Conditions of registration: 1. The total number of persons that may be accommodated at any one time must not exceed 23. 3rd March 2005 Date of last inspection Brief Description of the Service: St Andrews is a residential home providing care and support and accommodation to its residents. The home is registered for 23 residents over the age of 65. The home has 19 single rooms and two double rooms. Eleven bedrooms have the benefit of en suite facilities. The home is situated on the outskirts of Oxford city centre. The home is set back in its own grounds on a busy main route into Oxford. Local shopping and leisure facilities are available and the home has easy access to the city and beyond. The original home has been added to and extended. The garden has recently been landscaped and provides wheelchair access to all parts of the garden. The home is family run and has a homely atmosphere. This is an overview of what the inspector found during the inspection. The inspection took place on a weekday morning. The inspector had previously been supplied with feedback questionnaires from a range of residents, relatives and professional agencies associated with the home. Feedback in the main was positive and where less positive comment had been made the registered person undertook to review these situations. A tour of the building found evidence that residents are encouraged to have their own possessions around them and that privacy and dignity are being respected. Major improvements have been made to the house over the years. Most recently the grounds have been landscaped to provide a pleasant area where residents sit in the good weather. The area is also wheelchair accessible. No health and safety concerns were identified during the tour. One relative and two residents were spoken to on the day of the inspection, all expressed satisfaction with the service. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4,5. The home provides prospective residents and interested parties with sufficient information on which to make an informed decision. EVIDENCE: The inspector was shown the home’s brochure, which outlines what the home can and cannot provide. It was written in plain English and was easily understood. Each resident has a written contract/terms and conditions. The resident’s contract is signed by the resident or his/her advocate. The manager assesses all prospective residents prior to admission. Admission is dependent on a mutual agreement that the home can meet the resident’s needs. Prospective service users are encouraged to visit the home prior to admission. Friends and relatives are encouraged to maintain contact with residents and feedback from relatives indicated that they are made welcome when visiting

One resident spoken to visits up to 4 times a week and states she feels “very much a part of the home”. An individual care plan is drawn up stating what the resident’s care needs are and stating how the home will meet them. The home does not offer intermediate care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10,11. Residents’ health and social care needs are being met. Residents have regular access to mainstream medical practitioners. EVIDENCE: Evidence was found at the inspection by reading residents’ files and in talking to residents that individual health, personal and social care needs are being met. The home has appropriate policies and procedures to manage the storage, recording and distribution of medication. A random selection of residents’ medication was inspected and found to be satisfactory. The inspector spoke to two residents and received feedback from several others and was assured that residents are treated with respect and that their privacy is being respected. The inspector was of the opinion that a more comprehensive picture of the care being provided could be recorded in a way that truly reflects the holistic care being provided by the home. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15. The home endeavours to meet a wide range of expectation of its residents. Residents are encouraged to take as active a part in their lives as they are willing or able. EVIDENCE: Residents spoken to at the time of the inspection stated that they were happy with the activities provided and the opportunities to continue to be active in the community and with friends. On the day of the inspection a scheduled music session was planned to take place. The activities co-ordinator did not turn up. The session went ahead led by the staff on duty; a good time was had by all. The home has the use of its own minibus which is used for outings and transporting residents if required. There is a pleasant garden, which has plenty of shaded areas, which is very well used by residents in good weather. The local park provides added space for relaxation and reflection. The home has the benefit of a variety of communal areas, which are used for many of the home’s indoor activities. The inspector was present whilst the lunchtime meal was being prepared. The main meal is normally prepared by the registered manager. This is due to the fact that the home does not employ a cook. On the day of the inspection this role was taken by the deputy. The manager and the deputy are both trained

Residents were being consulted as to their choice of meal. Feedback to the inspector both from residents and relatives with regard to the food was positive. A selection of past menus inspected showed a good range of fresh foods being used with personal choice in evidence. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,17,18. The home has a formal complaints procedure. On the day of the inspection no formal complaints had been recorded. EVIDENCE: The size and management structure of the home ensures that concerns/complaints if any are dealt with at source and without delay. The home has a formal complaints procedure. No complaints have been logged for some time. Residents spoken to and feedback from questionnaires stated that if they had a complaint they would go to the proprietor or Manager and felt confident that it would be resolved. Evidence was found through reading case files and speaking to residents that their legal rights are being protected and that they are protected from abuse. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,23,24,25,26. The home is fit for purpose and is well maintained. The home provides a safe environment for its residents to live in. EVIDENCE: With the addition of extensions and the conservatory the home has many twists and turns in its layout. The positive side of this layout is it is able to provide private areas for residents to be separated from the main body of the home if they so wish. The inspector was shown around the home and with residents’ permission was shown their bedrooms. All bedrooms were individually styled with residents’ personal possessions much in evidence. The home has sufficient toilet and bathroom facilities. The home has sufficient communal areas, which can provide for entertainment or for peace and quiet. The home has appropriate equipment to assist staff in the safe moving and handling of residents. The home has many residents with continence problems; this is very well managed, on the day of the inspection the home smelt clean and fresh.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30. Evidence was found at inspection and through residents’ feedback that staff are provided in sufficient numbers to meet the needs of the residents. EVIDENCE: The home employs sufficient permanent care staff to meet the needs of its residents. A random selection of staff files was inspected and found to be satisfactory. Staff spoken to have been supplied with job descriptions and were clear of the expectation of the role. Evidence was found that staff are being appropriately trained. Many of the home’s staff have been there for a number of years, which is a testament to the fact that staff enjoy working in the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,34,35,36,37,38. The home is professionally run and managed. Documentation for the safe and secure running of the home is available for inspection and was found to be satisfactory. EVIDENCE: The registered manager is experienced and trained to manage the care provision offered by the home. The registered manager is supported to do this by a staff group of sufficient numbers and experience in the care of the elderly. Due to the size and nature of the home supervision and consultation has been taking place informally. Staff must be provided with the minimum formal and recorded supervision sessions. There is much evidence of the proprietor’s involvement in the home to support the Manager. The home has sufficient policies and procedures to assist in the protection of potentially vulnerable adults. The home is managed in a professional manner and the inspector was
