Conditions of registration: 1) LD(E) is restricted to one person whose date of birth is 20/10/1934 2) Admissions are restricted to those Service Users whose category is DE(E) Date of last inspection 15/03/05 Brief Description of the Service: St Brelade’s is a care home which will eventually provide personal care and accommodation for 37 older people with dementia. The home is currently changing from a mixture of older people and older people with dementia. The home only provides care for women. It is owned by St Brelade’s Retirement Homes Limited who also own another home close by. The home is located on the seafront and close to Herne Bay town centre with all of its amenities and facilities. The home was opened in 1991 and consists of two large houses joined together. Thirteen of the single rooms have en suite toilet facilities and so do seven of the shared rooms. There is a patio area to the front of the house and gardens to the rear. This is an overview of what the inspector found during the inspection. This was an unannounced inspection. Some records were inspected and a tour of parts of the building was made. Some staff spoke to the inspector and observations were made of interactions between staff and residents. What the service does well: What has improved since the last inspection? Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 and 10 Residents care needs are set out in individual plans which reflect that needs are fully met. The homes policies and procedures relating to medication ensure that residents are protected. Residents are treated with dignity and privacy. EVIDENCE: Four care plans were viewed during this inspection. They contain information about actions to be taken by staff to provide appropriate care for individuals. They are based on pre-admission assessments. They contain risk assessments and are regularly reviewed to ensure they are up to date. They cover a range of health and welfare care needs. Records are maintained to show the involvement of health care professionals such as dentist, optician, chiropodist and community nurses. The Home retains the services of a local general practitioner who visits weekly on a regular basis. There is a nutritional assessment tool in use which includes an assessment, as well as monthly comments were necessary. Risk assessments regarding skin integrity and mobility are also undertaken. Staff members who give out medications are either registered nurses or have undertaken a safe handling of medication training course. There are policies

Storage and record keeping is appropriate. The Inspector observed medication being given correctly and sensitively. A copy of the Royal Pharmaceutical Society of Great Britain’s guidelines is available to staff. The Manager and other staff confirmed that dignity and privacy are an important part of the care provided at St Brelade’s. The Mission Statement for the company emphasises this and each member of staff has a copy of this in their Orientation Folder which is part of their induction. Staff spoken to also confirmed that the ‘shadowing’ of an experienced member of staff during induction allows for these attitudes to be reinforced. Medical examinations would take place in the residents’ own room and personal care would be in own rooms or bathroom areas. There are 8 shared rooms, 7 of which have en suite toilets and all have screens for privacy when required. Some residents have their own phones and there is also a portable phone for residents to use which would allow for privacy. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 15 Residents benefit from good food and appropriate support from staff. EVIDENCE: The meal that was observed looked and smelt appetising. The food was nicely presented and there were three options on the day – minced beef or sausages with fresh vegetables and potatoes and vegetarian lasagne. Some people were using plate guards, some needed the full time help of staff and others needed some encouragement. Staff were observed to be sensitive and responsive. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20 and 24 Residents benefit from a well maintained environment which is safe and comfortable. EVIDENCE: The Home is located close to the centre of Herne Bay with all its amenities. The seafront is within easy walking distance. There are no outstanding requirements from the local Fire Safety Office or Environmental Health Department of the local council. The building is well maintained and staff are employed for maintenance and gardening. There is an area to the front of the building which overlooks the sea. Access to the garden area to the rear is limited to when staff can supervise. Opportunities to spend time outside and allow access to sunlight are provided. There is sufficient communal space at St Brelade’s which allows for different activities to take place. There is outside space which can be used by residents when supervised. Lighting in communal areas are domestic in character and furniture is attractive and good quality. Screening is provided in shared rooms. Furniture and fittings are provided based on risk assessments and residents’ wishes/needs. The person carrying out the health and safety risk assessments monitors what is in each room.

JUDGEMENT – we looked at outcomes for standard(s) 27 Residents’ needs are met by the number and skill mix of staff. EVIDENCE: A rota is maintained indicating who is on duty and what their role is. Domestic, catering and maintenance staff are employed as well as a structured team of care staff. There is also a structured team of senior staff to provide guidance and back up for the team of care staff. Training is an important part of supporting staff at St Brelade’s and this was confirmed by staff spoken to.
