Conditions of registration: One person whose name was made known to the Commission in August 2004,aged under 65 years of age, who requires care by reason of disability. Date of last inspection 6/10/2004 Brief Description of the Service: Stradbroke Court is a purpose built home for older people, which was refurbished a few years ago to a high standard. It currently provides 13 residential care places, 10 special needs places and 12 short-term care and rehabilitation places. The accommodation is all located on the ground floor and is divided into separate units. A day service is also provided in the building. This is an overview of what the inspector found during the inspection. This was an unannounced inspection, which followed an announced inspection in October 2004. The acting manager was present, although she was due to be moved to another County Council Home shortly to provide maternity leave cover. All 3 units were visited. 6 residents spoke to the inspector and all of them were very satisfied with their care. One person on the rehab unit had been anxious on arrival but now felt confident and ‘I’m looking forward to going home’. Another person on the same unit thought it was so good ‘I don’t want to go home.’ What the service does well: What has improved since the last inspection?
A new permanent manager has been appointed, and the last 2 team leader posts have been filled. This will give more stability to the staffing of the home, and ensure that the training, supervision and qualification programmes continue. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,4,6. The home has different levels of service to meet the needs of people for rehabilitation, dementia or residential care. Needs are carefully assessed and reviewed with the residents and their relatives to ensure that the care is appropriate. Strict guidelines for the use of the rehab unit ensure no one stays longer than the planned period with an agreed programme of rehabilitiation. EVIDENCE: The home was able to offer different levels of care for a variety of needs. There were proper admission assessment procedures leading to appropriate care planning for each unit. The Statement of Purpose had been updated for the new organisational structure and names of staff, the Business Plan and training plan, the short break brochure, and the East Point care pathway for hospital and community services. The Service User Guide met the standard. There were 9 residents on the rehab unit. They stay for 6 weeks to prepare them to return home usually with a support package. A planning meeting for one resident was taking place within the unit. The 3 residents in the dining

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9. Care plans are comprehensive and regularly reviewed to ensure residents’ current needs are being met, with appropriate use of outside professionals. The procedures for the administration of medicines, including for those who wish to self-medicate, ensured that residents were properly and safely medicated. EVIDENCE: Individual plans of care were drawn up on admission and regularly reviewed. The plans included appropriate risk assessments. Care information was very comprehensive, but there was a useful summary sheet prepared by keyworkers in the form of spider diagrams. Daily record sheets were comprehensive and provided clear information. Relatives were always invited to be present at reviews. Plans for those admitted to the rehabilitation unit were prepared in a different format after a multi-disciplinary planning meeting with the user. One resident with dementia was in hospital for re-assessment following some incidents of aggressive behaviour. Proposals on how to reduce or eliminate these incidents were being discussed by the staff. Drugs were properly stored on each unit. The Administration charts were fully completed and up-to-date.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15. The home arranged many of the aspects of daily life around the residents’ wishes, to give them maximum independence and influence over their lifestyle. EVIDENCE: The home provided a variety of activities which the manager was hoping to expand to give more choice, as well as the activities in the day centre. There was a well equipped hobbies room. A digital camera was going to be bought for starting an activities file for residents. Photos on noticeboards in the units recorded special events. There was a reminiscence room with examples of everyday life over the past century. One new resident had already noted this room and liked it. Residents who wished to hold their own money were helped to do so and had secure facilities for storage. There were dining areas on each unit with a varied menu. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17,18 The low level of complaints reflects the home’s standards and its good relationships with residents and their families. Residents have protection of their rights and are protected by policies, training and supervision from abuse. EVIDENCE: 2 complaints were being investigated. The manager described them and explained that one had been upheld after an internal investigation, and the other was still being investigated. There was a list of those residents in the office who were eligible to vote through the post. Abuse policy and training was organised through the County Council. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) ALL. This purpose-built home maintains good environmental standards, providing all the required facilities for residents. EVIDENCE: The home was built to meet the differing requirements for residential, special needs and rehab care. The home is visited weekly by an occupational therapist who can assess the equipment needs of new residents. There are assisted bathrooms with various kinds of specialist equipment. The home had experience last year of dealing with infection and can seek professional guidance when necessary. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,30. The home is appropriately staffed in each unit for the needs of the residents. Staff have good training programmes which maintain their competence. EVIDENCE: There were no current staff vacancies. 2 team leaders had just started. There was an on-going NVQ programme which would shortly cover 50 of care staff. The training record lists all types of training and which staff have undertaken each one. The manager had a new training package on the Prevention of Abuse which all staff were scheduled to go through. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 34,35,37,38. Residents are safeguarded by the home being part of the Suffolk County Council Social Care Services, with a full range of care, financial and health and safety policies, procedures and audits. EVIDENCE: A new permanent manager had been appointed on the week of this inspection. Evidence on Standard 31 will be collected during the Commission’s registration process. The home follows the County Council’s accounting and financial budgeting and control procedures. Those residents who wish to look after their own financial affairs do so. Relatives or legal advisers take that responsibility otherwise. If residents wish to keep small amounts of cash with them for everyday needs, they are able to use secure facilities in their rooms.
