Service users in the categories DE and MD are not to be admitted under the age of 60 years 13th June 2005 Date of last inspection Brief Description of the Service: The home provides a service for up to fifteen older people (over the age of 60) who may also have dementia or a mental disorder. There are 11 single bedrooms and 2 shared bedrooms (located on the ground and first floors). Communal areas comprise of the lounge, conservatory and dining room. There is a garden at the side/rear. Car parking is available at the front. Local amenities (including access to the beach) are nearby. This is an overview of what the inspector found during the inspection. The inspection (unannounced, between circa 10.30 am and 15.00 pm) consisted of meeting with the owners, members of staff, visitors and residents. Some of the home’s records were seen (care plan records, medication records, risk assessment documents). All bedrooms and communal areas were visited. On 20th December 2005, a further visit was made to discuss the home’s service user’s guide, complaints procedure and AP (adult protection) procedures. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3. Prospective service users receive an assessment to make it as likely as possible, that their subsequent residence would be long-term and successful. EVIDENCE: Mr and Mrs Bartlett explained how they are reluctant to accept prospective service users if they feel unable to care fully for them and meet each of their assessed needs. Both owners have an NHS background and, during discussions about profiles of most service users, they demonstrated how thorough their initial assessments are. Many service users are very elderly and have lived at the home for some years. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9. Service users receive good health and social care support. EVIDENCE: All service users were met during the inspection visit: care plan records seen included information about their care and support needs and how these are being met. The profiles of a number of service users were discussed. The disposition of service users is observed and recorded in a number of ways (including through the use of Barthel scores). Examples of intervention by GP’s and district nurses were discussed. Particular cases were discussed where medical attention was needed and received. The owners and members of staff have carried out risk assessments to identify potential vulnerabilities of each service user. Residents have access to other healthcare specialists (dentist, optician, chiropodist, psycho-geriatrician). Part of a medication “round” was seen [tablets removed from BOOTS dosage container, placed in lidded containers, taken to service users (who each

The medication fridge is to be replaced soon because of malfunction (some medication was temporarily stored in the domestic fridge, with recorded risk assessment to cover this period). It was agreed that a CSCI pharmacy inspector would be asked to assess the complete medication system as part of the home’s commitment to quality assurance measures. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13, 14. Service users are able to have visitors at any reasonable time and each, depending on capacity, has control over their lives. EVIDENCE: Visitors met during the inspection visit spoke well of the standard of services at the home. Those met visit very often and have a good knowledge of the home. At the beginning of the inspection visit, many service users were involved in activities with assistance or supervision by members of staff. The home was well decorated for Christmas. Some Christmas items were being made by residents: the finished product was seen later in the day. There were newspapers, magazines and books available as well as a TV/DVD. Some residents stated that, at their advanced ages, they prefer to spend time in their bedrooms. However, most were in communal areas. The practice of locking bedroom doors on the 1st floor (due to the owner’s attempts to limit incursions by a service user who goes into other people’s bedrooms) was discussed. Other service users are escorted on request to bedrooms when a member of staff unlocks the door.

The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Members of staff receive good information about adult protection procedures and this is a reasonable safeguard for the continuing safety of service users. EVIDENCE: Mrs Bartlett demonstrated the way (including through the use of charts for training) she instructs members of staff on the topic of adult protection. They are aware of the types of abuse that might occur, what to do if they identify or suspect poor practice and who to turn to if they have concerns. The owners agreed to prepare and prominently display a copy of a simple complaints procedure that invited service users and their supporters to feel confident in commenting about how they felt the home was being run (a copy would also be placed as an appendix in the revised service user’s guide). Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 26. The premises are suitable for the care and accommodation of older people. EVIDENCE: There are 6 single bedrooms and 1 shared bedroom on the ground floor and 5 single 1 shared on the 1st floor (accessed by stairs and stair-lift). The shared bedrooms are suitable for purpose by being well equipped (including ceilingglide curtains). Solar heating panels are being fitted. There are 3 bathrooms, two on the 1st floor 1 on the ground floor. Most use is made of the ground floor bathroom which is to be renovated soon and a new manual hoist installed. The home has a number of wheelchairs and a mobile hoist that are stored in the front hall when not in use. The garden is easily accessible by frail older people. Bedrooms are personalised with resident’s possessions.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for the following standard(s): 27 There is a staff group that supports the needs of residents and running of the home. EVIDENCE: Two members of staff are on duty at night [one awake and one asleep (between 11.00 pm and 08.00 am)]. The owners share aspects of management and care work between them. There is a deputy manager to support them. Two to three members of staff are on duty during the day, in addition to a cook and cleaner. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 37. The home is well established and service users interests are protected. EVIDENCE: Both owners have an NHS background: Mrs Bartlett was a nurse (RGN) and Mr Bartlett was a microbiologist (BSc & PhD). They take an active part in running the home. Mrs Bartlett has achieved the Registered Manager’s Award. The deputy manager is working towards this award. Profiles of service users discussed during the inspection visit, in particular those relating to some relatively seriously ill persons, indicated that good healthcare and personal support is provided.
