Care Homes for Older People Page 3 of 8 What we found:
We visited the home to look at the care records of two people who had sustained fractures whilst receiving personal care by staff. People had appropriate care plans giving staff instructions on how their needs should be met. A moving and handling risk assessment was available for both people whose care records we looked at. The moving and handling risk assessment gave information how staff should move the person in a variety of situations such as up the bed, from bed to chair and into the bath. One person needed to be hoisted yet there was no information on the type of hoist and size of sling that should be used. The moving and handling risk assessment did not give information on either persons limitations of mobility or general movement. One incident occurred when just one member of staff was washing and dressing the person, although care records identified a need for two members of staff to provide care. The other incident occurred when two care staff were moving the person out of bed. Care records did specify a need for two people to move the person. There are appropriate records of both accidents and incidents. The home had provided required information to us. Referral had been made to the Health and Safety Executive for one incident only, although referral should have been made for both incidents. No referral had been made to the local authority Safeguarding as should have been. We also found that care records were generally detailed and provided information about the care that had been given. We did find one inaccurate entry that showed incorrectly that the person had been taken to the toilet before the incident, although this was not the situation. We spoke to the staff who made the entry, they confirmed that this person had not been taken to the toilet before the incident . We advised staff to ensure that only factual information is recorded. We found that the home are investigating both incidents and staff statements in relation to the incidents are available. A conclusion of the investigations will be made by the home when the Safeguarding investigation has been concluded. The homes Manager, proprietor and a member of night staff are all moving and handling trainers. The three senior members of staff have had required training and updates to enable them to train and assess staff practice in moving and handling. Staff files that we looked at had regularly had moving and handling training. What the care home does well: What they could do better:
The home was advised to include additional information in peoples moving and Care Homes for Older People
Page 4 of 8 handling risk assessments. Problems that people have moving, the type of hoist and type and size of the sling that is to be used should all be recorded. If this information is available it will give greater assurance that people are moved both appropriately and safely. Referrals must be undertaken to the Local Authority of all incidents and accidents that are represented under safeguarding procedures. This means that an outside agency investigation may be undertaken to review the incident and actions can be undertaken to give assurance that people are kept safe at the home. Records need to be inaccurate and reflect the care given to enable a true and accurate picture of the care that people receive. If you want to know what action the person responsible for this care home is taking following this report, you can contact them using the details set out on page 2.
