Conditions of registration: Adults aged 40 years and over Date of last inspection 25 January 2005 Brief Description of the Service: Sun Court is a detached property situated near the town centre of Sheringham. It provides accommodation on three floors for 32 elderly residents who are in need of nursing care. Most of the rooms are single with en suite accommodation. There is a small paved garden, which is accessible to residents. The Home is well equipped with adaptations and facilities and has good liaison with community health resources. This is an overview of what the inspector found during the inspection. This was an announced inspection that took place over four hours. A tour of the premises took place and staff and care records were inspected. Eight of the thirty residents were spoken to and four staff members. The providers/manager and deputy manager were present for the inspection process. What the service does well:
This is an exceptionally well run home that has a group of staff that has been employed at the home for a considerable length of time. The residents spoken with stated that they had a very good relationship with the staff and that they were very caring and respectful. The home promotes a sense of community and encourages all forms of social interaction. The home works extremely hard at maximising the health status of its residents and provides a most homely environment with a high calibre of nursing care. It is well equipped with facilities that might be required and if more equipment is needed then the home is very good at providing what is needed. The home provides an exceptional high standard of palliative care and ensures that staff are well trained and equipped to give this specialised care. The home is always very positive about meeting all regulatory activities and is exceptionally good at keeping up to date with current legislation; they also make certain that their policies and procedures are kept up to date and are in line with the current legislation. The home’s records are always maintained and are easily accessible for inspection purposes. Care plans are well augmented and are used to meet the adequately assessed needs of the residents. The provider/manager and deputy manager put a lot of effort into providing an open transparent atmosphere within the home.

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,9 The resident’s health, personal and social care needs are set out very clearly in individual plans of care; this planned care is formulated using an assessment process that involves all significant agencies. The residents are protected by the procedures and policies for the administration and handling of medication. EVIDENCE: A number of care plans were inspected including that of the person receiving palliative care. The care plans were very clear and contained prescribed care that is evaluated on a monthly basis or more if required: records are well documented of all changes in care after care needs have been assessed. The Liverpool care pathway documents were inspected, these were found to be most informative giving very clear guidelines for care and the control of pain for the terminally ill resident. The care documents also indicated the involvement of relatives and significant others including other health care professionals.

A random check of medication and controlled drugs tallied with related medication records. The policy and procedures for administering medication were examined and were found to be clear and precise with no room for misunderstanding. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,15 Social activities and links with family, friends and the local community are handled exceptionally well. Meals are managed well and the food provided is varied and nutritious. EVIDENCE: Many activities are organised and also take place within the home that gives stimulation and pleasure to those who wish to participate. Staff, and residents spoken to by the Inspector confirmed this. On questioning, the residents felt that all the resources provided by the home were adequate and met their expectations. The residents told the Inspector that they enjoyed living in the home and “were able to do as they liked”. They also said that they enjoyed the mealtimes and had lots of fun. One resident is encouraged to continue her violin playing whilst others regularly join in other activities. For example the local school were doing a project on war memories and a number of the residents contributed to this and then also visited the local theatre to share these experiences. Those residents needing help were given it in an empathetic way and with dignity. The residents informed the Inspector that they were always given a choice and if they disliked something the kitchen was “very obliging and would readily provide an alternative”. JUDGEMENT – we looked at outcomes for standard(s) 18 The home takes steps to ensure that the residents are protected from abuse. EVIDENCE: Very clear and easy to follow guidelines are in place for dealing with abuse. The provider and deputy manager have attended sessions on abuse that aided them to formulate the guidelines. Carers who have or who are undertaking NVQ training have chosen to cover abuse issues in one of their modules. Recruitment is robust and CRB checks are all up to date. The staff that were interviewed were very conversant with the policies and guidelines for dealing with abuse and stated that they would not hesitate to ‘blow the whistle’ if they suspected any form of abuse taking place. