The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

There is a very stable staff team that ensures continuity of care for the residents. Five residents discussed their care with the inspector and all were very happy. They said the staff were kind and helpful and they really enjoyed the meals. The home`s atmosphere was relaxed and the inspector observed carers to be gentle and caring when assisting residents. Staff interviewed said they were happy with the support they received from the staff team and that training opportunities were good. The home is comfortable, homely, attractive and well maintained and residents have ample choice of where they choose to spend their day. All visitors to the home are made to feel very welcome.

What has improved since the last inspection?

The kitchen has been redecorated and new equipment installed. New risk assessments are in place and are carried out and documented at each review. Recruitment procedures are more robust. A visitor`s book has been introduced to invite comments from families and visitors if they so wish. A new policy in relation to staff returning to work following sickness has been introduced.

What the care home could do better:

The protection of vulnerable adults training needs to be updated.

Condition 1 The overall number of registered beds is 36, however 3 of the registered beds cannot be used to accommodate service users until such time as Building Control have passed them, a further site visit has taken place and the Commission for Social Care Inspection are satisfied that they are fit for the purpose. 30th September 2005 Date of last inspection Brief Description of the Service: Sotwell Hill House is a late Victorian converted country house set in attractive and extensive grounds and provides accommodation for those older people who require day-to-day supervision and care. The home does not provide nursing care. Independence is encouraged and the layout of the grounds enables service users to take exercise in safety. Service users are able to visit the nearby town of Wallingford and other outings are arranged on an occasional basis. This was an unannounced inspection and took place on Thursday 2nd March 2006. The inspector was made to feel very welcome by both staff and residents and was appreciative of the cooperation during the inspection process. The inspector toured the building looking at the communal areas and spent time with care staff, the chef, the management team and residents. The availability of training was discussed with staff and inspection of documents took place. A majority of the key standards were assessed and met at the announced inspection in September 2005, therefore only a few standards needed to be assessed at this inspection. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11 Key standards 7,8,9 &10 were assessed and met at the previous inspection in September 2005. Residents are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. EVIDENCE: The manager confirmed that the resident’s wishes concerning terminal care and arrangements after death are discussed and carried out. Residents are able to spend their final days in their own rooms, surrounded by their personal belongings, unless there are strong medical reasons to prevent this. Residents’ families are supported throughout the process. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): All key standards were assessed and met at the previous inspection in September 2005. EVIDENCE: The inspector spoke with the chef who has worked hard and almost completed the “Healthy Choice Award”. He is now awaiting the environmental health officer to visit to further inspect his kitchen and his records in anticipation of achieving the award. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17 & 18. Key standard 16 was assessed and met at the previous inspection in September 2005. All residents have their legal rights protected and are protected from abuse. EVIDENCE: All residents have access to television, radio and newspapers. Residents have the choice of voting at a polling station or using a postal vote. The manager confirmed that she would arrange postal voting forms for those residents who request one and for those wishing to visit the polling station, staff would arrange this. The home have their own in house training on the protection of their vulnerable adults. Staff confirmed that this was in place but the inspector felt the team would benefit from advice from Health and Social Care. The training manager will be contacting the appropriate individual to arrange further training in this area of practice and to obtain the Oxfordshire Multi Agency Codes of Practice for the prevention of abuse. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28, 29 & 30. Key standard 27 was assessed and met at the previous inspection in September 2005. Staff are trained and competent to ensure that residents are well cared for. EVIDENCE: The staffing rota on the day of the inspection showed there were sufficient staff on duty to meet the needs of the current resident group. Staff interviewed confirmed that they felt they received adequate training to enable them to feel confident in their caring role. One member of staff was undergoing her NVQ 2 and was happy with the support she received from her assessor. It was also confirmed that every staff member has an individual training file and that training is considered a priority and easy to access. The inspector spoke with an in house training assessor and she confirmed that the home gave specific NVQ time, which she found really helpful. It was also confirmed that training in food hygiene, first aid, manual handling and safe handling of medication was refreshed on a regular basis. The inspector looked at the new procedures that have been implemented for recruitment and it appears to be working well. The new front sheet checklist now ensures that nothing is missed through the recruitment stages. It is further recommended that any gaps in the employment history, no matter how small, are identified, verified and documented. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35 Key standards 31,33 &38 were assessed and met at the previous inspection in September 2005. Residents’ finances are safeguarded. EVIDENCE: The inspector discussed the residents’ money management with the administrator and she manages the petty cash and handles any receipts and records of transactions. Most of the residents either manage their own money or family members assist. From the records seen, it was clear that the financial records are well maintained and stored safely. There is also a property book for those residents who wish their valuables to be kept safe and appropriate safe storage.
