Conditions of registration: Date of last inspection 23rd September 2005 Brief Description of the Service: 80 St James Road is a care home registered with the Commission for Social Care Inspection (CSCI) to provide care to three service users who have a learning disability. Only one service user is currently resident in the home. The Registered Providers also live in the home and their aim is very much to provide a family setting for care provision. The home is situated on the busy St James Road in Croydon; it is well served for bus routes to Croydon, Thornton Heath and Streatham and is within walking distance of Croydon town centre. The home is an ordinary domestic house, providing three single bedrooms. It has a kitchen, a dining room, a comfortable living room and a very pleasant garden. The bathroom and toilet are on the first floor. 24 hour staffing is provided by the Registered Providers. The weekly fee is £658. This is an overview of what the inspector found during the inspection. This was the home’s first inspection for the year 2006/07. It was an unannounced inspection and took place over two hours. Some times were spent looking at the policies and procedures, talking to the registered manager and to the service user. They are all thanked for their time and assistance. A tour of the building was also carried out. Requirements and recommendations from the previous inspection were also discussed. Overall the inspection confirmed that the home provides a good level of care for the service user who lives there. What the service does well: What has improved since the last inspection?

Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. All prospective service users have their needs assessed prior to admission to ensure that the home and staff are aware of their assessed needs. EVIDENCE: The manager indicated that service users are only admitted to the home after a full assessment of their needs has been carried out by the home and the Placing Authority for individuals referred through Care Management, involving the prospective service user/recognised representative. This covers all aspects of the person’s life, including strengths, social and cultural needs and psychological needs. The home then develops its own care plans from these assessments. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7 and 9 Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. Care plan is comprehensive and include detailed information about service user’s needs, personal goals, wishes and risk assessments. The service user is involved in decision making about his life and can take some risks so that he lives as normal a life as possible. EVIDENCE: The service user’s care plan was sampled and it identified a wide range of care and support needs. The plan was completed with the involvement of the service user and was last reviewed in March 2006. The right of service user to make decisions about his own life is central to the ethos of the home, support and guidance is given in all areas to ensure that he is making decisions, which are in his best interest. From discussion with the service user and manager no evidence of restrictions on service user was found.

Potential risks are identified covering all aspects of their daily living both inside and outside the home. The home was able to demonstrate that this standard was met as individualised care plan was in place for the service user that referred to action required to minimise identified risks and hazards. The home also has a comprehensive missing person’s policy, which gives staff clear guidance if a service user was to go missing. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16 and 17 Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. The service user is encouraged to explore opportunities to enhance his quality of life as well as maintain and participate with friends and the local community, with the aim of maximum integration. The daily routines and house rules promote the service user’s rights, and ensure equality and that all rights are enjoyed by the service user. EVIDENCE: The service user is supported to participate in domestic activities; he undertakes laundry, basic ironing, gardening and cleans his own room all with the Registered Provider’s support. He also attends church on a weekly basis. Evidence recorded in his care plan also indicated staff offer support, advice and other input that help to enhance and develop independent living skills.

Staff support him in pursuing these activities if and when necessary / requested. The service user attends clubs, groups and shopping independently in Croydon. The service user is actively encouraged to maintain links with his brother and friends. The home has an ‘open’ visitor’s policy and simply recommends that visitor’s telephone to say they are coming. Service user, who was at home at the time of this inspection, appeared to enjoy some level of independence. Routines can be very flexible and are well observed to take into account the service user’s individual needs. The current service user is fed by tube and is not able to consume any food or liquid orally. The dietician reviews his feed as and when necessary. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19 and 20 Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. The service user’s personal, physical and emotional health needs are being appropriately met and reviewed. This ensures that his physical and emotional health is well maintained and therefore the quality of life experienced is also maximised. EVIDENCE: Staff provide sensitive and flexible personal support to maximise the service user’s privacy, dignity, independence and control over his life. The manager stated that where needed, guidance and support is provided. Times for getting up/going to bed, baths, meals and other activities are flexible. The service user is registered with a local General Practitioner. Records checked indicate that GP’s and other community based medical/health care professionals are contacted on an as needed basis. The service user’s health is monitored and potential complications and problems are identified and dealt with at an early stage, including prompt referral to an appropriate specialist. The service user has recently been prescribed medication for a fungal infection. The medication administration record was audited and no gap in signatures was identified. JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23 Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. Complaints are generally managed well, which should ensure that service users’ and relatives’ concerns are listened to. Generally the home’s policies and procedures help protect service users from abuse and help staff if they need to tell someone about any bad care practice they may observe. EVIDENCE: The current complaints procedure is a good and gives clear step-by-step guide of how to make a complaint. There had been no official complaints at or since the last inspection. The home has a copy of the Croydon Adult Protection Procedure. The staff has had training on abuse. There have not been any adult protection concerns raised since the last inspection. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 and 30 Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. The home is generally hygienic and clean, homely and comfortable; this environment therefore facilitates the service users’ health and emotional wellbeing. EVIDENCE: The home is suitable for its stated purpose. It is accessible, meet residents’ individual and collective needs in a comfortable and homely way. The service user’s bedroom was personalised to reflect his individual needs, and personality. Overall the home was decorated to a good standard throughout and appeared to be very comfortable, bright and warm. The service user is planning to redecorate his room in the very near future. He is a Crystal Palace Football Club fan and looking to match the décor to his team colours. The home is kept very clean and hygienic and free from offensive odours throughout. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34,35 and 36 Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. This is a family run home. The registered providers both support each other and keep themselves up to date with training to meet the needs of the service user. EVIDENCE: Presently the home does not employ staff as there is only one service user. The registered providers have both attended a wide range of training courses to keep themselves up to date and to ensure the needs of the service user are being met. There is no one to one staff supervision being carried out at present, as the registered providers are the only staff. However it was previously recommended that the providers receive supervision from an independent person on a regular basis, which will contribute to the standards of care being provided. The registered manager stated that she is actively looking into this issue and will inform the Commission when a suitable person is found. Service users benefit from competent and accountable management of the service. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39 and 42 Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. The home is managed by an experienced and well-qualified manager who ensures a quality service. Generally the health, safety and welfare of the service users and staff were being promoted and protected. EVIDENCE: Throughout the course of the inspection the registered manager demonstrated a good competent management skills. She undertakes periodic training and development meeting Skills For Care specifications, to maintain and update her knowledge, skills and competence while managing the home. The home has a satisfaction questionnaire for service users and their relatives to gain feedback on the services being provided.

Records are maintained of fire drills and tests. Hazardous substances are appropriately stored. Gas, electric and fire checks have all been undertaken appropriately.
