Conditions of registration: Date of last inspection 14 December 2004 Brief Description of the Service: The Shrubbery Nursing Home occupies a prominent position in Shrubbery Avenue in central Worcester. The Home can be easily accessible from the city centre via Tything and Barbourne Road.The home has three floors serviced by two lifts and provides single and double accommodation. The Home accommodates 36 services users.The Home is registered to provide twentyfour hours nursing and personal care.The registration certificate does not reflect the home’s true status; the home can accommodate a maximum of 36 service users and not 41 as stated on the certificate. This is an overview of what the inspector found during the inspection. The inspection took place during the lunchtime and afternoon period. Care plan documentation, some policies and procedures, recruitment and contract records were examined. The inspector spoke with five residents who were enjoying a session of Bingo with the activities co-ordinator, time was spent talking with the activities coordinator the registered manager and administrator. What the service does well: What has improved since the last inspection?
The home had addressed some of the requirements from the last report and had an action plan for those that had not been fully addressed. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 All residents were assessed prior to admission; the documentation used ensured the home was able to meet the resident’s needs when they were admitted. EVIDENCE: Three care plans were inspected, the assessment of need document was seen and covered all aspects of daily living. Information gained at assessment ensured new resident’s needs could be met and equipment necessary to nurse residents was in place, on admission. All assessments were carried out in the residents own environment whenever possible by a registered nurse from the home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8 &10 Residents are well cared for by experienced staff and have their individual health care needs met. EVIDENCE: Residents spoken to during the inspection looked and said they were well cared for by staff. Staff were overheard talking to residents in a kindly and friendly manner. All residents had a care plan in place, documentation continued to improve further development of these documents was discussed with the registered manager. Risk assessments were in place for all residents and there was evidence of resident and family involvement in the drawing up and continual reviews of the care plans. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13 &15 Residents were offered a stimulating programme of activities, together with a well balanced, nutritious and varied diet. EVIDENCE: Time was spent talking to the activities co-ordinator who works three afternoons a week, records of activities evidenced activities are offered to all residents in either groups or individually. Residents were enjoying a game of bingo on the afternoon of the inspection. A recent strawberry and cream tea had taken place and the home was planning an Autumn Bazaar. A number of residents require assistance at mealtimes and this was given in a dignified manner. Menus are on a four weekly rolling programme and offered residents choice at all mealtimes; menus are varied and offer a well balanced diet. Residents said they enjoyed the food on offer. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 &18 The policies and procedures for complaints and protection of the vulnerable adult were robust. Although staff had not received formal training in this field they were aware of their responsibilities in protection of residents. EVIDENCE: There has been one complaint to the Commission for Social Care Inspection since the last inspection. The compliant was investigated by the home using their complaints procedure and was found not to be upheld. The complaints procedure was available to residents and had recently been reviewed and updated. Policies and procedures regarding protection of the vulnerable adult were in place, further information and contact telephone numbers needed to be added to the document. Staff had received no formal training in this field and a requirement has been made in this report for this to be addressed. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29 &30 There are enough staff on duty at all times to meet the needs of the residents and recruitment procedures are robust. EVIDENCE: The home had a committed and experienced staff group who are able to meet the complex needs of residents in the nursing home. There were catering, laundry and domestic staff on duty seven days a week, which meant care staff did not have to undertake these duties at weekends. The registered manager has just completed her NVQ in management and felt the course had been useful. A number of care staff had commenced and/or competed an NVQ in care. The home had a full induction programme but the foundation programme in place did not meet the required standard. Trained staff are given the opportunity to attend courses. Four staff files were inspected and evidenced robust recruitment procedures are in place. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38 The home is managed by the registered manger who has the experience and knowledge to ensure the needs of residents are met. EVIDENCE: The registered manager Mrs Mayne is a first level nurse with many years experience of caring for the frail elderly in a nursing home setting. She with the administrator acts as the health and safety officer. Trained staff are not all first aid trained but have signed a declaration to say they are competent to deal with emergencies, which may arise in the home. Records of service contracts were in order. Staff had received fire training; further evidence was required to ensure all staff received fire training every three months. All policies and procedures were currently being reviewed and updated.
