Conditions of registration: A maximum of 22 persons in the category (DE(E) Dementia to be accomodated. Date of last inspection 29 11 04 Brief Description of the Service: Silver Court is a care home offering personal care and accommodation to 42 older people, 22 of whom are in the category DE(E). It is owned by Anchor Trust (Organisation) which runs nearly 40 care homes in the southern region. It is situated in a quiet residential area in the outskirts of East Grinstead relatively close to some shops and a post office. It is near to Ashdown Forest. It is a purpose built single storey building and was first registered in 1994. It is laid out in four blocks three consisting of ten flats with communal sitting and one with 12 flats all with dining and kitchen areas. All residents rooms have en suite facilities. This is an overview of what the inspector found during the inspection. This unannounced inspection was carried out over the morning and afternoon of June 1st 2005. The manager was in attendance. The intention of this inspection was to look at requirements of the last inspection and assess if there has been an improvement in the daily lives of the residents. Residents spoken to were satisfied with the care offered but some commented that their opinions are not always considered. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 Residents have a contract and terms and conditions of living in the home. EVIDENCE: Residents can make a fully informed choice about the suitability of the home. Each residents room had a pack of information highlighting the terms and conditions in the home. Relatives spoken to stated that they were happy with the information provided to them. Residents have a choice of room as far as possible, in the recent addition of two rooms and a change in use of unit residents were consulted and their wishes met. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12-15 Most residents have a lifestyle that matches their expectations; they can maintain contact with family and friends. Residents can exercise choice and control. A balanced and satisfying menu is served. EVIDENCE: Many residents spoken to say they are happy with the social activities of the home although one resident was not having outings and shopping experiences. One resident stated that she frequently goes out to clubs and lunches, taken by her family. Another resident stated that their opinions are not listened to. One less mobile resident said that they are rarely assisted to the garden and that it is not accessible as it is overgrown. There is not an activities assistant at present. The meals seen prepared and served were appetising and residents were helped appropriately. One resident said that there is a long wait for breakfast. This was discussed with the manager who stated that she would remind the morning shift to serve breakfast to anyone waiting in the dining room. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 Not all residents are confident that their complaints will be listened to. Relatives are confident that their complaints would be listened to and acted upon. Residents are protected from abuse. EVIDENCE: Most residents spoken to felt that their complaints would be listened to but two did state that their comments are ignored. One resident cited a carpet that is stained but management have not acted on her reporting this. This was discussed with the manager who said that it would be replaced. There are Adult Protection Procedures in place to protect residents from abuse and residents state that they are treated with respect. At a recent Adult Protection conference it was confirmed that the home is reinstating two staff that had previously been suspended, investigated and dismissed for alleged abuse following further investigation by Silver Court management. The manager stated that the residents involved in this incident were not happy these staff were returning. The reinstatement is further evidence that residents opinion is not being considered. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,23, 24,25,26 Residents live in a safe and well-maintained environment. They have limited access to the garden. Resident’s room suit their needs and are personalised. The home is clean, pleasant and hygienic. EVIDENCE: The home is well maintained and well decorated. The central heating system is noisy today. Residents state that it has been keeping them awake. The manager stated that it is to be serviced but that she is also going to apply for a budget to have the valves replaced. The garden is not maintained at present as the gardener is off sick. The grass is overgrown and one resident commented that they cannot go onto the grass. There is a patio area that can be accessed. The resident’s rooms are personalised with evidence of their own belongings. Residents spoken to said the home was comfortable. The home was clean and without unpleasant odour with the exception of room 22 which had a blocked drain. The manager stated that a contractor is coming this afternoon. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28,29, 30 Resident’s needs are met by the provision of adequate numbers and skill mix of staff. Residents are supported by the homes recruitment policy and practices. EVIDENCE: Staffing hours are assessed using the residential forum. Staff rota shows which staff are on duty. There are sufficient numbers of domestic staff and the manager has advertised for additional administration support. Staff files seen show all documentation required to ensure that residents are in safe hands, except one staff file that was missing a birth certificate copy and the manager is pursuing this. A staff-training programme was seen which is evidence that the home is committed to providing competent staff to meet the needs of residents. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,36 The residents live in a home which is run by a manager who is fit to be in charge, is of good character and is able to discharge her responsibilities EVIDENCE: At the last inspection the manager was spending much time at another home, that is no longer the case and the residents benefit from her increased input. Staff, residents, and visitors spoken to stated that the manager is approachable, although as stated in standard 16 some residents did feel that not al of their comments are acted on. The environment in the home today and the majority of the comments made are evidence that the residents generally benefit from the management ethos of the home. The manager ensures that staff receive supervision and new documentation was available to ensure that the incident of staff disciplinary and reinvestigation which led to the reinstatement of staff as discussed in standard 18 does not occur again.
