk assessments and daily notes. All were clear, comprehensive and up to date. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 15 and 16 The inspector noted clear documentary and statement evidence to conclude the home meets service users needs for age, peer and culturally appropriate activities. The inspector notes and commends the considerable efforts by the team to meet the multi cultural and social needs of one particular service user within a sensitive and difficult situation. The inspector recognises the effort and degree of success achieved by the team. Service users are consulted and involved in all aspects of their care. The home has to address issues of risk management and enabling service users rights. The inspector notes the home has addressed this issue successfully and supports the home in its therapeutic approach. EVIDENCE: The inspector reviewed correspondence from external professionals, social services departments and internal documentation. The inspector noted clear evidence of an orchestrated approach to balance service user rights with sound risk assessment and risk management.

Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, Service users receive appropriate personal support in accordance with their wishes. EVIDENCE: The inspector sampled care plans, policies and procedures within the home. Staff statement evidence outlined the staff team approach toward meeting personal needs identified the assessment process. The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22, 23 The inspector noted evidence that service user views are sought over issues relating to their care or well being. The home protects service users from abuse and actively encourages open discussion with service users over what constitutes abuse and their part in detecting or becoming aware of it. EVIDENCE: The inspector noted service user involvement in all processes of their care. In particular the inspector notes negotiated agreements between staff and service users over socialising outside of the home, the consumption of alcohol and general behaviour off the premises. The inspector was shown documentary evidence of staff training relating to the identification of types of abuse and action to be taken. The home also had a range of appropriate policies and procedures relating to appropriate action in the event of suspected abuse. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 30 Severn Oaks is well presented, clean and pleasant. EVIDENCE: The inspector did not audit these standards on this occasion. However on inspection the communal areas and kitchen were clean, free from offensive odours and well presented. JUDGEMENT – we looked at outcomes for standard(s) 32, 34 and 36 The inspector notes evidence that the staff team are aware of their roles, appropriately supported and supervised. EVIDENCE: The inspector noted good recruitment policies and procedures were in place, a evidence of references, C.R.B and identity checks were present. The inspector noted staff statement evidence from 2 sources and documentary evidence that staff are able to support service users. Documentary of supervision of staff was present. JUDGEMENT – we looked at outcomes for standard(s) 38 and 40 The inspector noted a particularly strong culture of open problem solving between staff and service users. The staff team address sensitive and complex issues well. The inspector commends the team. EVIDENCE: The manager has developed a clear sense of direction within the team, the inspector noted this throughout documentation, staff statement and policies and procedures.
