Conditions of registration: none Date of last inspection 16 August 2004 Brief Description of the Service: 80 St James Road is a care home registered to provide care to two residents who have a learning disability. Only one resident is currently resident in the home. The Registered Providers also live in the home, their aim is very much to provide a family setting for care provision. The home is situated on the busy St James Road in Croydon; it is well served for bus routes to Croydon, Thornton Heath and Streatham and is within walking distance of Croydon town centre.The home is an ordinary domestic house, providing two single bedrooms. The home has a kitchen, a dining room and a comfortable living room. The bathroom and toilet are on the first floor; there is also a second toilet although this is currently only accessible from the outside of the home. This is an overview of what the inspector found during the inspection. This was the home first inspection for the year 2005/06.It was unannounced and took place over two hours. There is only one living in the home at present and unfortunately he was out during the course of the inspection. Inspection time was spent examining records, touring the building, and meeting with the registered providers. What the service does well: What has improved since the last inspection? It is recommended that the home review its Statement of Purpose when the application for variation has been approved. The home should include a copy of the latest inspection report in the Service User’s Guide. A recommendation is made for the home to have a written visiting policy in place. It is recommended that resident’s wish about not wanting to contact his mother be documented in his care plan.

The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,4 and 5 The Statement of Purpose, and Service User Guide provide prospective service users with details of the services the home offers. This enable them to make an informed decision about admission to the home .All prospective service users have their needs assessed prior to admission to ensure that the home and staff are aware of their assessed needs. EVIDENCE: The home has a Statement of Purpose that complies with the minimum standards. This is very comprehensive. However it requires amendment to reflect the change in regulatory organisation to the Commission For Social Care Inspection. The home has also applied for a variation to increase the home from a twobedded unit to three- bedded. The application is in progress. The home would have to review its Statement of Purpose when the application has been approved. The Service user’s Guide was comprehensive but did not contain a copy of the latest inspection report. The home must ensure that this is included in the Service User’s Guide.

The prospective resident is also involved in the process. This covers all aspects of the person’s life, including strengths, social and cultural needs and psychological needs. The home then develops its own care plans from these assessments. No residents have moved to the home since the last inspection. The resident has a contract between the home and the placing authority. This is in the process of being reviewed at present by the care manager. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,9 and 10 Care plans are comprehensive and include detailed information about service users’ needs, personal goals, wishes and risk assessments. Residents are involved in decision making about their lives, they participate and can take some risks so that they live as normal a life as possible. EVIDENCE: The resident care plan is detailed and identifies a wide range of care and support needs. The plan is completed with the involvement of the resident who also sign it. There are regular reviews where the resident’s needs and supports are discussed .The last review was in January of this year. The resident is fully able to make his wishes heard; there are no limits on his choice and freedom. He has an advocate who keeps in contact with him on a regular basis. The resident manages his own bank account and has limited assistance with the management of his weekly spending money. There was a recommendation made during the last inspection that the arrangements for the support of the

There was evidence that this has been documented. The resident’s care plan includes risk assessments that identify risks and hazards. The resident travels independently by public transport. The home has a ‘missing persons’ policy and procedure that clearly states what action staff is required to take in the event of an unplanned absence of a resident. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,13,15 and 17 Service users are encouraged to explore opportunities to enhance their quality of life as well as maintain and participate with friends and the local community, with the aim of maximum integration. The special dietary needs of the resident are being met with the assistance of the dietician. EVIDENCE: The resident is supported to participate in domestic activities; he undertakes laundry, basic ironing, gardening and cleans his own room all with the Registered Provider’s support. He also attends church on a weekly basis. The resident goes to work five mornings per week. He has a busy social life attending two weekly clubs and a fortnightly self-advocacy group. The resident is independent in his leisure activities attending clubs, groups and shopping independently in Croydon. The resident does not wish to contact his mother; it is recommended that this be recorded in his care plan. The current resident is fed by tube and is not able to consume any food or liquid orally. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,20 and 21 Residents’ personal care needs and physical and emotional health needs are met well by this home. This ensures that the residents’ physical and emotional health is well maintained and therefore the quality of life experienced is also maximised. The medication at this home is well managed promoting good health. EVIDENCE: The resident is independent in most areas of care, but still needs support and encouragement. The home has a medication policy and procedure. The current resident is not on any medication however he is sometimes prescribed antibiotics for chest infections. The Registered Provider has discussed with the resident about his wishes in the event of his death, he said he “does not want to know” and this information has been documented in his case notes. JUDGEMENT – we looked at outcomes for standard(s) 22 and 23 Arrangements for complaints and protection from abuse are well managed and ensure that service users feel listened to and safe. EVIDENCE: There had been no official complaints at or since the last inspection. The complaints procedure was clear and contained all of the elements required to meet Standard 22 including a minimum response time of less than 28 days. The home has a copy of the Croydon Adult Protection Procedure. The staff has had training on abuse. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24 and 30 80 St James Road is homely, clean and comfortable and promotes a family-like atmosphere that further enhances the service users emotional well-being. EVIDENCE: The home is a normal family home; it meets the needs of the service user. The home is clean and well presented. The home is well decorated and furnished. The home is within walking distance of West Croydon Station and Croydon’s shopping centre and facilities. The home was clean and hygienic and free from offensive odours throughout on the day of the inspection. JUDGEMENT – we looked at outcomes for standard(s) 33,34and 35 This is a family run home. The registered providers both support each other and keep themselves up to date with training to meet the needs of the resident. EVIDENCE: Presently the home does not employ staff. The Registered Providers intend to employ staff when the variation has been approved for the home to have three residents. Both Registered Providers have had their CRB done. They have both attended a wide range of training courses to keep themselves up to date and to ensure the needs of the resident are being met. JUDGEMENT – we looked at outcomes for standard(s) 37 and 42 The home is generally managed well however, the system in place for keeping cleaning materials is poor and potentially place service users and staff at risk. EVIDENCE: One of the Registered Provider is doing her NVQ Level 4 through Bromley College and hoping to complete the course in June or July of this year. The home has domestic arrangements for fire prevention. The smoke detectors are checked on a weekly basis. Records are maintained of fire drills and tests. The home must ensure that the COSHH cupboard is kept locked at all times when not in use.