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,26 The residents live in a very well maintained safe environment both internally and externally. Sufficient facilities are provided to maximise the resident’s independence; their rooms suit their needs and there is provision for them to have their own possessions around them. The home is pleasant, homely and free from any unpleasant odours. Sun Court Nursing Home I55 S15687 Sun Court V222361 010605 Stage 4.doc Version 1.30 Page 15 EVIDENCE: The Inspector undertook a tour of the home and all the internal décor was found to be in very good order, it was both pleasing and comfortable. When asked, the residents felt that they had all the facilities they needed to meet their requirements and so maintain their independence. There are numerous assisted washing and bathing facilities also toilets to meet the assessed needs of the residents. It was noted by the inspector that there were a number of hoists and overhead hoists to enable residents to be moved safely. Although some of the rooms are small this aspect does not seem to have any detrimental effect on the care given; rooms are accommodated by persons who have been assessed and who it is felt can function in their allotted accommodation. The home has a very pleasing atmosphere and was very clean and tidy, the residents themselves commented on the cleanliness of the home and that this is always maintained to a high level. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,30 The residents benefit from staff who have the skills to care for them and meet their needs. EVIDENCE: Examination of the duty rosters and training records confirmed that there was a good skill mix amongst the staff and that they possessed the necessary expertise to carry out their jobs and meet the needs of those residents in the home. The qualified nurses quite often work along side the carers thus giving them the benefit of their knowledge and expertise. One of the nurses has had immense experience in nursing persons with neurological disorders and this is most beneficial to the rest of the team. This nurse also does in house training sessions for the staff; these sessions are geared towards helping the staff meet the needs of residents with relative conditions. NVQ training is an on going project and well within the targets set by the Commission. Some of the qualified staff are undertaking an assessing course, one member of staff is also furthering her expertise in palliative care so that the home can be a centre of excellence.
Sun Court Nursing Home I55 S15687 Sun Court V222361 010605 Stage 4.doc Version 1.30 Page 17 There is in place a staff development programme and this was seen by the Inspector and those staff spoken with confirmed that training was good and they felt that they had the skills to do their job. One staff member commented that the home was the best she had been in for ensuring that staff training was an on going process. Staff are given basic skills training once a year, this is used as a refresher period where basic training is re visited. Some of these sessions are used to identify areas that are needed in order to meet the needs of the residents already in the home. Quite often supervision sessions are used as a group activity and just recently was used as a teaching session in relation to the death of a young resident. There is a formal training plan and a Work Force development training plan for TOPS; these were seen. The staff spoken to commented on the training sessions and stated that the home was “very good at providing training for them”. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 31,32,37,38 It is very evident that the home benefits from the ethos of the leadership. Record keeping is excellent. Sun Court Nursing Home I55 S15687 Sun Court V222361 010605 Stage 4.doc Version 1.30 Page 19 EVIDENCE: Both the providers/manager have had extensive experience in running a care home and this is very evident. The deputy is being prepared for the position of manager and the providers/manger are mentoring her ready to take up this position. The deputy manager has successfully completed NVQ level 4 in Management. The Matron keeps up to date with current practice and this is evident in relation to innovations that take place within the home setting. On discussion with staff it was clear that there are clear lines of accountability within the home and that the safety and welfare of the residents are being promoted on a continuous basis. Discussion with staff members and the residents confirmed that there is an open and inclusive atmosphere and that all the staff and the residents benefit enormously from the ethos of the home and the persons managing it. All new staff are given information that sets out the philosophy of the home, policies and a development plan for the home. All staff have been given copies of the Code of Professional Conduct and this is often used for training sessions. Records are in place to confirm that the home carries out safe working practices. Staff spoken to also confirmed that they had had recent training in Fire, food hygiene and first aid. The inspector was impressed with the risk assessments that were in place and the manner in which they are recorded; these evidenced frequent checks of the environment thus ensuring that the residents lived in a safe home. All records of incidents and accidents are recorded and these tallied with the care plans and the risk assessments. All staff receive induction and foundation training that meet all the TOPS specification.
