Conditions of registration: Date of last inspection 8th August 2005 Brief Description of the Service: The Shrubbery Nursing Home occupies a prominent position in Shrubbery Avenue in central Worcester. The Home can be easily accessible from the city centre via Tything and Barbourne Road. The home has three floors serviced by two lifts and provides single and double accommodation. The Home accommodates 36 residents. The Home is registered to provide twenty-four hours nursing and personal care. The registration certificate does not reflect the homes true status; the home can accommodate a maximum of 36 service users and not 41 as stated on the certificate. The registered manager Mrs Sandra Mayne is a first level nurse and oversees the day-to-day running of the home. Shrubbery Nursing Home, The DS0000004142.V258407.R01.S.doc Version 5.0 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. The inspection took place during the late morning and early afternoon, this inspection focused on standards not inspected at the last inspection and requirements from the last report. A number of residents and staff were spoken to during the course of the inspection and time was spent with the registered manager and the administrator, both have worked in the home for a number of years and know the residents well What the service does well: What has improved since the last inspection? A number of environmental issues which posed a risk to the health and safety of staff and residents were identified during a tour of the premises, these were discussed with the registered manager and have become requirements of this report. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 Medication policies and procedures are robust and medication is handled and administered safely to residents. EVIDENCE: Medication policies and procedures have been reviewed and updated to reflect current practice. The home did not have in place a disposal of medication contract with a waste supplier (under new legislation nursing homes must dispose of medication using a waste supplier, as chemists no longer take medication returns from nursing homes). There were no residents currently self-medicating due to their frailty. Records of medication were inspected and there were no signature gaps medication charts, which were hand written, were signed and dated by the registered nurse. The registered manager who is a first level nurse undertook audits of medication periodically. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Residents are enabled within their disabilities to live as full a life as possible and retain their own autonomy. EVIDENCE: Residents made positive choices about their lives with regard to getting up and going to bed, mealtimes and where they choose to take their meals. Bedrooms were personalised and care plans contained information about furnishings belonging to residents. The home did not have any facilities to hold monies on resident’s behalf. The home has responded to the complaint using their complaints procedure and has co-operated with the Commission throughout the investigation. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): The home is warm, cosy and homely; there are some areas of the home, which risk the health, and safety of residents. EVIDENCE: Staff have fire training regularly and have written assessments three monthly on the use of fire fighting equipment. A number of bedrooms have been fitted with door guards (the fire door closes automatically if the fire alarm is sounded) however the laundry room door was wedged open, unattended and the roller iron was left on. One fire door did not close on its rebate; this was dealt with before the inspector left the home. One of the passenger lifts in the home was not working on the day of the inspection; the home was waiting for parts and said the lift should be operational within six weeks. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 33 & 35 There was no quality assurance audit programme in place; the home did not have any facilities for the safekeeping of resident’s monies. EVIDENCE: The new owners of the home are working towards developing a quality assurance programme with the registered manager that complies with current legislation and requirements. The home does not have any facilities for safekeeping of resident’s monies, extras are charged either to the resident or their next of kin by invoice. The registered manager and her staff are aware of their responsibilities to safeguard residents from financial abuse.
